تأثیر تمرینات ثبات مرکزی بر تعادل ایستا و درد زانوی سالمندان مبتلا به استئوآرتریت زانو
هدف: استئوآرتریت زانو یکی از شایعترین مشکلات عضلانی- اسکلتی و واروس زانو جزء شایعترین ناهنجاری اندام تحتانی در افراد مبتلا به استئوآرتریت زانو میباشد که میتواند روی فعالیت روزانه سالمندان، ایجاد درد و کنترل پاسچر تأثیرگذار باشد. هدف از پژوهش حاضر بررسی تأثیر تمرینات ثبات مرکزی بر تعادل و درد سالمندان مبتلا به واروس و استئوآرتریت زانو میباشد. روش بررسی: در این مطالعه نیمه تجربی، 24 بیمار زن مبتلا به ناهنجاری واروس و استئوآرتریت زانو در دامنه سنی50 تا 65 سال با میانگین سنی 55/62 به طور داوطلبانه آمادگی خود را جهت شرکت در پژوهش اعلام کردند و به صورت تصادفی در دو گروه تجربی و کنترل قرار گرفتند. میزان درد بیماران با مقیاس دیداری درد VAS (Visual analogue scale)و جهت سنجش تعادل از دستگاه تعادل سنج بایودکس (BIODEX) استفاده گردید. گروه تجربی به مدت 8 هفته، هر هفته سه جلسه، تمرینات ثبات دهنده را انجام دادند. جهت تجزیه و تحلیل آماری از آزمون تی استیودنت وابسته و مستقل استفاده گردید. یافتهها: تعادل ایستا و شدت درد در گروه تجربی پس از هشت هفته تمرین ثبات مرکزی تغییرات معنیداری را نسبت به پیش آزمون نشان داد (0/05>p). تغییرات بین گروهی برای میزان درد و تعادل ایستا نیز تفاوت معنیداری را نشان داد (0/05>p). نتیجهگیری: نتایج پژوهش نشان داد یک دوره تمرینات ثبات مرکزی توانسته بر تعادل ایستا و درد سالمندان مبتلا به واروس و استئوآرتریت زانو تأثیرگذار باشد. لذا میتوان از تمرینات ثبات مرکزی به عنوان یک روش به منظور کاهش درد در مفصل زانو و ایجاد ثبات و بهبود تعادل در بیماران سالمند مبتلا به واروس و استئوآرتریت زانو استفاده کرد.

ارزیابی حساسیت بینایی محیطی و مرکزی در نزدیک بینی خفیف و متوسط
هدف: بررسی همبستگی میزان نزدیکبینی و حساسیت بینایی محیطی و مرکزی در موارد خفیف و متوسط نزدیکبینی به کمک دستگاه پریمتری هامفری روش بررسی: ۲۹ فرد (۵۸ چشم) با میانگین عیب انکساری نزدیکبینی 1/2±2/98- تحت ارزیابی حساسیت بینایی محیطی با روش Peripheral 60-4 و توسط دستگاه پریمتری هامفری قرار گرفتند. محدوده مورد بررسی به سه حلقه 40-30، 50-40 و 60-50 درجه تقسیم شد و میانگین حساسیت هر حلقه مورد مقایسه قرار گرفت. همچنین حساسیت مرکزی (فووآیی) نیز ارزیابی گردید. یافتهها: در مقایسه نزدیکبینی خفیف (0/75- تا 3- دیوپتر) و متوسط ( 3- تا 6- دیوپتر) تفاوت معناداری در حساسیت بینایی در محدوده 40-30 درجه (0/42=p) در محدوده 50-40 درجه (0/45=p) و در محدوده 60-50 درجه (0/35=p) مشاهده نشد. اما بین مقدار نزدیکبینی و حساسیت بینایی مرکزی یک همبستگی منفی مشاهده شد (0/33- =r و 0/01=p) نتیجه گیری: عدم مشاهده تفاوت معنادار بین دو گروه نزدیکبینی خفیف و متوسط در حساسیت بینایی محیطی، مبین این موضوع است که نمیتوان پارامتر حساسیت بینایی محیطی را یک فاکتور متغیر در بررسیهای مرتبط با عیب انکساری نزدیک بینی محسوب نمود. نیاز است تا مطالعات بیشتر و در ابعاد گستردهتر در این رابطه صورت پذیرد.

بررسی قابلیتهای سیستم اطلاعات آزمایشگاه در دانشگاه علوم پزشکی مشهد -1392
هدف: یکی از زیر مجموعههای سیستم اطلاعات بیمارستانی، سیستم اطلاعات آزمایشگاه است که از آن برای درخواست آزمایشات، کمک به پردازش نمونهها، دریافت نتایج از آنالایزرها و تهیه و ارسال گزارشها استفاده میشود. علیرغم تمام مزایایی که این سیستمها دارند مشکلاتی نیز در پیادهسازی و بکارگیری آنها وجود دارد. لذا در این پژوهش به بررسی قابلیتهای سیستم اطلاعات آزمایشگاه بیمارستانهای آموزشی دانشگاه علوم پزشکی مشهد در سال 1392 پرداخته شده است. روش بررسی: مطالعه حاضر از نوع توصیفی – مقطعی بود که بر روی سیستم اطلاعات آزمایشگاه بیمارستانهای آموزشی دانشگاه علوم پزشکی مشهد در سال1392 انجام شد. در این مطالعه قابلیتهای ضروری سیستم اطلاعات آزمایشگاه از طریق بررسی متون، شاخصهای ارزیابی سیستم اطلاعات بیمارستانی وزارت بهداشت (نسخه 4/3) و بررسی نظرات کاربران نهایی تعیین گردید و سپس در بیمارستانهای تحت مطالعه بر اساس چک لیست محقق ساخته جمعآوری گردید. تجزیه و تحلیل دادهها نیز با استفاده از تعیین فراوانی و آمار توصیفی انجام پذیرفت. یافتهها: براساس ارزیابی انجام شده مشخص گردید که سیستم اطلاعات آزمایشگاه بیمارستانهای آموزشی دانشگاه علوم پزشکی مشهد در (58/8) موارد قابلیتهای مورد نظر را داراست. تقسیمبندی قابلیتها در سه دسته الف) فرآیند آزمایشگاهی ب)گزارشات مالی و ج) گزارشات مدیریتی نشان داد که سیستم اطلاعات آزمایشگاه مورد بررسی در بخش مربوط به فرآیند آزمایشگاهی 52/7 درصد در گزارشات مالی صد در صد و در بخش گزارشات مدیریتی 66/6 درصد از قابلیتهای مطلوب را دارا بود. نتیجه گیری: با توجه به یافتههای پژوهش حاضر قابلیتهای سیستم اطلاعات آزمایشگاهی بیمارستانهای دانشگاه علوم پزشکی مشهد در برخی موارد با وضعیت مطلوب فاصله دارد و باید مورد بازنگری قرار گیرد. لذا به منظور ارتقاء و افزایش کیفیت خدمات آزمایشگاهی بهتر است قابلیتهایی از جمله ارتباط سیستم اطلاعات آزمایشگاه با سایر سیستمهای اطلاعاتی، کنترل کیفیت آزمایش، اولویت قرار دادن گروههای خاص بیماران در سیستم لحاظ گردد.

بازشناسی همخوانهای خیشومی در نویز همهمه
هدف: برخی مداخلات محیطی مانند نویز همهمه میتوانند درک گفتار را دچار اشکال کنند. درک درست همخوانها پایه و اساس درک کلی گفتار است. معمولاً در محیطهایی ارتباط گفتاری برقرار میشود که چند گوینده در حال صحبت کردن هستند. 3 همخوان از همخوانهای زبان فارسی را همخوانهای خیشومی تشکیل میدهند که این همخوانها در درک گفتار نقش مهمی دارند. هدف ما از این مطالعه تعیین تأثیر نویز همهمه بر امتیاز بازشناسی همخوانهای خیشومی است. روش بررسی: این مطالعه توصیفی تحلیلی بر روی 48 مرد و زن به نسبت برابر و با شنوایی هنجار در محدوده سنی 24-19 سال از بین نمونههای در دسترس انجام شد. پس از انجام ارزیابیهای شنوایی و گفتاری، بازشناسی همخوانهای خیشومی در قالب هجای همخوان_واکه_همخوان و در حضور نویز همهمه در نسبتهای سیگنال به نویز صفر، 5- و 10- مورد آزمون قرار گرفت. یافتهها: با افزایش میزان نویز بازشناسی همخوانهای خیشومی در ابتدای هجا کاهش یافت. بازشناسی همخوانهای خیشومی در ابتدای هجا با واکه /a/ در نسبتهای سیگنال به نویز صفر (24/18 ± 87/5) در 5- 24/71 ± 80/2 )و در10- ( 35/66 ± 64/58 ) بود و تفاوت معناداری داشتند (0/0005>p)، اما بازشناسی این همخوانها در ابتدای هجا با واکههای/i/ ( 19/26±48/95) (0/341=p)، /e/ (33/42± 62/5) (p=0/057) ، /â/ (17/74± 47/91) (p=0/741) ،/o/ (28/69±2/30) (p=0/082) و /u/ (17/83± 7/29) (p=0/178) در نسبتهای سیگنال به نویز ذکر شده تفاوت معناداری نداشتند. در بازشناسی همخوانهای خیشومی انتهای هجا با واکه های /e/(24/73± 75/00) (p=0/008)، /a/ (24/09± 64/58) (p=0/002)، /â/ (24/32± 47/39) (p=0/006)، /i/(24/54± 79/68) و /o/ (28/67± 60/93) (p<0/0005) در نسبتهای سیگنال به نویز ذکر شده تفاوت معناداری وجود داشت. همچنین بازشناسی این همخوانها در ابتدای کلمه در نسبت سیگنال به نویز صفر در زنان و مردان تفاوت معناداری نشان داد (0/39=p) که امتیاز بازشناسی در زنان (23/21± 64/58) بیشتر از مردان (24/07± 58/33) بود. نتیجهگیری: افزایش میزان نویز، امتیاز بازشناسی همخوانهای خیشومی را کاهشمیدهد. بازشناسی همخوانهای خیشومی در حضور نویز همهمه فقط در نسبت سیگنال به نویز صفر، با جنسیت رابطه دارد.

مقایسه فشار کف پایی و نیروی عکس العمل زمین در کاراته کاران نخبه مرد و زن
هدف: کاراته ورزشی است با ماهیت تحمل وزن همراه با پرشهای گوناگون که این پرشها فشار مکانیکی زیادی را به اندام تحتانی ورزشکار وارد می کند. از این رو هدف از تحقیق حاضر، مقایسه فشار کف پایی و نیروی عکس العمل زمین در کاراتهکاران نخبه مرد و زن بود. روش بررسی: 11 کاراته کار نخبه مرد و 12 کاراته کار نخبه زن به صورت غیر تصادفی و هدفمند انتخاب شدند. فشار کفپایی و پارامترهای مرتبط با آن با استفاده از صفحه emed (دستگاه اندازه گیری فشار کف پایی)، در 5 گام اندازه گیری شد. پس از تقسیم کف پا به 10 ناحیه، حداکثر فشار، حداکثر نیرو، سطح تماس در هر ناحیه محاسبه شد. از آزمون شاپیرو-ویلک جهت آزمون نرمال بودن توزیع داده ها استفاده گردید؛ سپس برای مقایسه پارامترهایی که دارای توزیع نرمال بودند از آزمون تی وابسته و آزمون تی مستقل و برای متغیرهایی که دارای توزیع نرمال نبودند از آزمونهای ناپارامتری ویلکاکسون و یو من ویتنی در سطح معناداری (0/05p≤) استفاده گردید. شاخص تقارن نیز برای تعیین تقارن بین پاها بکار برده شد. یافته ها: نتایج این تحقیق، اختلاف معناداری در اوج فشار و سطح تماس و نیروی عکسالعمل زمین بین دو گروه و همچنین بین پاهای جلو و عقب گروهها نشان داد. نتایج شاخص تقارن نیز نشان داد که در اغلب ناحیه ها بین دو پای کاراتهکاران عدم تقارن وجود دارد. نتیجه گیری: تحقیق حاضر متغیر جنسیت را یک عامل مهم در نحوه توزیع فشار کاراته کاها معرفی می کند و این احتمال را می دهد که کاراته کاران نخبه اغلب از پای عقب خود به عنوان تحمل کننده وزن و از پای جلو جهت پیشروی و ضربه استفاده میکنند که چنین استفاده متفاوتی از دو پا در دراز مدت بر الگوی توزیع فشار کفپایی این افراد تأثیر خواهد گذاشت.

مدلسازی خطی و غیرخطی ماهیچههای چشم در بیماری استرابیسم
هدف: در جراحیهای اصلاح انحراف چشم، یکی از مشکلات اصلی که پس از عمل پیش میآید این است که چشم بیمار دچار اصلاح بیش از حد یا کمتر از حد میشود که این امر مربوط به ویژگیهای خاص ماهیچهی هر بیمار میشود. بنابراین داشتن اطلاعات دقیقی از ماهیچههای چشم و عملکرد آنها برای شبیهسازی و مدلسازی دقیقتر ضروری است هدف از این تحقیق ارائه یک مدل و کمک به دقیق تر شدن جراحی استرابیسم و تحلیل عملکرد ماهیچههای چشم میباشد. روش بررسی: مدل ارائه شده یک مدل سه جزئی مرکب از یک فنر و دمپر سری که با یک فنر دیگر موازی شده میباشد. پاسخ مدل سه جزئی خطی و مدل سه جزئی غیرخطی بر منحنیهای خزش بدست آمده از آزمایشات گذشته، منطبق شد و میزان انطباق آنها با هم بدست آمد. یافتهها: پاسخ مدلهای خطی و غیرخطی هر دو با میزان انطباق بسیار بالایی بر منحنیهای خزش بدست آمده از نتایج آزمایشگاهی منطبق شدند. نتیجه گیری: انطباق پاسخ مدلهای خطی و غیرخطی برنتایج میتواند بیانگر طبیعت خطی بودن ماهیچههای افقی چشم یا انجام آزمایشات در محدودهی خطی عملکرد ماهیچههای چشم باشد. اما یکی از علل پاسخ غیرخطی ماهیچه ها پس از عمل جراحی، مکانیزم پولی کامپلکس میباشد که باعث تغییر زاویهی خروج ماهیچه از کرهی چشم متناسب با کسینوس آن زاویه خواهد شد.

ارایه پارامترهای بهینه صندلی ماشین (پراید) جهت کاهش آسیب به مفصل زانوی راننده
هدف: رانندگان بدلیل استفاده مداوم از کلاچ و ترمز در حین رانندگی نیروی فشاری خارجی زیاد و متناوبی در زوایای مختلف فلکشن و اکستنشن زانو، بر روی مفصل زانو وارد میکنند. که این آسیبها شامل بروز درد کمر و زانو می باشد. طراحی و نوع صندلی خودرو در راحتی و سلامت سرنشین و جلوگیری از دردهای عضلانی،کمر و زانو به طور مستقیم ارتباط دارد. روش بررسی: در این پژوهش آزمایشهای مختلفی بر روی 4 راننده بر روی صندلی پراید با نصب سنسور اندازهگیری نیرو زیر پای راننده و روی پدال کلاچ و ترمز، نیروی تماسی پدال ماشین و کف پای راننده اندازه گیری شده است. همزمان با دوربینهای سرعت بالا اطلاعات سینماتیکی حرکتی اندام تحتانی طی مراحل گرفتن کلاچ ثبت شده و به عنوان ورودی به مدل عضو تحتانی شبیهسازی شده توسط جعبه ابزار SimMechanics در محیط متلب اعمال شده و گشتاور مفصل زانو حین عمل پدالگیری به عنوان خروجی بدست می آید سپس با تغییر در پارامترهای صندلی از طریق کدنویسی، بهینه ترین حالت برای صندلی که در حین پدالگیری کمترین گشتاور به مفصل زانو وارد شود استخراج می شود. یافتهها: با استفاده از مکانیزمهای پدالگیری و بهینهسازی پارامترهای صندلی این نتیجه حاصل می شود که اگر ارتفاع صندلی پراید 6 سانتیمتر و فاصله افقی پاشنه تا نشیمنگاه 9 سانتیمتر قابلیت جابجایی یابد میتوان گشتاور وارده بر مفصل زانو را در حدود 10-15 در صد کاهش داد. نتیجهگیری: با توجه به اینکه صندلی پراید امکان حرکت به جلو و عقب یعنی تغییر فاصله افقی نشیمنگاه تا پدال را دارد میتوان با اضافه کردن اهرمی که 6 سانتیمتر قابلیت جابجای ارتفاع نشیمنگاهی را بدهد شرایط بهتری را در رانندگی با این خودرو فراهم کرد. روش ارائه شده با توجه به راهکارهای در نظر گرفته شده در مطالعات قبلی دارای مزیتهای بسیاری میباشد. سادگی مدل بیان شده، سرعت بالای محاسبات و همچنین در محاسبات آینده با اعمال ضریب فنر و دمپر، می توان نقش ماهیچه ها را اعمال نمود.

بررسی زیرساخت های لازم برای پیاده سازی فناوری پزشکی از راه دور در بیمارستان های وابسته به دانشگاه علوم پزشکی زابل
هدف: امروزه با توجه به اهمیت ارائه مراقبتهای سلامت در مناطق دور افتاده ایجاد بسترهایی برای همکاری، آموزش و مشاوره از راه دور برای بیمارستانها امری ضروری است. در پژوهش حاضر، زیرساختهای لازم برای پیادهسازی فناوری پزشکی از راه دور در بیمارستانهای وابسته به دانشگاه علوم پزشکی زابل در سال 1393 مورد بررسی قرار گرفته است. روش بررسی: پژوهش حاضر از توع توصیفی - تحلیلی بود. جامعه پژوهش را ریاست، کلیه پزشکان فوق تخصص و مسئولان واحد فناوری اطلاعات بیمارستانها تشکیل میدادند و حجم نمونه با حجم جامعه برابر بود. ابزار جمعآوری داده پرسشنامه استانداردی بود که روایی آن توسط اساتید و متخصصین این رشته و همچنین پایایی آن با استفاده از ضریب آلفای کرونباخ (87%) مورد بررسی گرفت. از نرمافزار آماری SPSS برای تحلیل دادهها استفاده گردید. یافتهها: بر اساس یافتههای پژوهش، مشکلات بیمه و بازپرداخت (89/1 درصد)، هزینههای اولیه زیرساخت (82/6 درصد) و کمبود کادر فنی (80/4 درصد) بعنوان اساسیترین موانع زیرساختی برای فناوری پیادهسازی پزشکی از راه دور بود که برای غلیه بر این مشکلات، آموزش کارکنان، تامین هزینه های لازم برای زیرساختهای فنی، استخدام نیروهای فنی، ارائه مراقبت و انجام اعمال جراحی اولویتهای کاربرد فناوری پزشکی از راه دور معرفی شدند. نتیجهگیری: علیرغم وجود فرهنگ اجرائی مطلوب، بیمارستانهای مورد بررسی زیرساختهای لازم برای پیادهسازی فناوری پزشکی از راه دور را نداشته و قادر به ارائه خدمات پزشکی از راه دور نبودند.

اثر خستگی عملکردی بر میزان زاویه پشت پای افراد با ساختار پای طبیعی و چرخیده به خارج
هدف: هدف از این پژوهش، بررسی اثر خستگی ناشی از تمرین بر میزان زاویه پشت پای افراد با ساختار پای طبیعی و چرخیده به خارج طی فاز اتکای دویدن بود. روش بررسی: تعداد 24 نفر از دانشجویان پسر دانشگاه بیرجند (13 نفر با ساختار پای طبیعی، 11 نفر با ساختار پای چرخیده به خارج) بهصورت هدفمند و بر اساس شاخص افتادگی استخوان ناوی انتخاب، و پس از قرارگیری نشانگرهای مورد نظر، اطلاعات دو بعدی زاویه پشت پای آزمودنیها با سیستم آنالیز حرکت حین دویدن در سرعت 3/5 متر بر ثانیه برای مدت 15 دقیقه روی تردمیل در حالت بدون استفاده از کفش در دو وضعیت با و بدون خستگی جمعآوری شد. بهمنظور تعیین میزان خستگی از مقیاس میزان درک تلاش (Rating of Perceived Exertion) در حین دویدن استفاده شد. یافتهها: آزمونی تی وابسته نشان داد که بین میزان زاویه پشت پای افراد گروه با پای طبیعی در قبل از خستگی (شروع پروتکل تمرین) و حین خستگی (انتهای پروتکل تمرین)، بهجز در مرحله برخورد پاشنه با زمین (0/001=p)، در دیگر مراحل فاز اتکای دویدن اختلاف معنیداری وجود ندارد. با وجود این، بین میزان زاویه پشت پای افراد گروه با پای چرخیده به خارج در قبل و حین خستگی در کلیه مراحل فاز اتکای دویدن افزایش معنیداری مشاهده شد (0/05p≤). نتیجهگیری: بر اساس نتایج این پژوهش، خستگی عامل مؤثری در افزایش زاویه پشت پای افراد با پای چرخیده به خارج محسوب میشود. بنابراین، توصیه میشود ورزشکاران و دوندگان دچار این عارضه بهمنظور کاهش آسیبدیدگی به این فاکتور توجه ویژهای نمایند.

مروری بر مکانیسم حساسیت کانتراست رنگی و غیر رنگی
هدف: بررسی و مقایسه مکانیسم حساسیت کانتراست رنگی و غیر رنگی در سیستم بینایی میباشد. روش بررسی: کلیه مقالات چاپ شده در این زمینه از 1956 تا سال 2016 با استفاده از موتورهای جستجو PubMed ،Science Direct, Ovid و Google Scholar مورد بررسی دقیق قرارگرفت. تعداد 100 مقاله بدست آمد که با حذف مقالات تکراری (duplicates) و مقالاتی که در راستای موضوع تحقیق قرار نداشتند، در نهایت 40 مقاله مرتبط، مورد بررسی دقیق قرار گرفت. یافتهها: اکثر محققین بیان کردند که شکل منحنی عملکرد حساسیت کانتراست، برای حساسیت کانتراست غیر رنگی به صورت band-pass و برای حساسیت کانتراست رنگی به صورت low-passاست همچنین اکثر مطالعات نشان دادهاند، مکانیسم حساسیت کانتراست رنگی سبز و قرمز با هم مشابه و با مکانیسم رنگ آبی متفاوت است. نتیجهگیری: نتایج مطالعات مختلف نشان میدهد که حساسیت کانتراست رنگی و غیر رنگی از نظر شکل منحنی عملکرد حساسیت کانتراست و همچنین مکانیسم متفاوت هستند،که این امر با مدل چند کانالی سیستم بینایی قابل توضیح است.
