اثر کفی طبی قوس دار به همراه گوه خارجی بر گشتاور نزدیک کنندگی بخش داخلی زانوی بیماران مبتلا به استئوآرتریت
هدف: استئوآرتریت زانو شایع است و سالیانه تعداد زیادی از افراد را مبتلا میسازد. درمان استئوآرتریت زانو به دو شیوه درمان جراحی و غیر جراحی است. استفاده از زانو بند و کفیهای طبی در درمان غیر جراحی رایج است. هدف تحقیق حاضر ارزیابی تأثیر یک مدل کفی طبی قوس دار همراه با گوه خارجی بر متغیرهای کینتیکی و کینماتیکی مفصل زانو حین راه رفتن بود. روش بررسی: تعداد 10 آزمودنی با استئوآرتریت بخش داخلی زانو به صورت نمونهگیری غیر احتمالی در دسترس با دامنه سن 30 تا 60 سال، میانگین قد14/20±176/10 سانتیمتر و میانگین وزن6/3±75/5 کیلوگرم در این تحقیق نیمه تجربی شرکت کردند. جهت اندازهگیری متغیرهای کینتیکی و کینماتیکی راه رفتن از یک صفحهی نیروی Kistler و 7 دوربین آنالیز حرکت پرسرعت با فرکانس نمونهبرداری 100 هرتز استفاده شد. کلیه متغیرها حین راه رفتن در دو حالت با کفی قوس دار همراه با گوه خارجی و با پای برهنه ثبت گردید. یافتهها: میانگین سرعت راه رفتن با کفی طبی3/10±63/10 میلیمتر بر ثانیه بود که هیچگونه تفاوت معنیداری با راه رفتن با پای برهنه نداشت (0/42=p). حداکثر قله اول نیروی عمودی عکسالعمل سطح وارد شده به مفصل زانو 0/55N/BW به طور قابل توجهی حین استفاده از کفی افزایش پیدا کرد (0/03=p). گشتاور نزدیککنندگی(adductory) وارد شده به مفصل زانو حین راه رفتن با کفی 0/05± 0/45 نیوتنمتر بدست آمد که به طور معنیداری کمتر از راه رفتن بدون کفی بود (p>0/01). نتیجهگیری: کفی قوسدار همراه با گوه خارجی، گشتاور نزدیککنندگی وارده به مفصل زانو را کاهش داد. کاهش گشتاور نزدیککنندگی با کاهش نیروهای اعمال شده به بخش داخلی مفصل زانو همراه است. بنابراین میتوان گفت استفاده از کفی قوسدار، به خاطر کاهش گشتاور نزدیککنندگی، به کاهش درد و تسهیل راه رفتن بیماران مبتلا به استئوآرتریت میانجامد. پیشنهاد میشود متخصصان توانبخشی کفی حاضر را جهت کاهش درد زانو به بیماران مبتلا به استئوآرتریت تجویز کنند.

تغییرات رفرکتیو بعد از جایگذاری مایورینگ
هدف: بررسی تغییرات عیوب انکساری و حدت بینایی قرنیه قبل و شش ماه بعد از عمل جایگزاری مایورینگ در بیماران با قوز قرنیه در بیمارستان فارابی تهران، ایران. روش بررسی: این مطالعه گذشتهنگر پرونده 34 چشم 28 بیمار قوز قرنیهای با میانگین سن 7/41±29 که تحت عمل مایورینگ قرار گرفتند و بعد از 6 ماه ارزیابی مجدد شده بودند، مورد بررسی قرار میدهد. دراین مطالعه حدت بینایی دور اصلاح شده، نتایج انکساری و دادههای پنتاکم (اکولوس GmBh) ارزیابی شدند. درهمهی بیماران مرکز قرنیه شفاف بود و امکان تجویز یا تحمل لنز برای آنها وجود نداشت و ضخامت مرکزی قرنیه در همهی نمونهها بیشتر از 360 میکرومتر بود. یافتهها: شش ماه بعد از عمل جراحی میانگین حدت بینایی دور اصلاح شده (در سیستمLogMAR) به طور معنیداری از 0/30±0/22 به 0/20±0/20بهبود پیدا کرد (0/006=p) و میانگین عیب انکساری اسفریکال از 3/76 ±4/66- دیوپتر به 3/72±1/48- دیوپترکاهش پیدا کرد (0/001=p). میانگین عیب انکساری سیلندریکال از 3/15±4/27- دیوپتر به 1/63±2/18- دیوپتر کاهش پیدا کرد (0/001=p). درصد فراوانی محورهای سیلندریکال رفرکشن قبل از عمل به ترتیب 21%، 44% و 35% موافق قاعده، مایل و مخالف قاعده بودند و بعد از عمل به ترتیب 18% ، 24% و 58% محورهای موافق قاعده، مایل و مخالف قاعده شده اند. نتیجهگیری: عمل مایورینگ بهبود قابل توجهی در حدت بینایی دور اصلاح شده، عیب انکساری اسفریکال و سیلندریکال و میزان آستیگماتیسم قرنیه فراهم میکند.

اثر خستگی ناشی از دویدن بر الگوی Roll-Over پای دوندگان مبتدی

بررسی رابطه ی بین میزان آگاهی و سطح اضطراب قبل از عمل در بیماران کاندید عمل جراحی مغز و اعصاب
هدف: اضطراب از شایعترین مشکلات قبل از عمل جراحی است و با توجه به تکنیکهای پیشرفته جراحی، همچنان اکثر بیماران اضطراب قبل از عمل را تجربه مینمایند. این مطالعه با هدف تعیین رابطهی بین میزان آگاهی بیماران از روشها و عوارض عمل جراحی مغز و اعصاب و اضطراب قبل از عمل در بیمارستانهای آموزشی مشهد انجام شد. روش بررسی: این پژوهش از نوع توصیفی- تحلیلی بود که در سال 1393 انجام شد. جامعهی پژوهش کلیه بیماران مشمول عمل جراحی مغز و اعصاب با هوشیاری نسبی که هنوز عمل جراحی نشده و یا منتظر عمل جراحی بودند را تشکیل داد. بر اساس مطالعات قبلی و فرمول محاسبه حجم نمونه تعداد 10 درصد بیماران کاندید عمل جراحی مغز و اعصاب در 8 ماهه نخست سال 1393 (104بیمار) انتخاب شد. ابزار جمعآوری اطلاعات در قسمت بررسی میزان آگاهی بیماران از روش جراحی و عوارض عمل، پرسشنامه محقق ساخته و در قسمت بررسی شدت اضطراب بیماران از عمل جراحی، پرسشنامه استاندارد بک بود. تجزیه و تحلیل دادهها با استفاده از نرم افزار SPSS صورت گرفت. یافتهها: در خصوص اضطراب کل بیماران 3/8 درصد اضطراب بسیار کم و یا اضطراب نداشتند، 34/6 درصد اضطراب خفیف،43/3 درصد اضطراب متوسط و 18/3 درصد اضطراب شدید داشتند. متوسط اضطراب در بیماران 14/05± 19/16 بوده است. آشنایی بیماران از عوارض عمل جراحی و روش عمل در حد متوسط بود. افرادی که سابقه بستری در بیمارستان داشتند اضطراب کمتری نسبت به گروه دیگر داشتند (0/027=p). نتیجهگیری: با توجه به یافتهها، بیمارانی که با محیط آشنایی داشتند اضطراب کمتری احساس میکردند بنابراین ارائهی سطح مناسبی از اطلاعات و پاسخگویی به نیازهای اطلاعاتی بیماران و اطرافیان آنها هنگام اخذ رضایت آگاهانه و آشنایی بیمار با محیط و شرایط اتاق عمل میتواند در کاهش سطح اضطراب بیماران کاندید عمل جراحی مغز و اعصاب تأثیرگذار باشد. توجه به نیازهای اطلاعاتی این بیماران باید به طور مداوم مورد ارزیابی و پایش قرار گیرد.

مقایسه تغییرات مرکز فشار به مرکز جرم در حین مانور برش جانبی در مردان کاراتهکای نخبه با و بدون زانوی پرانتزی
هدف: با توجه به اینکه اختلال در وضعیت بدن، ثبات دینامیک را در حین انجام حرکات و مهارتهای ورزشی تحت تاثیر قرار می دهد هدف از انجام این مطالعه مقایسه تغییرات مرکز فشار نسبت به مرکز جرم حین برش جانبی در مردان کاراته کای نخبه با زانوی پرانتزی و نرمال بود. روش بررسی: در این مطالعه نیمه تجربی، تغییرات مرکز فشار نسبت به مرکز جرم بدن حین برش جانبی20 مرد کاراته کای نخبه (10 نفر زانو پرانتزی و 10 نفر نرمال) محاسبه شد. یافته ها: نتایج نشان داد، تغییرات مرکز فشار نسبت به مرکز جرم در جهت داخلی- خارجی در هر دو اندام غالب (0/024=p) و غیرغالب (0/004=p) در گروه زانوی پرانتزی به طور معناداری از گروه نرمال بیشتر بود. در حالی که در جهت قدامی- خلفی تفاوت معناداری بین دو گروه مشاهده نشد (0/05≤p). ضمن اینکه تفاوت بین اندامهای غالب و غیرغالب در هیچکدام از مولفه های قدامی- خلفی و داخلی- خارجی معنادار نبود (0/05≤p). نتیجه گیری: با توجه به نتایج مطالعه حاضر می توان چنین نتیجه گیری کرد که افزایش واروس زانو می تواند عاملی تاثیرگذار بر تغییرات مرکز فشار نسبت به مرکز جرم حین انجام مانورهای برشی باشد.

مقایسه شاخصهای آکوستیک صوتی افراد سیگاری و غیر سیگاری
هدف: مصرف سیگار به عنوان یکی از مهمترین عوامل موثر در ایجاد ضایعات حنجره و اختلالات صوتی شناخته میشود. حنجره و صوت گفتار را قابل شنیدن میکنند. ارتباط گفتاری نقش مهم و غیرقابل انکاری در زندگی انسان دارد. بررسی تاثیر سیگار بر وضعیت صوت سیگاریها از منظر پیشگیری، درمان و توانبخشی ضرورت دارد. روش بررسی: مطالعه حاضر از نوع مقطعی و بر روی 45 فرد سیگاری مراجعه کننده به مرکز ترک سیگار دانشگاه علوم پزشکی شهید بهشتی (34 مذکر و 11 مونث) با میانگین سنی (9/7±38/4) و 32 نفر غیرسیگاری (19 مذکرو 13 مونث) بامیانگین سنی (7/8±28/43) انجام شد. جهت تجزیه وتحلیل شاخصهای آکوستیک واکه /a/ به مدت 5 ثانیه تولید و با نرم افزار MDVP فرکانس پایه و تغییرات آن، جیتر، شیمر، تغییرات دامنه شدت صوت و نسبت نویز به هارمونی مورد بررسی قرار گرفت. یافتهها: نتایج نشان داد که شاخصهایF0،Fmin در افراد سیگاری کاهش یافته و با افراد غیرسیگاری تفاوت معناداری دارد (0/05>p). مقدارNHR، میانگینvAm ، میانگینAtri ، میانگین vF0 در افراد سیگاری به طور معنیداری بالاتر از افراد غیرسیگاری است (0/05>p). همچنین بررسی ارتباط تعداد نخ سیگار مصرفی در روز نشان داد که این عامل با شاخصهایF0 (0/010=p، 0/374-=r) و Fmin (0/004=p، 0/423-=r) و همبستگی معناداری دارد اما با سایر شاخصهای اکوستیک صوتی همبستگی معناداری ندارد. نتیجهگیری: شاخصهای آکوستیکی صوت در افرادی که مدت طولانی سیگار مصرف میکردند به طور معناداری نشاندهنده اختلال در عملکرد حنجره و صوت آنها میباشد. مصرف سیگار به مقدار 15 نخ در روز و مدت زمان بیشتر از 10 سال میتواند تاثیرات مشخصی را بر روی حنجره و در نتیجه شاخصهای صوتی گذاشته و منجر به اختلال در صوت شود.

اثر تعدیل اطلاعات آوران حسی (بینایی، دهلیزی، حس عمقی) بر تعادل افراد قطع عضو زیر زانو
هدف: کاهش در یکپارچگی حسی، افت عملکرد گیرندههای حسی عمقی، بینایی و دهلیزی از عوامل مهمی هستند که منجر به کاهش تعادل میشوند. هدف تحقیق حاضر برسی تأثیر دستکاری سیستمهای بینایی، حس عمقی، و دهلیزی بر تعادل افراد قطع عضو اندام تحتانی میباشد. روش بررسی: در این مطالعه مقطعی، 23 فرد قطع عضو اندام تحتانی شرکت کردند. جهت ارزیابی کنترل پاسچر و تعادل آزمودنیها از آزمون سازماندهی حسی دستگاه پاسچروگرافی استفاده شد. این دستگاه سطح تعادل را در شش وضعیت اول (وجود هر سه حس بینایی، دهلیزی و حس عمقی)، دوم (حذف حس بینایی)، سوم (حذف حس دهلیزی)، چهارم (حذف حس عمقی)، پنجم (حذف بینایی و حس عمقی) و ششم (حذف هر سه حس بینایی، دهلیزی و حس عمقی) ارزیابی نمود. آزمون سازماندهی حسی نمره تعادل را بر اساس متغیرهای پایداری و جابجایی مرکز کنترل ثقل تعیین نمود. یافته ها: نتایج تحلیل واریانس با اندازههای تکراری نشان داد افراد آمپوته اندام تحتانی در وضعیت دوم به طور معنیدار بر اساس نمره پایداری، تعادل بهتری نسبت به وضعیتهای چهارم تا ششم داشتند. افراد آمپوته اندام تحتانی در وضعیت سوم تعادل بهتری نسبت به وضعیتهای چهارم و پنجم داشتند (0/05p<). همچنین افراد آمپوته در وضعیت اول بر اساس نمره جابهجایی مرکز ثقل، تعادل بهتری نسبت به پنج وضعیت دیگر داشتند (0/05p<). نتیجه گیری: با حذف دو یا هر سه اطلاعات آوران حسی تعادل افراد قطع عضو نیز کاهش مییابد.

ارائه دو عامل آسیب رسان شنوایی، سطح آسیب را افزایش می دهد یا سبب محافظت می گردد؟
هدف: نویز و داروهای اتوتوکسیک از جمله مهمترین عوامل ایجاد کنندهی کمشنوایی دائمی میباشند. بطور معمول تصور بر این است که ارائه همزمان این عوامل آسیبرسان غالبا سطح آسیب شنوایی را به شکل غیر خطی بالا می برند، اما اگر روش ارائه این عوامل با دقت انتخاب شوند، نه تنها میتوان میزان آسیب را کنترل کرد بلکه حتی میتوان سطوحی از محافظت را نیز ایجاد کرد. این مطالعهی مروری به بررسی این موضوع می پردازد که در چه شرایطی ارائه دو عامل آسیبرسان در کنار هم میتواند سبب محافظت شود و در چه مواردی سطح آسیب را بیشترمی نماید. روش بررسی: در این مطالعهی مروری آخرین مقالات پیرامون آسیبهای شنوایی ناشی از نویز و داروهای اتوتوکسیک در بانکهای اطلاعاتی Science direct, Google scholar ,Springer, Magiran, Scopus ,Proquest و Pubmed و با استفاده از کلید واژههای مرتبط در فواصل سالهای 1960 تا 2015 جستجو شد که از بین 125 مقاله مرتبط بدست آمده، 59 مقاله براساس یک روش گزینشی هدفمند جهت برررسی انتخاب شدند. نتیجهگیری: ارائه عامل آسیبرسان شنوایی در مقادیر پایین و در بازه زمانی محدود میتواند دستگاههای حفاظتی درون سلول را فعال کند. این امر به نوبه خود مقاومت حلزون را در مقابل عوامل آسیبرسان افزایش میدهد. درک دقیق این بازه زمانی امکان ارائه مداخلات درمانی مناسب را فراهم خواهد ساخت.
