اثر استفاده از بریس بر طول تار عضلات تنه و کمر بیماران اسکولیوز ایدیوپاتیک حین راه رفتن
هدف: اسکولیوز ایدوپاتیک نوجوانی یک ناهنجاری ستون فقرات میباشد که در آن انحنای جانبی ستون مهرهها با چرخش مهرهها همراه است. بریس بر کاهش و کنترل پیشرفت انحنا اثر دارد با این وجود چگونگی تغییر طول تارهای عضلانی مشخص نیست. بنابراین هدف از تحقیق حاضر بررسی اثر استفاده از بریس بر طول تار عضلات تنه و کمر بیماران اسکولیوز ایدیوپاتیک حین راه رفتن بود. روش بررسی: تعداد 5 دختر مبتلا به اسکولیوز ایدیوپاتیک با انحنای C شکل (راس انحنای سه نفر به چپ و دو نفر به راست) با میانگین سن، قد و وزن به ترتیب1/16±12/68 سال، 0/08±1/53 متر و6/1±35/6 کیلوگرم به صورت نمونه در دسترس و 5 دختر سالم با دامنه قد و وزن و جنس مشابه در این مطالعه شرکت کردند. سیستم تحلیل حرکتی و یک صفحه نیروی جهت ثبت متغیرهای کینماتیکی (دامنه حرکتی مفاصل) و کینتیکی(قله گشتاور عضلانی، نیروهای تماس مفصلی و طول تار عضلانی) استفاده شدند. دادهها با استفاده از نرمافزار SPSS در سطح معنیداری0/05=α) تجزیه و تحلیل شد. یافتهها: تغییرات طول عضله اکسترنال ابلیک بیماران اسکولیوز 4 میلیمتر در سمت راست (0/02=α) و 18 میلیمتر در سمت چپ (0/01=α) کمتر از گروه سالم بود. دامنه حرکت مفصل ران بیماران اسکولیوز در صفحه عرضی به مقدار 6/5 درجه از گروه سالم کمتر بود (0/01=α). چرخش لگن در بیماران اسکولیوز به مقدار 9/8 درجه درصفحه افقی از گروه سالم کمتر بود (0/04=α). نتیجهگیری: اکثر متغیرهای کینتیکی، کینماتیکی و نیروی تماس مفصلی بین افراداسکولیوزی و سالم و همچنین در سمت تحدب و تقعر تفاوت معنیدار نداشت که می تواند نشانگر آن باشد که بریس در کاهش دامنه حرکتی لگن و تنه بیماران اسکولیوز مفید است. بنابراین بریس در جلوگیری از پیشرفت بیماری، نامتقارن ساختن نیروی تماس مفصلی و طول تارهای عضلانی میتواند موثر باشد. با این حال توانبخشی در جهت افزایش طول تارهای عضله اکسترنال ابلیک سمت تحدب و تقعر بیمارانی که از بریس استفاده میکنند باید مد نظر متخصصین توانبخشی قرار گیرد.

بررسی عیوب انکساری و میدان بینایی در ناشنوایان
هدف: بررسی فراوانی عیوب انکساری و نقایص میدان بینایی در دانشآموزان دبیرستانی پسر ناشنوای شهر تهران روش بررسی: در این مطالعه مقطعی 64 نمونه از لحاظ میدان بینایی و عیوب انکساری مورد بررسی قرار گرفتند. همهی افراد ضریب هوشی بالای70 با تست وکسلر مخصوص اطفال، داشتند. نقایص ناشنوایی بر اساس شدت نقص و مدت زمانِ ایجادِ نقص شنوایی تقسیمبندی شدند. میدان بینایی با استفاده از دستگاه پریمتر اوتوماتیکHumphrey جهت اندازهگیری حساسیت فووه آ، میانگین انحراف و بررسی محدوده تست نیمه میدان گلوکومی مورد بررسی قرار گرفت. یافته ها: بررسی ما بر روی دانشآموزان ناشنوا نشانگر میزان بالای میدان بینایی غیر طبیعی در افراد ناشنوا نسبت به جامعه شنوا است. نتایج حاکی از این است که در 50 درصد آنها ناشنوایی عمیق وجود داشت. نقص مادرزادی و اکتسابی شنوایی به ترتیب در(10/9%)57 و 89/1%)7 نفر مشاهده شد. نقایص میدان بینایی بر اساس تست نیمه میدان گلوکومی، میانگین انحراف و حساسیت فووه آ به ترتیب 31/2%، 60/09% و 40/6% بود. هیچکدام از فاکتورهای غیرطبیعی نظیر حساسیت فووه آ، تست نیمه میدان گلوکومی و میانگین انحراف رابطهای با نوع نقص شنوایی وشدت آن در افراد ناشنوا نداشت. بیشترین فراوانی عیوب انکساری، در هایپروپیا آستیگماتیسم (37/5%)و کمترین میزان فراوانی نیز بیماران در افراد مایوپیا میباشد (9/4%). تنها 12/5 درصد از دانشجویان کم شنوا بهترین حدت بینایی دور تصحیح شده کمتر از 10/10 داشتند. نتیجه گیری: فراوانی بیشتر نقایص حساسیت فووه آ نسبت به سایر نقایص میدان بینایی در افراد ناشنوا، میتواند نشاندهنده فراوانی بیشتر نقایص در مرکز سیستم بینایی نسبت به محیط آنها باشد. که این نشان می دهد یک فرد ناشنوا نسبت به افراد شنوا ،دارای ریسک بیشتری برای ابنرمالیتیهای میدان بینایی میباشد.

هدف: غربالگری فرایندی برای جداسازی افراد دارای ناهنجاری از جمعیت افراد طبیعی است. تحقیقات مختلفی برای بررسی وضعیت شنوایی کودکان سن مدرسه انجام شده است که موارد متفاوتی از کم شنوایی و ناهنجاری گوش میانی را نشان میدهد. در این تحقیق با توجه به تاثیرات کم شنوایی و ضایعات گوش میانی در سنین کودکی، روشهای مختلف غربالگری مورد بررسی قرار گرفت. روش بررسی: 1160 کودک ( نمونه 2320 گوش) 7 تا 11 سال در محیط مدرسه تحت ارزیابیهای غربالگری شنوایی با آزمونهای تاریخچهگیری، اتوسکوپی و ادیومتری قرار گرفتند و از این تعداد برای 269 نفر (538 گوش) آزمون تمپانومتری نیز انجام شد. یافته ها: میانگین سنی کودکان به ترتیب1/26±8/69 سال بدست آمد. نتایج تاریخچهگیری و ادیومتری نشاندهنده وضعیت طبیعی در اکثر گوشها (99% موارد) بود اما یافتههای غیرطبیعی بیشتری (5 تا10درصد) در آزمونهای اتوسکوپی و تمپانومتری مشاهده شد که نشاندهنده ضایعات گوش میانی بیشتری در سنین کودکی بود. نتیجهگیری: نتایج این بررسی نشاندهنده میزان پایین کم شنوایی در کودکان مدرسهای به علت برنامههای غربالگری شنوایی نوزادن و بدو ورود به مدرسه و همچنین شیوع بیشتر ضایعات گوش میانی میباشد. استفاده ترکیبی از آزمونهای اتوسکوپی، تمپانومتری برای شناسایی ضایعات گوش میانی توصیه میگردد.

تأثیر آنی تمرینات تعادلی بر حس عمقی مفصل مچ پای بازیکنان فوتبال
هدف: اطلاعات حاصل از گیرندههای حس عمقی نقش مهمی در ثبات مفاصل و کنترل عصبی-عضلانی ایفا میکند. هدف از پژوهش حاضر بررسی تأثیر آنی تمرینات تعادلی بر حس عمقی مفصل مچ پای بازیکنان فوتبال لیگ دسته دوم کشور بود. روش بررسی: به منظور انجام این مطالعه نیمه تجربی، 28 بازیکن لیگ دسته دوم کشور به دو گروه 14 نفره تجربی و کنترل تقسیم شدند (هرگروه متشکل از 14 نفر بود). پس از 5 دقیقه گرم کردن، برنامه تمرینی با تخته تعادل و تخته لرزان به مدت 10 دقیقه توسط گروه تمرینی انجام شد. در پیشآزمون و پسآزمون از گونیامتر حس عمقی مفصل مچ پا برای اندازهگیری خطای حس عمقی در زوایای 10 و 15 درجه Inversion و Eversion استفاده شد. از آزمون تی زوجی و تی مستقل برای تجزیه و تحلیل دادهها استفاده گردید و نتایج در سطح معنی داری 0/05 مورد بررسی قرار گرفتند. یافتهها: با توجه به نتایج بدست آمده از تحقیق حاضر، اختلاف معنیداری بین دو گروه در کاهش خطای حس عمقی مفصل مچ پا در زوایای 10 و 15 درجه Eversion و 10 درجه Inversion وجود داشت. نتیجهگیری: مرینات تعادلی موجب بهبود حس عمقی مفصل مچ پا میگردد. لذا به بازیکنان و مربیان فوتبال توصیه میشود از این تمرینات در برنامه گرم کردن قبل از مسابقه و تمرین به منظور کاهش خطر بروز آسیب استفاده کنند.

ارزیابی تغییرات عملکرد بینایی ناشی از مصرف داروی متادون در مرحلهی نگهدارندهی ترک اعتیاد در افراد معتاد
هدف: عملکرد بینایی یکی از مهمترین فاکتورهایی است که بر بسیاری از فعالیتهای روزانه افراد موثر بوده و مصرف داروهای مختلف میتواند آن را تغییر دهد. متادون یکی از داروهای مورد استفاده در ترک اعتیاد است که هم در مرحلهی سمزدایی (تجویز متادون و کاهش تدریجی دوز دارو طی مدت چند روز) و هم در مرحلهی نگهدارنده(درمان طولانی مدت با متادون تا چندین ماه یا سال) به وفور استفاده میشود. بررسی عملکردهای بینایی در پاسخ به مصرف مداوم متادون در جهت افزایش درک اثرات این دارو و بهبود مراقبتهای بینایی در این دسته از بیماران امری ضروری است که هدف ما در این مطالعه میباشد. روش بررسی: در این مطالعه (مورد شاهدی) 30 نفر فرد معتاد که به کلینیکهای ترک اعتیاد مراجعه کردند با میانگین سنی 9/90 ±40/97 سال به دو گروه تقسیم شدند. گروه مطالعه شامل 20 نفر معتاد و گروه شاهد شامل 10 نفر معتاد بودند. چندین تست عملکرد بینایی شامل: حدت بینایی، حساسیت کنتراست, میدان بینایی، و استریوپسیس انجام شد. بیماران گروه مطالعه بیش از 6 ماه از داروی متادون استفاده میکردند و بیماران گروه شاهد به تازگی وارد مرحلهی نگهدارندهی ترک اعتیاد شده بودند. یافتهها: نتایج مطالعه نشان میدهد که حدت بینایی استاتیک و میدان بینایی با مصرف داروی متادون تغییری نمیکنند. حساسیت کنتراست در سه فرکانس 3 (0/03= p)، 6 (0/001= p) و 18 (0/00003= p) سیکل در دقیقه از نظر آماری کاهش قابل ملاحظهای را نشان داد. دید عمق نیز در اثر استفاده طولانی از متادونبه میزان 19/5 ثانیه بر کمان کاهش یافت (0/04=p). میزان عیب انکساری در گروه شاهد نسبت به گروه مطالعه به میزان تقریبی 1 دیوپتر منفی تر بدست آمد. نتیجهگیری: با توجه یه این که داروی متادون بصورت طولانی مدت در مرحلهی نگهدارنده ترک اعتیاد استفاده میشود بایستی به مشکلات ناشی از این دارو مانند مشکلات دید عمق و کاهش حساسیت کنتراست توجه داشت و مراقبتهای چشمی و آگاهی لازم را برای بیمار فراهم آورد.

هدف: سندروم استرس داخلی درشت نی وضعیتی شامل افزایش درد در دو سوم دیستال استخوان درشت نی و در لبه ی خلفی داخلی این استخوان می باشد. هدف از مطالع هی حاضر بررسی تفاوت راستای آناتومیک ران و زانو در دوندگان دارای سندروم استرس داخلی درشتنی می باشد. روش بررسی: نمونه آماری مورد مطالعه در تحقیق حاضر 70 دونده نخبه پسر میباشد که در دو گروه مبتلا به سندروم استرس داخلی درشت نی و گروه کنترل قرار گرفتند. متغیرهای زانو ضربدری و پرانتزی (شاخص IM-IC)، زانوی عقب رفته، زاویه Q، Anteversion ران، چرخش داخلی و خارجی مفصل ران در این افراد اندازهگیری شدند. اطلاعات جمعآوری شده با استفاده از روش آماری تی مستقل و نرم افزار SPSS نسخه 18 مورد تجزیه و تحلیل قرار گرفت. یافتهها: یافتههای حاصل از تجزیه و تحلیل دادهها نشان داد که بین میانگین زانوی عقب رفته، Anteversion ران و دامنه چرخش خارجی مفصل ران در دو گروه مورد مطالعه تفاوت معنیداری وجود دارد، به طوری که میزان باز شدن بیش ازحد زانو (2/62± 6/0) (2/33 ±3/51) و Anteversion ران (2/87 ± 16/7) (2/52 ± 14/38) در گروه مبتلا بیشتر میباشد و همچنین افراد گروه مبتلا به سندروم استرس اخلی درشت نی دارای محدودیت دامنه حرکتی چرخش خارجی ران (6/23 ± 30/50) (6/25 ± 33/15) میباشند (0/05>p). در حالی که بین میانگین شاخص IM-IC (2/94 ±1/90) (3/30 ± 1/98)، زاویه Q (3/20 ± 11/6) (3/90 ± 10/7) و دامنه حرکتی چرخش داخلی ران (5/98 ± 32/3) (5/06 ± 31/8) در دو گروه مورد مطالعه تفاوت معنیداری یافت نشد (0/05<p). نتیجه گیری: با توجه به نتایج مطالعه حاضر احتمالا زانوی عقب رفته، افزایش زاویه Q، افزایش میزان Anteversion و محدودیت در چرخش خارجی مفصل ران از عوامل موثر و خطر زا در وقوع سندروم استرس داخلی درشت نی میباشند، که میتوان با تشخیص به موقع ناهنجاری و مداخله زود هنگام از وقوع این آسیب پیشگیری نمود.

بررسی میزان آمادگی دانشجویان جهت استفاده از فناوری یادگیری الکترونیکی
هدف: توسعه فناوری اطلاعات در آموزش، موجب گسترش استفاده از شیوههای جدید آموزش و یادگیری در دانشگاهها گردیده است. اجرای پروژههای یادگیری الکترونیکی در دانشگاههای کشور و بررسی پیش نیازها و میزان آمادگی یادگیرندگان جهت حضور فعال در محیط یادگیری الکترونیکی مستلزم مطالعاتی گسترده در این زمینه میباشد. لذا این پژوهش با هدف بررسی میزان آمادگی دانشجویان دانشگاه علوم پزشکی زابل جهت استفاده از نظام یادگیری الکترونیکی انجام گرفت. روش بررسی: پژوهش حاضر از نوع توصیفی – تحلیلی بود که به صورت مقطعی در سال تحصیلی 1394-1393 انجام گرفت. جامعه پژوهش، دانشجویان دانشگاه علوم پزشکی زابل بودند که تعداد 300 نفر از آنها با استفاده از روش نمونه گیری طبقه ای تصادفی به عنوان نمونه آماری انتخاب شدند. ابزار پژوهش پرسشنامه محقق ساخته بود. دادهها پس از جمع آوری با استفاده از آمار توصیفی و استنباطی در نرم افزارSPSS مورد تجزیه و تحلیل قرار گرفت. یافته ها: نتایج پژوهش حاضر نشان داد که میانگین شاخصهای میزان آمادگی جهت استفاده از یادگیری الکترونیکی به ترتیب در بعد دسترسی به فناوری 10/92، مهارتها و ارتباطات پیوسته 32/86، انگیزش 9/52، توانایی یادگیری از طریق رسانهها 11/17، گفتگوهای گروهی اینترنتی10/77 و مسائل مهم جهت موفقیت در یادگیری الکترونیکی 18/23 بود. همچنین، بین میزان آمادگی دانشجویان و مقطع تحصیلی رابطه معناداری وجود داشت (0/01=p). نتیجه گیری: بر اساس یافته های پژوهش حاضر، نبود زیرساخت های مناسب فناوری جهت استفاده از یادگیری الکترونیکی موجب شد تا تنها 3/32 درصد از دانشجویان آمادگی در سطح خوب جهت استفاده از نظام یادگیری الکترونیکی داشته باشند. لذا می توان نتیجه گرفت که برای پیاده سازی و بکارگیری موثر یادگیری الکترونیکی ابتدا باید ابعاد مختلف مورد بررسی در این پژوهش، بهبود و ارتقاء یابد.

بررسی تأثیر آموزش توجه اشتراکی بر میزان آغازگری-پاسخدهی اجتماعی کودکان دارای اختلال اوتیسم: پژوهش مورد منفرد
هدف: اختلال اوتیسم مشکلات زیادی را برای کودک ایجاد میکند. آغازگری -پاسخدهی اجتماعی، یکی از این مشکلات است که تأثیر قابل توجهی بر مهارتهای رشدی و تحولی کودک دارد. با توجه به این، پژوهش حاضر با هدف بررسی تأثیر آموزش توجه اشتراکی بر میزان آغازگری-پاسخدهی اجتماعی کودکان دارای اختلال اوتیسم انجام گرفت. روش بررسی: جامعه آماری شامل کلیه کودکان دارای اختلال اوتیسم در شهر اصفهان بود. از آنجا که پژوهش حاضر، به دنبال موارد خاص و یا غیرمعمول بود، از روش نمونهگیری هدفمند استفاده شد. بر این اساس، سه کودک زیر 6 سال که علائم و نشانههای اختلال اوتیسم را داشتند، به عنوان نمونه انتخاب شدند. ابزار پژوهش، مقیاس تشخیص اوتیسم گیلیام (GARS) و مقیاسِ سنجشِ رفتاریِ تعاملات اجتماعی در کودکان خردسال (BASYC) بود. در این پژوهش از روش مورد منفرد با طرح A-B و پیگیری استفاده شد. یافتهها: میانگین نمرات از 30/95، 40/77 و 40/37 برای هر سه آزمودنی در خط پایه به 51/05، 50/05 و 50/38 در موقعیت مداخله رسید. طی تحلیل دیداری نمودار دادهها، مداخله در هر سه آزمودنی اثربخش بود. درصد غیرهمپوشی دادهها (PND) در دو موقعیت خط پایه و مداخله برای هر سه شرکتکننده به ترتیب 70%، 90% و 90% بود. این اثربخشی در موقعیت مداخله نیز همچنان قابل مشاهده بود. نتیجهگیری: با توجه به نتایج بدست آمده میتوان اینگونه قضاوت کرد که آموزش توجه اشتراکی، روشی مناسب جهت بهبود آغازگری-پاسخدهی اجتماعی در کودکان دارای اختلال اوتیسم است و میتوان از این روش، در مراکز آموزشی و درمانی کودکان دارای اختلال اوتیسم استفاده کرد.

کدام تشخیص آزمایشگاهی صحیح است؟ سالک یا گرانولوم تنگ ماهی؟ بررسی موارد گرانولوم استخر شنا با تشخیص اولیه لیشمانیوز جلدی در سه بیمار در مشهد
هدف: مایکوباکتریوم مارینوم (Mycobacterium marinum) یک باکتری اسید فاست آزاد زی است که درگروه مایکوباکتریوم های آتیپیک قرار میگیرد و عامل یک بیماری پوستی لنفاوی بنام گرانولوم تنگ ماهی یا استخر شنا میباشد. این ارگانیسم بیشتر در استخرهای طبیعی، آکواریوم های خانگی و آبزیان آبهای شور و شیرین وجود دارد. در این گزارش سه نفر که در مشهد مبتلا به گرانولوم تنگ ماهی گردیده اند، معرفی و شرح داده شده اند. گزارش موارد: در طول 3 ماه از سال 1395، سه نفر که دارای ضایعات جلدی روی اندام های خود بودند به آزمایشگاه انگل شناسی و کلینک ویژه بیمارستان امام رضا مراجعه نمودند. تمام افراد توسط پزشک متخصص پوست معاینه شده و با تشخیص بالینی اولیه سالک جهت تشخیص قطعی علت ضایعه به آزمایشگاه معرفی شده بودند. از این تعداد 2 نفر مرد و 1 نفر زن بودند. ابتدا برای بیماران اسمیر مستقیم رنگ آمیزی گیمسا تهیه گردید و در صورت منفی بودن نتیجه با توجه به شرح حال بیماران رنگ آمیزی زیل نیلسن و کشت روی محیط لونشتاین جانسن تهیه شد . مشاهده باسیل های قرمز رنگ و یا رشدکلنی های زرد رنگ عفونت Mycobacterium marinum را در ضایعات پوستی تایید نمود. نتیجه گیری: گرانولوم تنگ ماهی یک بیماری پوستی نوپدید در مشهد است که بصورت اسپورادیک مشاهده می شود. ضایعات پوستی شباهت بالینی زیادی با سالک بویژه فرم اسپوروتریکوتیک دارند. بیمارانی که ضایعات پوستی آن ها مشابه سالک می باشد و سابقه تماس با ماهی و یا شنا در استخر های دارای ماهی دارند بایستی از نظر گرانولوم تنگ ماهی نیز آزمایش شوند.

رابطه بین «الگوی استفاده از زمان» و جنبه های رشد و نمو کودکان بر مبنای طبقه بندی بینالمللی کارکرد (یک بررسی متون روایی)
هدف: الگوی مطلوب استفاده از زمان میتواند منجر به رشد و بالندگی کودکان و نوجوانان شود. آگاهی از این موضوع که چگونگی استفاده از زمان میتواند چه تاثیری بر ویژگیها و رشد کودک بر جای بگذارد، میتواند در برنامهریزی صحیح و مدیریت زمان توسط خانوادهها و سازمانهای مسئول بسیار سودمند باشد. بنابراین هدف مطالعه حاضر عبارت بود از بررسی رابطه الگوی استفاده از زمان با ویژگیهای فردی، رشد و پیشرفت کودکان و نوجوانان بر مبنای سطوح طبقهبندی بین المللی کارکرد. روش بررسی: در این مطالعه از روش بررسی روایی استفاده شد. فرایند مطالعه شامل جستحوی مقدماتی و انتخاب کلید واژهها، مشخص نمودن راهبرد جستجو، جستجوی سازمان یافته، بررسی اجمالی عناوین مقالات و پایان نامهها، بررسی یافتههای اولیه و تطبیق آن با ملاکهای ورود مقالات به مطالعه با استفاده از مطالعه چکیده مقالات، مشخص نمودن مطالعات مرتبط، مطالعه و تحلیل متن کامل مقالات و جمعبندی و نتیجهگیری نهایی بود. جهت گردآوری اطلاعات از جستجوی پایگاه داد های PubMed، Science-direct و ProQuest با استفاده از کلید واژههای “time use”، child، adolescent در محدوده زمانی ده ساله از ابتدای سال 2005 تا 2015 استفاده شد. یافتهها: در جسجوی اولیه با استفاده از کلیدواژهها تعداد 658 مقاله و پایاننامه یافت شد که پس از بررسی عناوین و حذف موارد تکراری تعداد 243 یافته و بعد از مطالعه چکیدهها تعداد 64 یافته باقی ماند و پس از مطالعه متن کامل مقالات، تعداد 19 مقاله با ملاکهای ورود همخوانی داشتند. یافتههای نهایی شامل 10 محور بود که سطوح مختلف طبقهبندی بینالمللی کارکرد شامل سطح ساختارها و کارکردهای بدنی، سطح فعالیت و سطح مشارکت را در بر میگیرد. نتیجهگیری: یافتههای حاصل از مطالعه پشنهاد میکند که الگوی استفاده از زمان در هر سه سطح کارکرد بر مبنای طبقهبندی بینالمللی کارکرد (مشتمل بر سطوح کارکردها و ساختارهای بدنی، فعالیت ها و مشارکت) رابطه معناداری با چاقی و شاخص توده بدنی، مشکلات رفتاری، کارکردهای اجرایی، افسردگی، تاخیر رشد، فعالیتهای مناسب رشد، رفتارهای مسئلهدار و پر خطر، دست آوردهای تحصیلی و خواب دارد و جهت هرگونه اظهار نظر دقیق و علمی در این خصوص نیاز به پژوهشهای بیشتری وجود دارد.
