بررسی اثر سن، جنس و عیوب انکساری بر ضخامت مرکزی قرنیه در مراجعه کنندگان به بیمارستان چشم پزشکی الزهرا (س) زاهدان
هدف: ضخامت مرکزی قرنیه، به عنوان یکی از پارامترهایسگمان قدامی چشم، از فاکتورهای مهم در ارزیابی بیماران مبتلا به اختلالات چشمی به شمار می رود. هدف از این پژوهش، بررسی اثر سن، جنس و عیوب انکساری بر ضخامت مرکزی قرنیه در مراجعه کنندگان به بیمارستان چشم پزشکی الزهرا (س) زاهدان، با استفاده از روش مدلسازی رگرسیون چندکی خطی میباشد. روش بررسی: مطالعه مقطعی حاضر بر روی ۷۵۵ چشم از افراد واجد شرایط ورود به مطالعه که برای معاینات قبل از جراحی عیوب انکساری از سال ۱۳۹۲ تا ۱۳۹۴ به بیمارستان چشم پزشکی الزهرا (س) زاهدان، استان سیستان و بلوچستان مراجعه کرده بودند، صورت پذیرفت. پس از محاسبه شاخص های توصیفی، رگرسیون چندکی خطی به دادهها برازش و حدود طبیعی برآورد شدند. تحلیل ها با استفاده از نرم افزارهای SPSS(21) و R(3.3.0) انجام گرفت. یافتهها: در این مطالعه، تعداد 422 نفر (56%) از شرکت کنندگان را زنان تشکیل می دادند. میانگین سن افراد 7/6±۲۷ سال بود. بر اساس نتایج مدل رگرسیون چندکی خطی، سن به طور معناداری بر ضخامت مرکزی قرنیه مؤثر بود. در حالی که، جنس و مقدار عیب انکسار تاثیر آماری معناداری را نشان ندادند. همچنین حدود طبیعی برای زنان (۶۰۲ – ۴۸۹) و برای مردان (۶۰۰ – ۴۸۲) میکرومتر بدست آمد. به منظور اهداف مقایسه ای، این حدود به تفکیک گروههای سنی و جنس نیز گزارش شده است. نتیجه گیری: حدود طبیعی حاصل از این پژوهش میتواند توسط چشم پزشکان در معاینات قبل از انجام جراحی های عیوب انکساری و ارزیابی پاتولوژیهای چشمی از جمله گلوکوم مورد استفاده قرار گیرد.

مقایسه ویژگیهای آیرودینامیکی صدا در مبتلایان به اختلال صدای اسپاسمودیک و تنش عضله
هدف: صداسازی یا تولید انرژی صوتی قابل شنیدن ناشی از فعالیتهای عضلانی و آیرودینامیکی است که بر بافت تارهای صوتی اثر می گذارد. هدف اصلی این پژوهش بررسی پارامترهای آیرودینامیکی در دو گروه اختلال صدای اسپاسمودیک و اختلال صدای ناشی از تنش عضله بود. روش بررسی: آزمودنیها شامل 30 فرد (10 نفر اختلال صدا از نوع اختلال صدا اسپاسمودیک، 10 نفر اختلال صدا از نوع اختلال صدا ناشی از تنش عضلانی و 10 نفر دارای صدای طبیعی) بودند. ظرفیت حیاتی هریک از آزمودنیها با استفاده از دستگاه اسپیرومتر ثبت شد و با کمک زمان نگهدار حداکثر زمان آواسازی محاسبه شد. دادهها با نرمافزار SPSS 19 آنالیز شد. یافته ها: حداکثر زمان آواسازی در اختلال صدا ناشی از تنش عضله کمتر از دو گروه دیگر است و در گروه اختلال صدا اسپاسمودیک کمتر از گروه طبیعی بود،که تفاوت میانگینها بین سه گروه معنادار بود (0/001>p). ظرفیت حیاتی در گروه اختلال صدا ناشی از تنش عضله از دو گروه دیگر کمتر و در گروه اختلال صدا اسپاسمودیک و گروه طبیعی برابر بود، که تفاوت میانگین ها بین سه گروه معنادار بود (0/001>p). ضریب آواسازی در گروه اختلال صدا ناشی از تنش عضله بیشتر از گروه اختلال صدا اسپاسمودیک و در گروه اختلال صدا اسپاسمودیک بیشتر از گروه طبیعی بود و باز هم تفاوت میانگین ها بین سه گروه معنادار بود (0/001> p). نتیجه گیری: حداکثر زمان آواسازی بین سه گروه طبیعی، اختلال صدا اسپاسمودیک و اختلال صدا ناشی از تنش عضله تفاوت معنادار دارد. ظرفیت حیاتی در گروه طبیعی و اختلال صدا اسپاسمودیک تفاوت نداشت و در گروه اختلال صدا ناشی از تنش عضله متفاوت از دو گروه دیگر بود. به نظر می رسد کاهش ظرفیت حیاتی در این دسته از مبتلایان خود می تواند توجیهی برای بروز این نوع اختلال صدا باشد. نتایج نشان دادند که تغییرات در پارامترهای آیرودینامیکی علاوه بر تفکیک گروه طبیعی از گروه دارای اختلال صدا، وابسته به نوع اختلال نیز هست. این تفاوتها باید در روند ارزیابی و مداخلات درمانی مدنظر قرار گیرد.

درک جملات کنایه آمیز در بیماران فارسی زبان مبتلا به پارکینسون
هدف: علیرغم وجود پژوهشهای مختلف در زمینه شناسایی رفتارهای غیرحرکتی مبتلایان به پارکینسون، پژوهشهای محدودی در زمینه مشکلات شناختی و زبانی یافت شده است. بر این اساس به نظر می رسد که جنبه های پیچیده زبان نیز در این بیماری دستخوش اختلال شده باشد. یکی از حیطههای پیچیده و پیشرفته مربوط به این مهارت، زبان غیرتحت اللفظی است که یکی از زیرشاخه های آن، کنایه ها میباشند. هدف این پژوهش بررسی توانایی درک کنایه ها در مبتلایان به پارکینسون سطح 1(Stage1) بوده است. روش بررسی: در این مطالعه، 15 فرد سالم و 15 فرد مبتلا به پارکینسون سطح 1 مورد بررسی قرار گرفتهاند. به عنوان ابزار مورد استفاده، پرسشنامهای شامل 14 کنایه توسط آزمونگر ساخته شد. سپس براساس نظر 8 صاحبنظر روایی محتوایی آن 75/0 و پایایی آن براساس آلفای کرونباخ 74/0 بهدست آمد. کنایه ها به صورت شفاهی از هر فرد پرسیده می شد و او می بایست منظور آن را بیان می کرد. بر اساس نتایج آزمون کولموگروف-اسمیرنوف (Kolmogorov–Smirnov test) توزیع داده ها نرمال بوده و دادهها از طریق آزمون t مستقل بررسی شده و میانگین آنها با یکدیگر مقایسه گردید. یافته ها: میانگین امتیازات بدست آمده در پاسخ به کنایه ها در افراد سالم بیشتر بوده است اما بین دو گروه تفاوت معناداری دیده نشد (0/05<p).نتیجه گیری: در کنایه های مورد بررسی، عملکرد مبتلایان به پارکینسون سطح 1 پایین تر از حد انتظار بود. چنانچه در این جنبه تفاوتی وجود داشته باشد آزمونهای مفصلتر و پیچیده تری مورد نیاز است تا آن را نشان دهد، یا ممکن است در سطحهای شدیدتر بیماری، مشاهده شود (سطح 2 یا بیشتر).

تاثیر یک دوره برنامه تمرینی ترکیبی منتخب بر عملکردهای حرکتی تعادل و قدرت عضلانی پسران مبتلا به سندرم داون
هدف: سندرم داون شایعترین اختلال کروموزومی و شایعترین علت ژنتیکی کمتوانی ذهنی در انسان است. هدف از این مطالعه تاثیر یک دوره برنامه تمرینی ترکیبی منتخب بر عملکردهای حرکتی تعادل و قدرت عضلانی پسران مبتلا به سندرم داون است. روش بررسی: 20 پسر مبتلا به سندرم داون با سن 12-20 سال به طور تصادفی در دو گروه تجربی 10 نفر و گروه کنترل 10نفر قرار گرفتند. گروه تجربی، تحت برنامه تمرینی ترکیبی به مدت 45-60 دقیقه در روز و 3 بار در هفته، به مدت 8 هفته قرا گرفتند. برای ارزیابی قدرت عضلانی و تعادل در افراد مبتلا به سندرم داون، نیروی عضلانی با دینامومتر دستی (MMT , North coast, USA) و تعادل با شارپند رومبرگ (0/91 -0/90=r) با چشمان باز و بسته اندازهگیری شد. تجزیه و تحلیل آماری با استفاده از آزمون آماری واریانس دو طرفه (ANOVA) با اندازهگیری مکرر استفاده شد(0/05≥p). یافته ها: نتایج آزمون تحلیل واریانس بین گروهی با اندازههای مکرر نشان داد که قدرت Flexion زانو (0/05≥p)، Extension زانو(0/05≥p)، Flexion ران(0/05≥p)، Extension ران(0/05≥p)، Plantar Flexion مچ (0/05≥p)، Dorsiflexion مچ(0/05≥p) و همجنین تعادل ایستا با چشمان باز (سطح سفت) (0/05≥p)، تعادل ایستا با چشمان بسته (سطح سفت) (0/05≥p) ، تعادل ایستا با چشمان باز (سطح نرم) (0/05≥p)، تعادل ایستا با چشمان بسته (سطح نرم) (0/05≥p)، در گروه تمرینی به طور معناداری بهتر از گروه کنترل می باشد. نتیجه گیری: برنامه ترکیبی قدرتی و حس عمقی تعادل و پاسچرال در کودکان مبتلا به سندرم داون را بهبود می بخشد که ممکن است به علت افزایش قدرت عضلانی و یکپارچه سازی ورودی حس عمقی باشد.

بررسی ارتباط عملکرد خانواده با سازگاری فردی معلولین ضایعه نخاعی تحت پوشش اداره بهزیستی شهرستان الیگودرز
هدف: ضایعه نخاعی منجر به ایجاد تغییرات کلی در ساختار خانواده و مشکلات سازگاری برای فرد معلول میشود. اعضای خانواده که از این افراد مراقبت میکنند و تعامل نزدیکی با آنها دارند با تأثیرپذیری از این مسئله، دچار مشکلات جدیدی شده و فشارهای روانی زیادی را متحمل میشوند. این پژوهش با هدف بررسی ارتباط عملکرد خانواده با سازگاری فردی معلولین ضایعه نخاعی تحت پوشش اداره بهزیستی شهرستان الیگودرز انجام شد. روش بررسی: پژوهش حاضر توصیفی_تحلیلی از نوع مطالعات همبستگی است. نمونه این پژوهش را 50 نفر از معلولین ضایعه نخاعی و مراقب اصلی آنها در خانواده تشکیل داد. ابزارهای مورد استفاده در این پژوهش، عبارت بودند از پرسشنامه عملکرد خانواده و پرسشنامه سازگاری فردی کالیفرنیا. برای تجزیه و تحلیل دادههای بدست آمده از آزمونهای آماری کلموگروف اسمیرنوف، ضریب همبستگی پیرسون استفاده شد. یافتهها: یافتههای آماری نشان داد که بین متغیرهای حل مشکل (0/455=Pearson)، ایفای نقش(0/327=Pearson)، آمیزش عاطفی (0/465=Pearson)، کنترل رفتار(0/312=Pearson)، کارکرد عمومی (0/330=Pearson) عملکرد خانواده و سازگاری فردی معلولین رابطه معناداری وجود دارد (0/01>p). نتیجه گیری: نتایج این پژوهش نشانگر آن است که عملکرد خانواده میتواند بر سازگاری فردی معلولین ضایعه نخاعی تأثیر بگذارد. بنابراین با تمسک به روشهایی که طی آن عملکرد خانواده بهبود یابد، میتوان سازگاری فردی معلولین ضایعه نخاعی را بهبود بخشیده و به تبع آن سلامت معلولین و کل خانواده آنها را ارتقا داد.

ویژگیهای گفتمان روایتی در بزرگسالان فارسی زبان
هدف: زبان در طول زندگی انسان برای برقراری تعاملات اجتماعی اهمیت زیادی دارد و آسیبشناسان گفتار و زبان نیز در حفظ و ارتقای گفتار و زبان در افراد نقش مهمی دارند. در دوران بزرگسالی، با افزایش سن خصوصا در دورانسالمندی اغلب تغییراتی در عملکرد شناختی به وجود میآید. این تغییرات بر روی توانمندیهای درکی و بیانی زبان تاثیر میگذارد. بنابراین از آنجا که با آگاهی از تغییرات ویژگیهای گفتمان در سنین مختلف زندگی میتوان به بهبود گفتمان و ارتباط در سالمندی کمک کرد، مطالعه زبان در سنین مختلف بزرگسالی امری ضروری به نظر می رسد. روش بررسی: پژوهش حاضر یک مطالعهی توصیفی- تحلیلی و از نوع مقطعی است. در این مطالعه گفتمان روایتی 91 فرد بزرگسال در دامنه سنی 19 تا 74 سال که به چهار گروه سنی تقسیم شده بودند، مورد بررسی قرارگرفت. ابتدا هر فرد فرم رضایتنامه شرکت در پژوهش و فرم اطلاعات فردی را پر کرد. سپس ارزیابی شناختی با استفاده از آزمون غربالگری (Mini-Mental State Examination ;MMSE) و در نهایت، ارزیابی توانایی های گفتمان روایتی با استفاده از آزمایه ارزیابی گفتمان روایتی فارسی انجام شد. به منظور بررسی نرمال بودن داده ها از آزمون کولموگروف- اسمیرنوف استفاده استفادهگردید و پس از تایید نرمال بودن دادهها در مورد ابزار انسجام، برای مقایسه این متغیر در گفتمان روایتی در گروههای مختلف سنی از آزمون ANOVA و برایی متغیر پیوستگی که توزیع آن نرمال نبود از آزمون Kruskal-Wallis استفاده شد. تحلیلها با استفاده از نرمافزار SPSS انجام گردید. یافته ها: بین گروه سالمند و هر یک از سه گروه دیگر از لحاظ دو متغیر انسجام و پیوستگی تفاوت معنادار آماری وجود دارد (0/01>p). نتیجه گیری: نتایج این مطالعه نشاندهنده اینست که تواناییهای مربوط به گفتمان تحت تاثیر سن و خصوصا سالمندی قرار میگیرند. به همین دلیل نیاز است تا آسیبشناسان گفتار و زبان توجه به نیازهای زبانی و نیازهای شناختی زیربنایی آنها را در برنامههای بالینی خود جای دهند.

بررسی مقاومت آنتی بیوتیکی سویه های اشرشیا کلی جدا شده از عفونت ادراری بیماران سرپائی مراجعه کننده بیمارستان نبی اکرم (ص) شهرستان زاهدان
هدف: عفونتهای مجاری ادراری (Urinary tract infections) دومین عفونت شایع انسانی است که توسط خانواده انتروباکتریاسه و به خصوص Escherichia coli به وجود می آید. با توجه به افزایش مقاومتهای آنتی بیوتیکی بین سویه های اشرشیا کلی، هدف از این مطالعه تعیین الگوی کنونی و همچنین بررسی تغییرات الگوی مقاومت آنتی بیوتیکی سویه های اشرشیا کلی جدا شده از عفونتهای ادراری بیماران سرپائی مراجعه کننده به بیمارستان نبی اکرم شهر زاهدان در طی سالهای گذشته بود. روش بررسی: این مطالعه توصیفی بر روی 87 نمونه ی باکتری اشرشیا کلی جدا شده از بیماران مبتلا به عفونتهای ادراری بیماران یکی از بیمارستانهای شهر زاهدان انجام شد. در نهایت آزمون حساسیت دارویی برای آنتی بیوتیکهای: ایمی پنم (IPM)، جنتامایسین (GM)، سفتازیدیم (CAZ)، نالیدیکسیک اسید (NA)، آمیکاسین (AN)، نیتروفورانتوئین (FM) و کوتریموکسازول (SXT) برای ایزولههای اشرشیا کلی انجام شد. یافته ها: از مجموع 87 نمونه، 70/2% ایزوله ها مربوط به زنان و 29/8% ایزولهها از مردان جداسازی شده بودند. بیماران در محدوده ی سنی 45-12 سال بودند و میزان مقاومت به آنتی بیوتیکها به ترتیب: کوتریموکسازول (66/6%)، نالیدیکسیک اسید (63%)، سفتازیدیم (44/8%)، نیتروفورانتوئین (26/1%)، آمیکاسین (19/5%)، جنتامایسین (13/7%) و ایمی پنم (4/5%) ارزیابی شد. نتیجه گیری: براساس نتایج این مطالعه آنتی بیوتیک های ایمی پنم و کوتریموکسازول به ترتیب کمترین و بیشترین میزان مقاومت آنتی بیوتیکی را داشتند. لذا پیشنهاد می شود برای درمان عفونتهای ادراری تجویز آنتی بیوتیک کوتریموکسال متوقف گردد و از داروی ایمی پنم استفاده شود.

هدف: در در سالهای اخیر علاقۀ فزایندهای به بررسی رابطه بین زبان و نظریه ذهن در کودکان دارای اُتیسم به وجود آمده اما با وجود مطالعات انجام شده، هنوز ماهیت این رابطه به درستی مشخص نشده است و یافته های تحقیقات دربارۀ جهت اثر و مؤثرترین عنصر زبانی دخیل در نظریۀ ذهن این کودکان متفاوت بوده است. با توجه به انجام نشدن این مطالعه در زبان فارسی، هدف پژوهش حاضر بررسی رابطۀ بیان انواع ساختهای دستوری زبان فارسی و نظریۀ ذهن در کودکان فارسی زبان دارای اُتیسم است. روش بررسی: در این مطالعه، داده های پژوهش از 10 کودک دارای اُتیسم و 20 کودک طبیعی (10 همتای سنی و 10 همتای زبانی) جمع آوری شد. پس از اینکه کودکان اُتیسم با عملکرد بالا با آزمون غربالگری و تشخیصی اُتیسم تشخیص داده شدند، بیان ساختارهای مختلف دستوری و توانایی نظریۀ ذهن به ترتیب با استفاده از آزمون بیان دستور و آزمون نظریۀ ذهن بررسی شد و همبستگی داده ها با نرم افزار آماری SPSS نسخۀ 20 تحلیل شد. یافته ها: نتایج پژوهش نشان داد در گروه همتای سنی بین امتیاز مربوط به آزمون نظریه ذهن سطح سوم با هریک از امتیازات مربوط به تعداد ساخت درست بیانی و تعداد گویههای درست بیانی در آزمون بیانی دستور زبان همبستگی معناداری وجود داشت، اما در گروه همتای زبانی و اُتیسم همبستگی معناداری وجود نداشت. برای بررسی دقیقتر این ارتباط در کودکان دارای اُتیسم، همبستگی انواع ساختهای دستوری به صورت مجزا با نظریۀ ذهن بررسی شد و نتایج نشان داد بین ساختهای مربوط به بند موصولی قیدی، شناسه فعل و آزمون نظریۀ ذهن سطح دوم و نظریۀ ذهن کلی همبستگی معناداری وجود دارد. نتیجه گیری: بنابراین برای نتیجه گیری دربارۀ ارتباط زبان و نظریۀ ذهن در کودکان دارای اُتیسم، بررسی ساختارهای مختلف دستوری به صورت مجزا ضروری است و علاوه بر این، در درمان جنبه های شناختی آنها (رشد نظریۀ ذهن)، باید به ساختهای دستوری همچون موصولها و نشانه های افعال، توجهی جدی داشت.

تحریک دهلیزی: پروتکلهای درمانی، اثربخشی و کاربرد درمانی در توانبخشی اطفال (یک مطالعه مروری روایتی)
هدف: با توجه به کاربرد گسترده تحریک دهلیزی در کلینیکها نیاز به پروتکلهای سازمان یافته بیشتر در این زمینه ضروری به نظر میرسد. هدف از این مرور روایتی بررسی مبتنی بر شواهد پروتکل تحقیقات موجود و هدایت این جریان بالینی به سمت و سوی ساختار یافته است. روش بررسی: در مقاله مروری روایتی (Narrative) حاضر مستندات موجود بین سالهای 1972 تا 2014 در پایگاه های اطلاعاتی معتبر PubMed، Medline, Scopus ، Web of Scienceو Google Scholar بررسی شده اند. کلمات کلیدی "تحریک مکانیکی دهلیزی"، "کودکان"، "مجاری نیم دایره"، "اتولیت" و "توانبخشی" بودند. یافته ها: 203 مقاله یافت شد و در نهایت با رعایت معیارهای ورود 34 مطالعه مورد تجزیه و تحلیل قرار گرفت. پروتکلهای درمانی آنها بحث شده است. در بیشتر مقالات مورد مطالعه اثرات درمانی دهلیزی موثر شناخته شده است و در مجموع برآیند شواهد به نفع آن است که مداخلات دهلیزی می تواند نقش مثبتی در بهبود کودکان با ناتوانی داشته باشد. به علاوه، پروتکلهای پیشنهادی محققین نیز طبقه بندی و ارائه گردید. مطالعات نشان می دهند که درمانی کارآمد است که کلیه گیرنده های دهلیزی اعم از اوتریکول، ساکول و همه مجاری نیم دایره را در وضعیت های متفاوتی از جمله به شکم خوابیده، به پشت خوابیده، به پهلو خوابیده ، چهار دست و پا، نشسته، در حالت زانو زده و ایستاده تحریک کند. نتیجه گیری: با تکیه بر شواهد یافت شده موجود پیرامون اثر بخشی تحریک مکانیکال دهلیزی، قویا پیشنهاد می شود تحریک دهلیزی سازمان یافته با پروتکل تخصصی بخشی از برنامه درمانی کودکان جهت غنی سازی فرآیند توانبخشی باشد. توصیه می گردد که سن دریافت تحریک، نوع تشخیص، شدت اختلال، سابقه دریافت تحریک، کنترل وضعیت سر، نوع تحریک، فرکانس، بزرگی و توالی تحریک مورد توجه قرار گیرد. بهتر است که جهت تحریک، مسیر طولی تحریک از لحاظ دامنه انجام و حتی طول مدت هر تحریک منفرد در یک جلسه و تعداد جلسات، موقعیت بدنی فرد در زمان دریافت تحریک، میزان توقف بلافاصله بعد از حرکت، فاصله بین تحریکات، سلامت محیطی و مرکزی خود سیستم دهلیزی و ابزار ارزیابی مناسب، در تدوین و طراحی پروتکل مداخله بالینی تحریکات دهلیزی، در نظر گرفته شوند.
