ارزیابی مقایسه ای مشخصات توپوگرافی قرنیه در بیماران دارای کراتوکونوس دو طرفه همراه با خطوط وگت یکطرفه
هدف: هدف از این مطالعه، ارزیابی مقایسه ای مشخصات توپوگرافی قرنیه ای دو چشم در بیماران دارای کراتوکونوس دو طرفه همراه با خطوط وگت یکطرفه می باشد. روش بررسی: در این مطالعه آینده نگر مقطعی، بیماران کراتوکونوس بالینی دوطرفه دارای خطوط وگت یکطرفه، براساس یافته های بیومیکروسکوپ و نقشه های توپوگرافی مبتنی بر دیسک پلاسیدو و توموگرافی مبتنی بر شیمفلاگ مطالعه شدند. همه بیماران، تحت معاینات چشمی جامع شامل اندازه گیری حدت بینایی دور اصلاح شده و اصلاح نشده، سایکلورفرکشن (محاسبه شده توسط آنالیز وکتور)، قرار گرفتند. در این مطالعه، مشخصات توپوگرافی بیماران توسط سیستم توپوگرافی قرنیه Tomey (TMS-4; Tomey, Germany) ارزیابی شد و در صورتی که اختلاف پارامترهای اندازه گیری شده میان چشم های کراتوکونوسی دارای خطوط وگت و فاقد خطوط، توزیع نرمال دارد، از آزمون تی زوجی و در غیر این صورت از آزمون رتبه های علامت دار ویلکاکسون استفاده شد. مقدار 0/05p<، سطح معنی داری آزمون ها در نظر گرفته شد. یافته ها: 50 بیمار، با میانگین سنی 5/46±27/43، بین سن 38-20 سال، وارد این مطالعه شدند. نتایج نشان داد که به جز پارامتر J45 (0/58=p)، سایر پارامترهای حدت بینائی دور اصلاح شده و اصلاح نشده، اسفر، سیلندر، معادل اسفر و J0، بین دو گروه کراتوکونوس دارای خطوط وگت و فاقد این خطوط، تفاوتی نداشتند (همه مقادیر p کمتر از 0/001). همچنین، پارامترهای اندازه گیری شده توسط توپوگرافی شامل کراتومتری فلت (3/33±46/48 در برابر 2/22±44/44)، کراتومتری استیپ (4/39±52/48 در برابر 3/26±47/02)، کراتومتری حداقل (3/37±46/36 در برابر 2/17±44/35)، کراتومتری میانگین (3/96±49/27 در برابر 2/60±45/74)، شاخص منظمی سطح (1/39±1/12 در برابر 0/41±0/52)، شاخص عدم تقارن سطح (1/25±2/32 در برابر 1/04±1/42)، شاخص کراتوکونوس (28/83±75/07 در برابر 33/52±48/10) و شاخص شدت کراتوکونوس (22/25±65/88 در برابر 29/40±38/61)، (مقدار p برای تمام پارامترهای مذکور کمتر از 0/001) در چشم دارای خطوط وگت بالاتر از چشم های فاقد خطوط وگت در بیماران کراتوکونوسی بود. نتیجهگیری: پارامترهای توپوگرافی قرنیه، تفاوت قابل ملاحظه ای بین چشم دارای خطوط وگت و فاقد خطوط وگت در بیماران کراتوکونوس، نشان دادند. نتایج این مطالعه می تواند در ارزیابی و درمان بیماران کراتوکونوس، مد نظر قرار گیرد.

ارزیابی پردازش زمانی در افراد مبتلا به دیابت نوع1 با استفاده از آزمون توالی الگوی دیرشی
هدف: دیابت نوع1 یکی از شایع ترین بیماری های مزمن در سنین جوانی می باشد. Hypoglycemia و همچنین اثرات سمی Hyperglycemia از جمله عواملی هستند که در ایجاد تغییرات و اختلالات سیستم عصبی مرکزی، مشارکت می کنند. پردازش زمانی شنوایی از توانایی های مهم دستگاه عصبی شنوایی مرکزی است هدف ازمطالعه حاضر مقایسه توانایی پردازش زمانی در افراد مبتلا به دیابت نوع1 با گروه کنترل با استفاده از آزمون الگوی دیرشی می باشد. روش بررسی: این پژوهش مقطعی- مقایسه ای بر روی دو گروه افراد شامل 25 فرد مبتلا به بیماری دیابت شیرین نوع 1 و 25 فرد هنجار در محدوده سنی 20 تا 30 سال انجام شد. شرکت کنندگان با روش نمونه گیری در دسترس انتخاب شدند و تحت ارزیابی با آزمون الگوی دیرشی قرار گرفتند. برای مقایسه میانگین مقادیر عددی درصد پاسخ های صحیح آزمون الگوی دیرشی بین دو گروه کنترل و مبتلا به دیابت، از آزمون T-test مستقل و برای بررسی همبستگی بین HbA1c (Hemoglobin A1c) و درصد پاسخ های صحیح آزمون الگوی دیرشی از آزمون ضریب همبستگی پیرسون استفاده شد. یافته ها: در آزمون الگوی دیرشی اختلاف معناداری بین نتایج دو گروه در هر دو گوش و در زنان و مردان مشاهده شد. همچنین بین مولفه درصد پاسخ های صحیح آزمون الگوی دیرشی با میزان HbA1c همبستگی منفی معناداری مشاهده گردید. نتیجه گیری: آزمون توالی الگوی دیرشی یک آزمون حساس به ضایعات کورتیکال می باشد که نتایج حاصل از آن در پژوهش حاضر نشان می دهد افراد مبتلا به دیابت نوع 1 در آزمون الگوی دیرشی نتایج ضعیف تری نسبت به افراد گروه کنترل دارند که این مسئله می تواند به نوعی بیانگر وجود نقص در توانایی پردازش زمانی در این افراد باشد.

هدف: زمان بندی های مختلف تمرینات کششی می تواند تاثیرات متفاوتی بر عملکرد جسمانی افراد سالمند داشته باشد. هدف پژوهش حاضر بررسی تاثیر دو برنامه تمرین کششی- تعادلی با زمان بندی مختلف بر برخی از عملکردهای جسمانی در مردان مسن بود. روش بررسی: 60 مرد سالمند با دامنه سنی 74-60 سال، به صورت تصادفی به پنج گروه مساوی شامل 1) تمرینات کششی - تعادلی با برتری پویا در مدت60 ثانیه، 2) تمرینات کششی - تعادلی با برتری ایستا در مدت60 ثانیه، 3) تمرینات کششی - تعادلی با برتری پویا در مدت90 ثانیه، 4) تمرینات کششی - تعادلی با برتری ایستا در مدت90 ثانیه و 5) کنترل تقسیم شدند. برنامه تمرینی شامل حرکات کششی ترکیبی (ایستا و پویا) و حرکات تعادلی بود که سه بار در هفته در مدت هشت هفته انجام شد. داده ها با استفاده از روش های آماری کلموگروف- اسمیرنوف، آنالیز واریانس یک طرفه و تی وابسته در سطح معناداری 0/05≥p تحلیل شدند. یافته ها: نتایج نشان داد شاخص های قدرت عضلانی پایینتنه، استقامت عضلانی، انعطاف پذیری بالاتنه و پایینتنه، چابکی، تعادل ایستا و تعادل پویا در اکثر گروه های تجربی در مقایسه با گروه کنترل به طور قابل توجهی بهبود یافت. اما برنامه های تمرینی مختلف بر قدرت عضلانی بالاتنه و عملکرد استقامتی تاثیری نداشتند. نتیجه گیری: می توان گفت مردان مسن برای بهبود شاخص های آمادگی جسمانی و حرکتی بهتر است از برنامه های تمرینی گروه یک (تمرینات کششی-تعادلی با غالب پویا به مدت 60 ثانیه) و گروه چهار (تمرینات کششی-تعادلی با غالب ایستا در مدت 90 ثانیه) استفاده کنند.

بررسی ویژگی های روان سنجی آزمایه تصویری بیان افعال در کودکان کم شنوای شدید فارسی زبان
هدف: مقوله فعل از دیرباز یکی از پیچیده ترین بخش پژوهش های مربوط به دستور زبان در زبان فارسی به شمار می رود از طرفی کاهش شنوایی اثرات متفاوتی برمیزان گفتار افراد و عملکردهای آنان دارد. بررسی روایی محتوایی و ثبات درونی آزمایه تصویری بیان افعال فارسی در کودکان 4 الی 5 ساله کم شنوای شدید هدف پژوهش حاضر می باشد. روش بررسی: در این مطالعه مقطعی، ابتدا فهرستی از افعال روزمره و پر بسامد زبان فارسی انتخاب شد و فهرست انتخابی به همراه تصاویر آن در پانل کارشناسی متشکل از اساتید گفتاردرمانی دانشگاه از نظر مناسب بودن تصاویر و افعال انتخابی از نظر پر بسامد بودن مورد بررسی قرار گرفت. به منظور روایی محتوایی آزمایه، مورد قضاوت 14 نفر از آسیب شناسان گفتار و زبان قرارگرفت و روایی با استفاده از روش Lawshe مورد سنجش قرار گرفت. سپس 17 کودک پنج و شش ساله کم شنوای شدید (9 دختر و 8 پسر) که با معیارهای ورود و خروج پژوهش مطابقت داشتند، شناسایی شدند. پس از انتخاب نمونه ها و اجرای آزمون، ثبات درونی آزمایه تصویری بیان افعال کودکان کم شنوا 5 الی 6 ساله با استفاده از روش آلفای کرونباخ محاسبه شد. یافته ها: با بررسی پاسخ های 14 نفر از گفتاردرمانگرها مشخص شد که در کل 86/8% از افعال آزمایه از روایی محتوایی بالاتر از 51% برخوردار هستند به همین منظور 5 فعل از 38 فعل آزمایه حذف گردید و ارزش ثبات درونی 69/69% از گویه های آزمایه در بازه ارزشی 0/3 تا 0/7 قرار می گیرند. نتیجه گیری: آزمایه بیان افعال تصویری در کودکان کم شنوای فارسی زبان از روایی محتوایی و ثبات درونی نسبتاً مناسبی برای ارزیابی بیان افعال کودکان کم شنوا برخوردار است.

اثر کفی حمایت کننده قوس بر نیروی های عکس العمل زمین، ضربه و نرخ بارگذاری هنگام فرود دو پا
هدف: مقدار و نحوه توزیع سه بعدی نیروی عکس العمل زمین هنگام فرود با آسیب های مچ پا و زانو مرتبط است. کفی کفش در تعدیل نیروهای عکس العمل زمین موثر است. هدف از این مطالعه بررسی اثر استفاده از کفی آنتی پرونیشن هنگام فرود در مقادیر نیروی عکس العمل، ضربه و نرخ بارگذاری هنگام فرود دوپا بود. روش بررسی: 13 مرد سالم به ترتیب با میانگین سن، جرم و قد (1/6±21/9 سال)، (12/6±67/15 کیلوگرم) و (4/9±175/38 سانتی متر) در این مطالعه شرکت نمودند. با استفاده از یک صفحه نیرو (1000 هرتز) مولفه های نیروی عکس العمل زمین هنگام فرود در دو شرایط با و بدون استفاده از کفی اندازه گیری شد. سپس متغیرهای اوج نیروهای عکس العمل زمین و زمان رسیدن به آن ها، ایمپالس و میزان بار وارد شده استخراج شدند. آزمون آماری آنالیز واریانس اندازه های تکراری با سطح معناداری (0/05˂p) جهت تحلیل آماری مورد استفاده قرار گرفت. یافته ها: پوشیدن کفی موجب کاهش اوج ثانویه مولفه عمودی عکس العمل زمین (22%) در لحظه تماس پاشنه با زمین (0/006=p)، کاهش نرخ بارگذاری عمودی (23%) (0/01=p) و همچنین کاهش ضربه عمودی (14 درصد) (0/032=p) شد. نتیجه گیری: به نظر می رسد استفاده از کفی طبی میتواند با کاهش در نیروی عکس العمل عمودی، نرخ بارگذاری و ضربه از ریسک آسیب مفاصل اندام تحتانی هنگام فرود جلوگیری کند.

بررسی ارتباط دید رنگ و میزان سرب خون کارکنان معدن سرب و روی کوشک شهرستان بافق
هدف: بررسی وضعیت دید رنگ و میزان سرب خون کارکنان معدن سرب و روی کوشک شهرستان بافق. روش بررسی: این مطالعه مقطعی در معدن سرب و روی کوشک شهرستان بافق که تمامی افراد شرکت کننده مرد بودند، انجام شد. افراد شرکت کننده به دو گروه دارای مواجهه (114 نفر) و گروه بدون مواجهه (76 نفر) تقسیم شدند. با در نظر گرفتن معیارهای خروج 190 نفر با رضایت کامل وارد مطالعه شدند. معاینات کامل اپتومتری و بررسی دید رنگ برای تمام افراد انجام گرفت. دید رنگ با استفاده از تست D15-Fransworth بررسی شد. مقدار سرب خون افراد با توجه به برگه آزمایش خون موجود در پرونده ثبت شد. یافته ها: درصد فراوانی نقص دید رنگ اکتسابی در گروه کنترل 2/72 درصد و در گروه آزمودنی 11/55 درصد بود. تفاوت میانگین مقدار ضریب نقص دید رنگ بین دو گروه در چشم راست (مورد 0/283±1/169، کنترل 0/17±1/07) و چشم چپ (مورد 0/282±1/168، کنترل 0/14±1/06) معنادار بود (چشم راست 0/011=p، چشم چپ 0/002=p). نتایج بدست امده میانگین سرب خون در گروه مورد و کنترل را به ترتیب 15/81±10/07 و 9/03±6/46 میکروگرم بر دسی لیترنشان داد که این تفاوت بین دو گروه از لحاظ آماری معنی دار بود (0/047=p). نتیجهگیری: تفاوت موجود در نقص دید رنگ گروه دارای مواجهه و بدون مواجهه ممکن است بیانگر این موضوع باشد که تماس طولانی مدت با مواد معدنی در معدن سرب و روی میتواند باعث بروز نقص دید رنگ اکتسابی شود و با توجه به تفاوت معنادار میانگین سرب خون دوگروه یکی از عوامل ایجاد این اختلال ممکن است سرب باشد.

مطالعه فعالیت الکتریکی عضلات ساق پا در مردان ورزشکار مبتلا به Shin Splints
هدف: ثبت فعالیت الکتریکی عضلات می تواند اطلاعاتی در خصوص میزان فعال شدن عضلات ساق در افراد مبتلا به شینShin Splints بدهد، بنابراین هدف از مطالعه حاضر ارزیابی فعالیت الکتریکی عضلات ساق پا در مردان ورزشکار مبتلا به Shin Splints بود. روش بررسی: تعداد 30 دانشجوی تربیت بدنی در این تحقیق شرکت کردند.فعالیت الکتریکی عضلات درشت نئی قدامی و دوقلو (داخلی و خارجی) به ترتیب در وضعیت Dorsiflexion و Plantar flexion با استفاده از دستگاه الکترومایوگرافی Biovesion اندازهگیری و ثبت شد. نتایج با استفاده از آزمون t student در سطح معناداری 0/05≥p مورد آزمون قرار گرفت. یافتهها: نتایج تحقیق نشان داد که تفاوت معنی داری در فعالیت الکتریکی عضلات درشت نئی قدامی (0/054=p)، دوقلو داخلی (0/07=p) و خارجی (0/11=p) بین افراد مبتلا به Shin Splints و سالم وجود نداشت. اما تفاوت معنی داری در نسبت فعالیت الکتریکی عضله درشت نئی قدامی به عضلات دوقلو مشاهده شد (0/01=p). نتیجه گیری: نتایج تحقیق حاضر نشان داد که عدم تعادل عضلات درشت نئی قدامی نسبت به دوقلو ممکن است عاملی برای بروز Shin Splints باشد. بنابراین شاید با تقویت عضلات درشت نئی قدامی بتوان به کاهش درد افراد مبتلا به Shin Splints کمک نمود.

الزامات داده ای و قابلیت های فنی سامانه خودمدیریتی مبتنی بر موبایل برای افراد مبتلا به ویروس نقص ایمنی انسانی
هدف: در سال های اخیر شبکه های تلفن همراه و ابزار های هوشمند به فناوری های مهم در ارائه مراقبت سلامت در سراسر جهان تبدیل شده اند. این فناوری ها به منظور کنترل سلامت از راه دور و خود مدیریتی افراد مبتلا به بیماری های مزمن مانند HIV استفاده میشوند. مطالعه حاضر، یک پژوهش توصیفی- توسعه ای بود که به تعیین الزامات داده ای و قابلیت های فنی یک سامانه هوشمند خودمدیریتی مبتنی بر موبایل برای افراد مبتلا به HIV میپردازد. روش بررسی: در این پژوهش در مرحله اول، با جستجوی منابع اطلاعاتی از قبیل PubMed, Scopus, Science Direct, Web of Science الزامات و محورهای سامانه خودمدیریتی مبتنی بر موبایل برای افراد مبتلا به HIV به دست آمد. در مرحله دوم، از متخصصین عفونی و مدیریت اطلاعات سلامت در مورد الزامات سیستم مورد نظر، در قالب پرسشنامه محقق ساخته نظر خواهی شد. یافته ها: سه دسته از الزامات شامل عناصر داده ای دموگرافیکی و بالینی و قابلیت های فنی برای طراحی سیستم تعیین شد. یادآوری مصرف دارو، ارائه پیام های آموزشی، یادآوری رژیم غذایی، الزامات امنیتی، دسترسی به اینترنت و یادآوری ویزیت های حضوری از جمله مهمترین الزامات فنی سیستم خود مدیریتی مبتنی بر موبایل برای افراد مبتلا به HIV بودند. همچنین، عناصر داده ای برای قسمت بالینی سیستم به ترتیب اولویت شامل مواردی از قبیل تب، کاهش وزن، اسهال، بی اشتهایی، درد مفاصل و بروز اولین علائم بود. نتیجه گیری: الزامات مورد نیاز برای طراحی و اجرای سامانه هوشمند خود مدیریتی مبتنی بر موبایل برای حمایت از مراقبت از افراد مبتلا به HIV به صورت پایهای به دست آمد. با استفاده از این الزامات، طراحی معماری سیستم خود مدیریتی با ویژگی هوشمند از قبیل قابلیت ارائه طرح مراقبت خاص برای هر فرد به صورت منحصر به فرد امکانپذیر خواهد بود. استفاده از این سیستم، می تواند مهارت های خود مدیریتی بیماران را بهبود و ارتباط آن ها با ارائه دهندگان مراقبتی سلامت را تسهیل بخشد.

موارد آسیب سیستم دهلیزی در افراد کم شنوایی حسی عصبی
هدف: در بیماری های مختلف سیستم شنوایی ممکن است آسیب سیستم دهلیزی نیز مشاهده گردد. در این مقاله، در نظر داریم که به بررسی مقالات مرتبط با رابطه آسیب سیستم شنوایی و سیستم دهلیزی با تاکید بر عملکرد اندام اتولیت بپردازیم. روش بررسی: مقالات مرتبط با رابطه آسیب سیستم شنوایی و سیستم دهلیزی در پایگاه های PubMed،Web of Science و Scopus مورد بررسی قرار گرفتند و مقالاتی مورد استفاده قرار گرفتند که از آزمون های مناسبی برای بررسی سیستم دهلیزی استفاده کرده بودند. یافته ها: نتایج آزمون های متعدد سیستم دهلیزی نشان دهنده وجود آسیب در موارد کم شنوایی حسی عصبی بود اما میزان درصد افراد دارای آسیب بسته به اندام دهلیزی مورد ارزیابی و آزمون مورد استفاده متفاوت بود. نتیجه گیری: بین آسیب سیستم شنوایی و آسیب سیستم دهلیزی رابطه وجود دارد. در موارد کم شنوایی حسی عصبی مناطق مختلف سیستم دهلیزی دچار آسیب می گردند اما این آسیب بیشتر در مناطق اندام های اتولیت و مخصوصا ساکول قابل مشاهده می باشد که یکی از دلایل مطرح شده برای این موضوع می تواند مجاورت فیزیکی این عضو به ساختار های شنوایی و مخصوصا حلزون باشد.

درمان یک مورد کمردرد مزمن غیر اختصاصی به کمک تمرینات تنفسی و Visceral Manipulation
هدف: گزارش موردی از درمان موفق یک مورد کمردرد مزمن غیراختصاصی شدید همراه با کاهش تحرک پذیری احشای شکمی و با تغییرات الگوی تنفسی یافته های بالینی: خانمی 33 ساله با کمردرد مزمن غیر اختصاصی، با کاهش تحرک پذیری احشای شکمی، وجود Tenderness، درد در ناحیه راست شکم، نواحی دیافراگم لگنی، تنفسی و ناتوانی در تنفس دیافراگماتیک. شدت درد بر اساس معیار (Visual Analogue Scale; VAS) برابر با 10 گزارش شد. MRI وجود پروتروژن دیسک L5-S1 را نشان داد. تشخیص: درد ناشی از گرفتگی های احشای شکمی. درمان: چهار جلسه Visceral Manipulation به همراه ارائه تمرینات تنفسی دیافراگماتیک و تمرینات کششی عمومی. نتایج: درد بیمار در پایان جلسه اول به اندازه 80 درصد کاهش یافت. در پایان جلسه دوم بیمار قادر به تنفس دیافراگماتیک بود. شدت درد بر اساس VAS، 3 گزارش شد که اندکی افزایش درد را در این جلسه نشان داد. پس از جلسه سوم درد فقط در زمان بیدار شدن حس شد.
