اثربخشی بازیهای زبانشناختی بر بهبود زبان بیانی و دریافتی کودکان آسیب دیده شنوایی
هدف: نقص شنوایی یکی از مهمترین دلایل تأخیر زبانی در کودکان است که جنبه های مختلف تولید و درک زبان را تحت الشعاع قرار می دهد. رشد و ارتقای سطح مهارتهای زبان بیانی و زبان دریافتی در کودکان کم شنوا مستلزم به کارگیری برنامه های آموزشی و توانبخشی مناسب است. این پژوهش با هدف بررسی تأثیر بازیهای زبان شناختی بر رشد این مهارتها در کودکان کم شنوای پیش دبستانی انجام شده است. روش بررسی: روش بررسی در این پژوهش، مطالعه مداخله ای از نوع شبه تجربی با طرح پیش آزمون - پس آزمون با گروه شاهد است. مشارکت کنندگان 10 کودک کم شنوای سمعکی با دامنه هوشی طبیعی بودند که به شیوه در دسترس انتخاب و در دو گروه تقسیمبندی شدند (5 کودک در هر گروه)، برنامه بازیهای زبانشناختی برای گروه آزمایش ارایه شد و میزان مهارتهای زبان دریافتی و مهارتهای زبان بیانی تمامی کودکان در پیش و پس از مداخله با استفاده از آزمون رشد زبان- ویراست سوم ارزیابی شد. یافته ها: با مقایسه میانگین مهارتهای زبان بیانی و زبان دریافتی در گروه آزمایش و شاهد از طریق آزمون U مان- ویتنی مشخص شد تفاوت میانگین گروهی که بازیهای زبان شناختی را دریافت نمودهاند در مهارتهای زبان بیانی (0/009=p)، و در مهارتهای زبان دریافتی (0/009=p)، به لحاظ آماری معنادار است. بدین معنی که آموزش بازیهای زبان شناختی بر بهبود این مهارتهای زبانی در کودکان کمشنوا مقطع پیشدبستانی که مداخله را دریافت کرده بودند، اثربخش بوده است. نتیجه گیری: کودکان کم شنوا همواره با مشکلاتی در تولید و درک زبان مواجه هستند. این امر مهمترین دلیل برای برنامه ریزی های آموزشی در جهت یافتن و معرفی روشهای نوین می باشد. با توجه به یافته های پژوهش حاضر می توان نتیجه گرفت که آموزش مهارتهای زبان بیانی و زبان دریافتی به روش بازی درمانی با رویکرد بازیهای زبان شناختی می تواند مشکلات مربوط به این بخش از زبان کودکان پیش دبستانی کم شنوا را بهبود بخشد. پس از جلسات بازیهای زبان شناختی، این کودکان در درک معنای جملات و کلمات پیشرفت قابل ملاحظه ای داشتند. آنها همچنین قادر به شناختن، فهمیدن و به کارگیری اشکال تکواژشناختی و تولید و درک تمایزات بین صداهای گفتاری زبان فارسی خواهند بود.

تاثیر تمرینات ترکیبی بر سطوح فعالیت آنزیم پارااکسوناز١ و برخی شاخصهای لیپیدی پلاسما درخون زنان بدون فعالیت ورزشی
هدف: هدف از این مطالعه بررسی اثر تمرینات ترکیبی بر فعالیت سرم پارااکسوناز١ و شاخصهای لیپوپروتئین پلاسما در زنان چاق (بدون فعالیت ورزشی) بود. روش بررسی: بیست و چهار زن سالم بدون فعالیت ورزشی داوطلب برای شرکت در این پژوهش شدند که به طور تصادفی در دو گروه مساوی هدف (١٢ نفره) و گروه شاهد (١٢ نفره) قرار گرفتند. مدت زمان تمرینات ٤ هفته، هر هفته ۳ جلسه و هر جلسه به مدت 55-70 دقیقه به طول انجامید. تمرینات گروه ترکیبی با ٦٠ تا٨٠ درصد حداکثر ضربان قلب ذخیره انجام شده است. متغیرهای وابسته در دو مرحله از مطالعه، از جمله پیش آزمون و پس آزمون اندازهگیری شد. یافته ها: نتایج نشان دادند که افزایش معنیداری در فعالیت آنزیم پارااکسوناز١ مشاهده شد (0/01=p). اما شاخص لیپیدی این تحقیق از جمله (0/414=p=0/22) ،LDL، (p HDL، (p=0/152) VLDL و کلسترول (0/156=p) تغییر معنیدار نداشت. از سوی دیگر کاهش معنیداری در غلظت تریگلیسرید خون مشاهده شد (0/001=p). علاوه بر این، در زنان چاق بدون فعالیت ورزشی تغییرات سطوح پارااکسوناز١ با HDL ارتباط معنیداری داشت (0/02=p) و با دیگر شاخصهای لیپیدی این پژوهش رابطهای نداشت (سطح معنیداری 0/05 ≥ p) نتیجهگیری: نتایج تحقیق نشان داد که بعد چهار هفته تمرینات ترکیبی تغییرات مطلوبی بروی چربی و وزن را به همراه داشت و همچنین موجب افزایش معناداری در آنزیم پارااکسوناز١ و کاهش معنیداری غلظت تریگلیسرید خون داشت. اما به نظر میرسد که تغییرات آنزیم پارااکسوناز١ در زنان چاق بدون فعالیت ورزشی با HDL رابطه معنیداری داشت و با دیگر شاخصهای لیپیدی این پژوهش رابطهای نداشت. بنابراین این برنامه تمرینی میتواند در زنان چاق بدون فعالیت ورزشی با مهار اکسیداسیون LDL و با محدود کردن گردش HDL در خون با هدف پیشگیری از بیماریهای قلبی- عروقی تاثیرگذار باشد. سرانجام در انسانها ورزش میتواند با افزایش PON1 موجب افزایش سیستم آنتی اکسیدانی کل بدن شود.

اثر تنظیم تقارن وزن بر عملکرد تعادل و راه رفتن در بیماران سکته مغزی مزمن
هدف: هدف این مطالعه بررسی تاثیر تحمل وزن اجباری بهمراه تمرین درمانی بر آزمونهای عملکردی تعادل و راه رفتن در بیماران سکته مغزی بود. روش بررسی: 32 بیمار همی پارزی در گروههای تجربی و کنترل (16 بیمار در هر گروه) در این مطالعه شرکت نمودند. بیماران گروه تجربی کفشهایی را که یک کفی به ضخامت 6 میلیمتر در سمت سالم به منظور افزایش تقارن وزن جاسازی شده بود بمدت شش هفته پوشیدند و روزانه 90 دقیقه تمرینات بازآموزی تعادل و راه رفتن را انجام دادند. بیماران گروه کنترل فقط تمرینات درمانی را دریافت کردند. ارزیابیها شامل درصد تحمل وزن بر اندام مبتلا، سرعت راه رفتن، طول گام، آزمون (Timed up and go; TUG) و آزمون (Berg Balance Scale; BBS) بود که قبل و پس از درمان و 3 ماه پس از خاتمه درمان در هر دو گروه ثبت شدند. یافته ها: بررسیهای درون گروهی نشان داد که در هر دو گروه، کلیه متغیرها در پایان درمان بهبودی معنی داری نشان دادند و اثرات بهبودی در دوره پیگیری حفظ شد. در گروه تجربی، درصد تحمل وزن بر اندام مبتلا در دوره پیگیری نیز افزایش معنیدار داشت. مقایسه یافتههای بین گروهی نشان داد که درصد تحمل وزن بر اندام مبتلا و آزمون BBS افزایش بیشتری در گروه تجربی نسبت به گروه کنترل داشتند (0/001=p). نتیجه گیری: تحمل وزن اجباری بر اندام مبتلا به همراه بازآموزی تعادل و راه رفتن باعث بهبودی بیشتر تقارن وزن و تعادل عملکردی در بیماران سکته مغزی مزمن شد.

ارتباط بین ضخامت های مرکزی قرنیه و لایهی فیبر عصبی شبکیه با عیوب انکساری
هدف: بررسی ارتباط بین ضخامت مرکزی قرنیه و ضخامت لایهی فیبر عصبی شبکیه در افراد امتروپ، مایوپ، هایپروپ. روش بررسی: 180 چشم (64 چشم امتروپ،70 چشم مایوپ و 46 چشم هایپروپ) در محدودهی سنی 35-18 سال، به صورت تصادفی انتخاب شدند. افراد شرکت کننده در طرح دارای سلامت عمومی چشم و عدم سابقه ی جراحی چشم بودند. الگوی پاکیمتری آنان در بخش مرکزی قرنیه و همچنین پارامترهای شبکیه با استفاده از دستگاه OCT Optovue (Optovue Optical Coherence Tomography) اندازه گیری گردید. یافتهها: میانگین ضخامت قرنیه در گروه امتروپ 33/76±536/14، مایوپ 33/05±539/67 و در هایپروپها 43/40±543/28 میکرون (p<0/005) بود. ضخامت لایه فیبر عصبی رتین (RNFL) در گروه مایوپها 17/27±114/21 و امتروپ 22/84±121/05 و هایپروپ 21/22±120/45 میکرون بود (p<0/005). در تحلیل تک متغیری، در گروه مایوپی و هایپروپی، ضخامت مرکز قرنیه با ضخامت RNFL، ارتباط مثبت دارد. نتیجهگیری: وجود ارتباط مثبت بین ضخامت مرکزی قرنیه و لایه فیبر عصبی شبکیه (RNFL) در گروههای هایپروپ و مایوپ نشان میدهد که افراد با قرنیه های ضخیمتر، RNFL ضخیمتری دارند. در چشمها با مایوپی بیشتر، انتظار RNFL نازکتر و در چشمها با هایپروپی بیشتر انتظار RNFL ضخیمتر را داریم. ضخامت مرکزی قرنیه (CCT) با میزان عیوب انکساری رابطه ی معنی داری از نظر آماری نداشت در حالی که ضخامت لایه فیبر عصبی رتین با تغییر عیوب انکساری مرتبط است (p<0/001).

بررسی فرکانس سه سازه اول واکه های زبان فارسی در افراد مبتلا به پولیپ صوتی
هدف: این پژوهش با هدف مقایسه فرکانس فرمنتهای واکهها در افراد مبتلا به پولیپ صوتی و افراد طبیعی در کشیده گویی واکهها انجام شده است. روش بررسی: پژوهش حاضرکمّی است که از نوع توصیفی- تحلیلی بوده که به صورت مقطعی انجام شده است. متغیرهای مورد بررسی شامل: میانگین فرمنت های اول و دوم و سوم 6 واکهی زبان فارسی است. این پارامترها در افراد مبتلا به پولیپ تارهای صوتی و افراد طبیعی مقایسه شد. افراد از گروه سنی 20 تا40 سال انتخاب شدند. نمونههای مورد بررسی شامل 10 فرد با صوت طبیعی ( 5 زن و 5 مرد) و 10 فرد مبتلا به پولیپ صوتی (5 زن و 5 مرد) بودند. یافتهها: در مردان و زنان مبتلا به پولیپ صوتی بیشترین و کمترین مقدار فرمنت اول(F1 ) مربوط به واکه /a/ و /u/ است. بیشترین و کمترین مقدار فرمنت دوم (F2) در هر دو جنس مربوط به واکههای /e/ و/u/ است و بیشترین و کمترین مقدار فرمنت سوم (F3) مربوط به واکههای /u/ و /e/ است. سازه اول و دوم واکههای فارسی در صوت افراد طبیعی و افراد مبتلا به پولیپ صوتی متفاوت است (0/028=p) که این مسئله در مورد سازه سوم مطرح نیست (0/59=p) نتیجهگیری: توجه به سازهها در تشخیصگذاری سودمند است و فرمنت اول و دوم می توانند در ارزیابیهای ابزاری بیشتر مورد توجه قرار گیرند.

تأثیر تمرینات متقاطع بر قدرت عضلات چهارسررانی پس از بازسازی رباط صلیبی قدامی
هدف: پس از بازسازی رباط صلیبی قدامی قدرت عضلات چهارسررانی به میزان زیادی کاهش پیدا میکند. پیدا کردن یک راهحل برای جلوگیری از کاهش قدرت دارای اهمیت زیادی است. هدف این پژوهش بررسی تأثیر هشت هفته تمرینات متقاطع بر قدرت عضله چهارسررانی مردان پس از بازسازی رباط صلیبی قدامی بود. روش بررسی: تعداد20 بیمار با میانگین سنی 1/55±22/02 سال و میانگین وزنی 7/15±71/7 کیلوگرم که دچار پارگی رباط صلیبی قدامی شده بودند، انتخاب و به دو گروه تجربی و کنترل تقسیم شدند. گروه تجربی علاوه بر تمرینات فیزیوتراپی تمرینات متقاطع را با پای سالم به مدت هشت هفته، هر هفته سه جلسه، هر جلسه 20 دقیقه و گروه کنترل فقط تمرینات فیزیوتراپی خود را تمرینات را انجام دادند. سه روز قبل از عمل بازسازی رباط و بعد از گذشت 9 هفته و انجام 24 جلسه تمرینات متقاطع، قدرت ایزومتریک عضلات چهارسررانی هر دو پای بیماران در زاویه 60 درجه، با استفاده از دستگاه ایزوکنتیک بایودکس اندازهگیری شد. دادهها با استفاده از آزمون T Student در سطح معنی داری 0/05>p آزمایش شدند. یافته ها: نتایج نشان داد به دنبال هشت هفته تمرینات متقاطع کانسنتریک، قدرت عضله چهارسررانی پای سالم آزمودنی های گروه تجربی به طور معناداری افزایش یافت(0/002=p). همچنین میزان کاهش قدرت در عضلات چهارسررانی افراد پس از بازسازی ACL، در گروه تجربی نسبت به گروه کنترل به طور معناداری کمتر بود (0/001=p). نتیجه گیری: تمرینات متقاطع بصورت کانسنتریک می تواند اثرات مثبتی در جهت کاهش کمتر میزان قدرت داشته باشد. این تمرینات می تواند باعث افزایش قدرت در اندام سالم و همچنین جلوگیری از میزان کاهش زیاد قدرت در اندامی که مورد جراحی قرار گرفته شود.

بررسی کمی اطلاعات پرونده پزشکی بیماران بستری در بیمارستان های دانشگاهی و تامین اجتماعی شهر مشهد
هدف: پرونده پزشکی باید به صورت کامل و با توجه به فرآیند مراقبتی بیمار مستند گردد تا به منزله یک سیستم مادامالعمر (قبل از تولد تاپس از مرگ) عمل نماید. پرونده ناقص انعکاسدهنده مراقبت و درمان ناقص است. اطلاعات پرونده پزشکی وقتی سودمند است که در همان بار اول به دقت و درست ثبت شده باشد. هدف این مطالعه بررسی کمی اطلاعات پرونده پزشکی بیماران بستری در بیمارستانهای دانشگاهی و تامین اجتماعی شهر مشهد بوده است. روش بررسی: این پژوهش یک مطالعه توصیفی- مقطعی است. جامعه پژوهش شامل پرونده بیماران بستری در بیمارستان-های دانشگاهی و تامین اجتماعی شهر مشهد بود که مجموعاً 550 پرونده انتخاب گردید. تعداد نمونهها متناسب با تعداد پروندههای بیماران بستری هر بیمارستان انتخاب گردید. پس از بررسی روایی و پایایی ابزار (چک لیست)، دادههای مورد نیاز جمعآوری و وارد رایانه شد و با نرم افزار آماری SPSS تحلیل گردید. یافتهها: در میان پروندههای مورد بررسی، برگ پذیرش و خلاصه ترخیص فقط در یک مورد (0/01 درصد) در پروندهها وجود نداشت و بیشترین مورد نقص مربوط به برگ کنترل علائم حیاتی بود (34/5 درصد). بیشترین میزان نقص در میان عناصر اطلاعات بالینی برگ پذیرش و خلاصه ترخیص مربوط به توصیههای پس از ترخیص (89/5 درصد) و ثبت نتایج آزمایشات و رادیوگرافیها (88/9 درصد) بود. در اکثر موارد اختلاف معنیداری در تکمیل عناصر اطلاعاتی فرمهای پروندههای مورد بررسی در بیمارستانهای دانشگاهی و تأمین اجتماعی وجود داشت. نتیجهگیری: نتایج این بررسی نشان داد که فرآیند مستندسازی پروندههای پزشکی توسط ارائهکنندگان مراقبت بسیار ناقص انجام میگیرد و این امر موجب از دست رفتن اطلاعات مورد نیاز و خدشهدار شدن ظرفیتهای موجود در پرونده پزشکی بیماران برای استفاده اشخاص حقیقی و حقوقی خواهد شد. بنابراین توصیه میگردد آموزشهای لازم در زمینه مستندسازی کامل و صحیح پرونده به تمامی اعضای تیم مراقبت داده شود و اجرای تحلیل کمی اطلاعات پرونده ترجیحاً به صورت تحلیل همزمان توسط کارکنان بخش مدارک پزشکی انجام گیرد.

مقایسه تمرین درمانی در آب و ماساژ درمانی بر شدت درد و میزان ناتوانی مردان مبتلا به کمردرد مزمن غیراختصاصی
هدف: کمردرد از مهمترین معضلات بهداشتی افراد میباشد که طب مکمل مانند تمرین درمانی در آب و ماساژ در درمان آن نقش اساسی دارند. هدف این مطالعه مقایسه تأثیر دو روش تمرین درمانی در آب و ماساژ بر روی شدت درد و ناتوانی مبتلایان با کمردرد مزمن غیر اختصاصی بود. روش بررسی: در این مطالعه نیمه تجربی 24 بیمار مرد مبتلا به کمردرد مزمن غیراختصاصی در دامنه سنی 20-40 سال با میانگین سنی 27/23 به طور تصادفی به دو گروه تمرین درمانی در آب و ماساژ تحت درمان قرار گرفتند. مدت درمان 6 هفته و هر هفته 4 جلسه طول کشید. جهت ارزیابی شدت درد و ناتوانی افراد به ترتیب با پرسشنامه اوسستری و مقیاس عینی درد قبل و بلافاصله بعد از درمان استفاده شد. جهت تحلیل دادهها از آزمون تحلیل واریانس در سطح معنی داری (0/05=α) استفاده گردید. یافتهها: میانگین شدت درد و ناتوانی در گروه تمرین درمانی در آب در پیش آزمون به ترتیب 0/878±5/66 و 5/21±24/83 بود و در پس آزمون این نمرهها به 0/729±4/58 و 3/89±14/33 تغییر پیدا کرد. در گروه ماساژ نیز میانگین شدت درد و ناتوانی در پیش آزمون به ترتیب 0/668±5/41 و 3/45±23/166 بود؛ و در پس آزمون به 1/128± 3/00 و 4/10±16/50 تغییر پیدا کرد. بنابراین در هر دو گروه تغییرات معنیداری بوجود آمد. نتیجهگیری: ماساژ درمانی و تمرین درمانی در آب هر دو باعث کاهش شدت درد در بیماران مبتلا به کمردرد مزمن غیراختصاصی میشود، اما ماساژ نسبت به تمرین درمانی در آب اثرات بهتری در کاهش شدت درد ایجاد میکند.

هدف: تعیین میانگین قطر مردمک بوسیله دستگاه پنتاکم در جمعیت 40 تا 64 ساله شهر شاهرود روش بررسی: در فاز اول مطالعه بوسیله نمونهگیری خوشهای چند مرحلهای، 300 خوشه انتخاب شد. از 6311 نفر افراد دعوت شده تعداد 82/2% در مطالعه شرکت کردند. تمام افراد تحت معاینات اپتومتری و چشمپزشکی قرار گرفتند. قطر مردمک بدست آمده در این گزارش توسط دستگاه پنتاکم مورد تجزیه و تحلیل قرار گرفت، سابقه جراحیهای چشمی و معتبر نبودن دادههای بدست آمده توسط دستگاه پنتاکم به عنوان معیاری برای خروج دادهها از این آنالیز در نظر گرفته شد. یافته ها: میانگین قطر مردمک در افراد مورد مطالعه (3820 نفر)، 0/56± 2/74 (2/76-2/72 95CI%) میلیمتر و صدک 25%، 50%، 95% و 97/5% قطر مردمک در جمعیت حاضر به ترتیب 2/41، 2/67،3/60 و 3/85 میلیمتر بود. 95% دامنه نرمال قطر مردمک در این مطالعه از 1/63 میلمتر تا 3/85 میلیمتر بدست آمد. در مدل مولتیپل میانگین قطر مردمک در مردان تنگتر از زنان بود و با افزایش سن قطر مردمک به طور معنیداری کم میشود (0/001>p). قطر مردمک در افراد بدون عیب انکسار (امتروپ) به طور معنیداری از افراد نزدیکبین (میوپ) و افراد دوربین (هایپروپی) بیشتر بود (0/001>p) قطر مردمک در افراد با رنگ عنبیه قهوهای تیره بطور معنیداری کمتر بود(p=0/005). از9/43% افراد این مطالعه دارای آنیزوکوریا بودند (10/44-8/43 95CI%). نتیجهگیری: قطر مردمک در افراد نرمال توسط دستگاه پنتاکم نسبت به دستگاههای دیگر پایینتر و 85/3 میلیمتر به عنوان معیار قابل تامل جهت جراحیهای عیوب انکساری پیشنهاد میشود. همچنین باید توجه داشت که مردان دارای قطر مردمک تنگتری هستند و با افزایش سن قطر مردمک تنگتر میشود.

اثر تمرینات منتخب بر کارکرد عضلانی و ترکیب بدن در دختران کم توان ذهنی
هدف: پژوهش حاضر بررسی اثر یک دوره تمرینات منتخب بر کارکرد عضلانی و ترکیب بدن در دختران کم توان ذهنی بود. روش بررسی: 23 دختر کمتوان ذهنی (ضریب هوشی70-50) انتخاب شدند و به صورت تصادفی در دو گروه کنترل (10 نفر) و تجربی (13 نفر) قرار گرفتند. آزمودنیهای گروه تجربی به مدت 4 هفته (4 جلسه در هفته) به اجرای تمرینات منتخب با شدت 80-60 درصد حداکثر ضربان قلب پرداختند. قبل و بعد از دوره تمرینی، آزمونهای عملکرد عضلانی شامل تعادل ایستا، تعادل پویا، نیروی عضلانی و شاخص خستگی و همچنین شاخصهای ترکیب بدنی از همه آزمودنیها گرفته شد. دادهها با استفاده از روشهای آماری t مستقل و آنالیز کوواریانس در سطح 0/05 p≤ تحلیل شدند. یافتهها: نتایج نشان داد انجام تمرینات منتخب باعث افزایش معنادار تعادل ایستا با چشمان باز و بسته می شود اما بر شاخصهای تعادل پویا، نیروی عضلانی، شاخص خستگی عضلانی و ترکیب بدنی، تاثیر معناداری ندارد. نتیجهگیری: میتوان گفت دختران کمتوان ذهنی میتوانند برای بهبود تعادل ایستا و مقاومت به خستگی از این تمرینات منتخب استفاده کنند اما برای تعیین تاثیر این نوع تمرینات بر دیگر شاخصهای عملکرد عضلانی و ترکیب بدن به تحقیقات دیگری نیاز است.
