معرفی الگوریتم هماتولوژیکی و مولکولی در تشخیص کمبود فاکتور سیزده در آزمایشگاههای ایران
هدف: کمبود فاکتور سیزده یک اختلال خونریزی دهنده بسیار نادر است که در ایران شیوعی 12 برابری در مقایسه با جمعیت عمومی جهان دارد. الگوریتم استانداردی که توسط کمیته بینالمللی ترومبوز و هموستاز برای تشخیص دقیق و طبقهبندی کمبود فاکتور سیزده پیشنهاد شده است به دلیل عدم سرمایهگذاری مناسب و عدم راه اندازی آزمایشات در ایران، تمام بخشهای این الگوریتم نمیتواند برای بیماران ایرانی استفاده شود. این مطالعه قصد دارد با ارائه یک الگوریتم متناسب، به تشخیص دقیق و سریع بیماری بپردازد که منجر به کاهش هزینههای تشخیصی و درمانی برای این بیماری در ایران میگردد. روش بررسی: تمامی مقالات منتشر شده در زمینه کمبود فاکتور سیزده در ایران و جهان با استفاده از پایگاههای اطلاعاتی ,Science Direct Google Scholar ,PubMed مورد بررسی قرار گرفت. یافتهها: با توجه به امکانات موجود در کشور، استفاده از الگوریتم پیشنهادی این مقاله که براساس امکانات آزمایشگاهی، بررسی علائم بالینی و سابقه خانوادگی افراد طراحی شده است، منجر به تشخیص بموقع بیماران مبتلا به کمبود فاکتور سیزده با هزینهای بسیار کم می شود. نتیجهگیری: هر چند کمیته ISTH الگوریتم استانداردی برای تشخیص کمبود فاکتور سیزده ارائه داده است اما با توجه به هزینههای بالای این الگوریتم و نیز تعداد محدود بیماران مبتلا به کمبود فاکتور سیزده در ایران و سرمایه گزاری کم در این زمینه، امکان پیاده سازی آن در حال حاضر در کشور مقدور نیست. این مطالعه الگوریتمی را پیشنهاد میکند که با امکانات و شرایط کشور سازگاری داشته و قابل اجرا است. با اجرای این الگوریتم و ممکن سازی تشخیص بیماران دارای کمبود فاکتور سیزده، امید است نسبت مرگ و میر در بیماران ایرانی ناشی از این بیماری کاهش پیدا کند.

تأثیر فیت لنز تماسی سخت منفذدار بر انحناء، ضخامت مرکزی و حجم قرنیه
هدف: بررسی تغییرات قدرت انحناء، ضخامت و حجم قرنیه در بیماران کراتوکونوسی پس از تجویز لنزهای تماسی سخت RGP (Rigid gas permeable) روش بررسی: بیست و پنج چشم مبتلا به کراتوکونوس بدون سابقه استفاده از لنز سخت، توسط دستگاه پنتاکم مورد ارزیابی قرار گرفت و پارامترهای انحنای قرنیه ،ضخامت مرکزی قرنیه و حجم قرنیه، قبل و سه ماه پس از استفاده از لنز RGP آسفریک در دو سطح قدامی و خلفی قرنیه ثبت گردید. یافتهها: کاهش قدرت انحنای محور فلت سطح قدامی قرنیه به مقدار 0/43- دیوپتر (0/339=p)، کاهش قدرت انحنای محور استیپ سطح قدامی قرنیه به مقدار1/54- دیوپتر (0/358=p)، کاهش قدرت انحنای محور فلت سطح خلفی قرنیه به مقدار0/10- دیوپتر (0/121=p)، افزایش قدرت انحنای محور استیپ سطح خلفی قرنیه به مقدار0/02 دیوپتر (0/710=p)، کاهش ضخامت مرکزی قرنیه به مقدار3/43- میکرون (0/127=p) و 0/27 میلیمتر مکعب افزایش حجم قرنیه (0/331=p) سه ماه پس از استفاده از لنز RGP نسبت به قبل از آن، معنی دار نبود.. نتیجهگیری: عدم تغییرات معنیدار متغیرهای مورد مطالعه پس از سه ماه استفاده از لنزهایRGP میتواند مبین عدم پیشرفت کراتوکونوس در این مدت باشد. با توجه به چگونگی تغییرات مشاهده شده ضرورت انجام تحقیقات تکمیلی در چندین دوره پیگیری توصیه میگردد.

بررسی ارتباط شیب لگن و قوس کمری با قدرت عضلات کمربند کمری لگنی افراد ناهنجار خارجی مچ پا
هدف: راستای پا بر حرکت استخوانها و مفاصل پروگزیمال تأثیر میگذارد. طبق نظریه زنجیره کینماتیک، ضعف در عملکرد یک سگمان حرکتی میتواند سایر بخشهای حرکتی را نیز تحت تأثیر قرار دهد. لذا هدف از این مطالعه بررسی ارتباط شیب لگن و قوس کمری با قدرت عضلات کمربند کمری لگنی در افراد دارای ناهنجار خارجی مچ پا در وضعیت ایستاده بود. روش بررسی: 53 مرد دارای ناهنجار خارجی مچ پا با میانگین سنی 2/72±23/2، قد 6/76± 169 و وزن6/47± 67/2 در این مطالعه شرکت کردند. قدرت ایزومتریک عضلات با دینامومتر دستی، شیب لگن با شیب سنج، قوس کمری با استفاده از خطکش منعطف و انحراف خارجی مچ پا (Pronation) پا با استفاده از شاخص افت ناوی، ارزیابی شد. یافتهها: یافتههای تحقیق با استفاده از روشهای آماری همبستگی بین متغیرها تجزیه و تحلیل شد. بین قدرت ایزومتریک عضلات فلکسور ران و زاویه قوس کمری(0/59=p=0/003,r) و همچنین بین شیب لگن و زوایه قوس کمری (0/46=p= 0/012,r)، در افراد دارای ناهنجار خارجی مچ پا ارتباط معنیداری وجود داشت. بین سایر متغیرهای تحقیق ارتباط معنیداری وجود نداشت (0/05≤p). نتیجهگیری: ارتباط مثبت و معنیدار قدرت عضلات فلکسور ران با قوس کمری و همچنین شیب لگن با قوس کمری را میتوان یک استراتژی جبرانی وضعیتی در افراد دارای ناهنجاری Pronation مچ پا دانست.

ساخت آزمون تصویری واژگان درکی و بررسی روایی و پایایی آن برای کودکان طبیعی 71-30 ماهه فارسی زبان
هدف: ارزیابی واژگان از مهمترین اجزای شاخصهای غربالگری و ارزیابی زبان است. واژگان درکی، همبستگی بالایی با هوشبهر کلامی فرد دارد و معیاری برای پیشبینی موفقیتهای تحصیلی آینده کودک، به ویژه در زمینه خواندن می باشد. هدف این پژوهش ساخت آزمون تصویری واژگان درکی و بررسی روایی و پایایی آن برای کودکان طبیعی 71-30 ماهه فارسی زبان است. روش بررسی: این پژوهش از نوع ساخت آزمون و توصیفی – تحلیلی است که به صورت مقطعی انجام شده است. در این پژوهش،91 کودک طبیعی30 تا 71 ماهه فارسی زبان در 7 گروه سنی با فاصله زمانی 6 ماه، شرکت کردند. برای ساخت آزمون، کلمات از کتابها و پایان نامه های مرتبط با موضوع پژوهش، استخراج شدند، سپس برای هر کلمه یک تصویر انتخاب شد. برای بررسی روایی محتوایی تصویر منتخب برای هر کلمه، آزمون به همراه پرسشنامه در اختیار اساتید آسیبشناس گفتار و زبان قرار گرفت. در پایان، نسخه نهایی آزمون بر روی 7 گروه سنی اجرا شد. پایایی آزمون- باز آزمون و پایایی بین دو نیمه A وB با محاسبه ضریب همبستگیSpearman به دست آمدند. برای بررسی تفاوت در میانگین امتیازات آزمون بین گروههای مختلف سنی از آزمون آماری Kruskal-Wallis و برای بررسی رابطه جنسیت با نمره کل آزمون، از آزمون آماری t مستقل استفاده شد. یافته ها: نتایج این مطالعه نشان داد آزمون تصویری واژگان درکی، آزمونی روا و پایا است. شاخص روایی محتوایی آزمون برابر 1، ضریب همبستگی بین دو نیمه آزمون 0/91 و ضریب همبستگی بین آزمون و باز آزمون 0/81 است. همچنین نتایج حاصل از این مطالعه نشان می دهد که بین میانگین امتیازات کودکان در گروههای سنی مختلف تفاوت معنادار (0/000=p) وجود دارد و تفاوت معناداری( 0/08=p) در میانگین امتیازهای کودکان دختر و پسر مورد مطالعه وجود ندارد. نتیجه گیری: آزمون تصویری واژگان درکی می تواند تفاوت توانایی درک واژگان را در گروه های سنّی مورد مطالعه نشان دهد.

ارزیابی اثر پرتاب بر حس حرکت مفصل شانه غالب و غیرغالب ورزشکاران پرتابی
هدف: از آنجایی که حس حرکت مفصل از وضعیت کپسول و دامنه حرکتی مفصل گلنوهومرال تأثیرپذیر است و شل بودن قابل ملاحظه کپسول و دامنه حرکتی تغییریافته شانه پرتابی در ثبات داینامیک و عملکردی آنها تعیین کننده است، هدف تحقیق حاضر مقایسه اثر پرتاب بر حس حرکت مفصل شانه غالب و غیرغالب زنان بازیکن والیبالیست، بولینگکار و غیرورزشکار قبل از آسیب بود. روش بررسی: در این تحقیق مقطعی تحلیلی، 45 زن در دو گروه هدف والیبالیست و بولینگکار و یک گروه کنترل غیرورزشکار، در سنین 20 تا 30 سال تهران شرکت کردند و حس حرکت مفصل شانه آنان با دستگاه حرکت مداوم غیرفعال ارزیابی شد. برای وصف اطلاعات از آمار توصیفی نظیر میانگین و انحراف استاندارد و برای ارزیابی تکرارپذیری، ضریب همبستگی بکار برده شد. آزمون کلموگروف اسمیرنوف و آزمون لون، به ترتیب، برای بررسی نرمال بودن دادهها و همسان بودن واریانس گروهها، آزمون خی دو برای مقایسه حس حرکت شانه سمت غالب و غیرغالب و نیز برای مقایسه در بین گروههای بازیکنان والیبال، بولینگ و گروه کنترل غیرورزشکار (سطح معنیداری 0/05≥p) بکار گرفته شد. یافته ها: تفاوت خطای حس حرکت بین شانه غالب و غیرغالب در هر سه گروه، معنیدار نبود (0/33=p)؛ همچنین در مقایسه بین گروهی نیز خطای حس حرکت هر گروه نسبت به شانه همتای گروه دیگر تفاوت معنیداری نداشت (غیرورزشکار و والیبالیست 0/21=p، غیرورزشکار و بولینگکار 0/19=p، والیبالیست و بولینگکار 0/63=p). نتیجه گیری: شلی کپسول و دامنه حرکتی تغییریافته شانه غالب احتمالاً با ایجاد میکروترما، اثر منفی روی حس حرکت دارد. اما از طرف دیگر، حرکات چرخشی مکرر شانه غالب منجر به یادگیری ادراکی شده و بدین ترتیب احتمالاً باعث اثر مثبت افزایش دقت پردازش سیگنال و تقویت حس حرکت میگردد، که می تواند اثر منفی شلی کپسول و تغییر دامنه حرکتی بر حس حرکت را خنثی کند. احتمالاً، حرکات چرخشی مکرر در توانبخشی نیز می تواند از طریق ایجاد تطابقات عصبی-عضلانی اثر میکروترومایی که باعث اختلال در ثبات و درد شانه غالب میشود را جبران کند و این امر به نوع چرخش بالا یا زیر بازو نیز ارتباطی ندارد.

بررسی تأثیر طول و ساختارهای هجایی شبه کلمه ها بر بسامد ناروانی گفتار در بزرگسالان دارای لکنت
هدف: لکنت از شایعترین اختلالات گفتار و زبان است. سببشناسی لکنت از جنبههای گوناگونی مانند؛ زیستی، رشدی، عاطفی، یادگیری و زبانشناختی مورد بررسی قرار گرفته است. تحقیقات نشان میدهند برخی ویژگیهای زبانشناختی مانند طول و ساختارهجایی کلمه میتواند بسامد وقوع ناروانی در این اختلال را تحت تأثیر قرار دهند. بنابراین پژوهش حاضر با هدف تعیین تأثیر طول و ساختارهجایی شبه کلمه ها بر بسامد ناروانیها در بزرگسالان دارای لکنت انجام شده است. روش بررسی: پژوهش توصیفی- تحلیلی حاضر به صورت مقطعی در مورد 18 فرد بزرگسال دارای لکنت با دامنه سنی 20 تا 39 سال انجام شده است. بسامد ناروانیها از طریق درخواست از شرکت کنندگان برای خواندن فهرستی شامل 24 شبه کلمه مورد ارزیابی قرار گرفت. شبه کلمه ها بر اساس طول (دو و پنج هجایی) و ساختارهجایی (ساده و دشوار) انتخاب شدند. تجزیه و تحلیل داده ها توسط نرم افزار SPSS و با استفاده از آزمون آماری Paired T-test انجام شد. یافته ها: نتایج نشان دادند، بین بسامد ناروانی گفتار در گروههای دوهجایی و پنج هجایی فهرست شبه کلمه ها تفاوتها معنادار بود (0/000=p). بسامد ناروانی گفتار بین ساختارهای هجایی ساده و دشوار گروههای دو هجایی و پنج هجایی فهرست شبه کلمه ها دارای تفاوتها معنادار بودند (0/000=p). نتیجه گیری: یافته ها نشان م یدهند، برخی ویژگیهای زبان شناختی مانند طول و ساختارهجایی واژه عملکرد دستگاه حرکتی گفتار را در افراد بزرگسال دارای لکنت تحت تأثیر قرار می دهند.

بررسی کیفیت راه رفتن بیماران مبتلا به اسکولیوز ایدیوپاتیک بر اساس میزان انرژی مصرفی طی راه رفتن
هدف: اسکولیوز یکی از انواع ناهنجاریهای سیستم اسکلتی- عضلانی است که بر میزان انرژی مصرفی بیماران طی راه رفتن اثرگذار است. در مطالعات پیشین اکسیژن مصرفی افراد طی راه رفتن به عنوان معیاری از انرژی مصرفی مورد بررسی قرار گرفته است (oxymety). لذا هدف از مطالعه حاضر اندازهگیری میزان انرژی مصرفی بیماران اسکولیوزی در مقایسه با افراد سالم با ارزیابی ضربان قلب بود. روش بررسی: این مطالعه از نوع مشاهدهای (observational) بوده و آزمون به صورت مقطعی- مقایسهای (comparative cross sectional) انجام گرفت. در این پروژه 10 بیمار دچار اسکولیوز ایدیوپاتیک با راس انحنای واقع شده در زیر مهره چهارم پشتی (T4) و 10 فرد سالم فاقد انحراف جانبی ستون فقرات که از لحاظ متغیرهای مخدوش کننده با گروه بیمار تطبیق داده شده بودند مورد ارزیابی قرار گرفتند. به منظور تعیین مقدار انرژی مصرفی بیماران و افراد سالم هنگام راه رفتن از سیستم کنترل ضربان قلب (Heart rate monitoring) استفاده گردید. یافتهها: در این مطالعه دیده شد میزان انرژی مصرفی بیماران اسکولیوزی در راه رفتن نسبت به افراد سالم بیشتر است و این بیماران در مجموع زمان راه رفتن ضربان قلب بالاتری را داشتهاند. هر چند تفاوت در مقادیر ذکر شده از لحاظ آماری قابل توجه نبود (0/05<p). نتیجهگیری: نتایج این مطالعه نشان داد انحراف جانبی ستون فقرات تاثیر چندانی بر میزان انرژی مصرفی بیماران اسکولیوزی ندارد. ضمن آنکه سرعت راه رفتن افراد تحت تاثیر انحنای جانبی ستون فقرات قرار نمیگیرد (0/05<p).

مقایسه ارتباطات غیرکلامی کودکان کم شنوای استفاده کننده از کاشت حلزون و سمعک
هدف: بررسی اثر مهارتهای غیرکلامی در کودکان استفادهکننده از کاشت حلزون در مقایسه با کودکان با شنوایی طبیعی است که آیا این مهارت همانند گفتار، دارای تأخیر و اختلال هست و آیا عمل کاشت حلزون در بهبود این مهارتها تأثیری دارد. روش بررسی: پژوهش حاضر مطالعه ای مقایسه ای مورد- شاهدی بود. به روش نمونهگیری غیر تصادفی، 60 کودک دختر و پسر زیر سن هفت سال با شنوائی طبیعی، استفادهکننده از سمعک و کاشت حلزون بررسی شدند. آزمون مورد استفاده، آزمون اختلالات ارتباطی غیرکلامی Dyssemia بود. جهت تحلیل داده ها و مقایسه آنها از تحلیل واریانس Anova و آزمون Tukey استفاده شده است. یافته ها: تحلیلات آماری با خطای کمتر از یک درصد و 0/000>p نشان داد که مهارتهای ارتباطی در کودکان استفاده کننده از کاشت حلزون نسبت به کودکان با آسیب شنوایی شدید تا عمیق، خیلی بهتر بوده و همچنین از نظر ارتباطات اجتماعی نزدیک به کودکان با شنوایی طبیعی بودند. نتیجهگیری: با بهبود شنوایی، کودکان می توانند در معرض ارتباطات اجتماعی و آموزشها قرار گیرند. بنابراین، مهارتهای کلامی و غیرکلامی آنها بهبود خواهد یافت. همچنین پیشنهاد می شود والدین، کودک کم شنوای خود را بطور طبیعی همچون یک کودک شنوا در معرض محرکات کلامی و غیر کلامی مناسب قرار دهند.

آسیبهای مرتبط با سقوط در سالمندان کاشان
هدف: سقوط در سالمندان باعث انواع آسیبها شده و منجر به وابستگی، کاهش خودکارآمدی، افسردگی، محدودیت فعالیت روزانه، بستری در بیمارستان و یا پذیرش در خانه سالمندان و تحمیل هزینه به فرد و جامعه میشود. مطالعه حاضر با هدف تعیین نوع صدمات ناشی از سقوط در سالمندان کاشان انجام شده است. روش بررسی: در این مطالعه مقطعی، کلیه افراد60 سال و بالاتر که به علت سقوط در سال 1390 در بیمارستان شهید بهشتی کاشان بستریشده بودند، بررسی شدند. کدهای علل خارجیw00 تا w19 بر اساس ICD-10 مربوط به سقوط از سیستم اطلاعات سلامت بیمارستان انتخاب و پس از فراهم شدن دسترسی به پروندهها، متغیرهای دموگرافیک، علل خارجی سقوط و نوع آسیب استخراج گردید. دادهها با استفاده از نرمافزار SPSS تحلیل شد. ارتباط مدت بستری با متغیرهای زمینهای به وسیله آزمونهای کای اسکوئر؛ تی تست و آنووا و یا معادل ناپارامتریک آن بررسی شد. سطح معنیداری 05/0 در نظر گرفته شد. یافته ها: تعداد251 نفر سالمند60 سال و بالاتر مورد بررسی قرار گرفتند که تعداد 127 نفر 50/6% زن و124 نفر 49/4% مرد بودند. میانگین سن بیماران 7/3 ±76/82 سال بود. شایعترین علت خارجی، زمین خوردن در سطح همتراز با 196 نفر 78% بود. شکستگی اندامهای فوقانی با 109 مورد 44/1%، شکستگی اندامهای تحتانی با 73 مورد 28/9% و صدمات سربا40 مورد 15/9%، شایعترین نوع آسیب بود. میانگین مدت بستری 4/5± 5/68 روز بود. نتیجهگیری: فراوانترین آسیب به ترتیب شکستگیهای اندام فوقانی، اندام تحتانی و صدمات مغزی است که باعث بیشترین مدت بستری و بالاترین هزینه بستری در سالمندان می گردد.

مروری بر مکانیسم عصبی دید رنگ
دید رنگ از عملکردهای مهم سیستم بینایی میباشد که در جمعآوری اطلاعات بینایی، درک بینائی، فرآیند تجزیه و تحلیل اطلاعات بینایی و ارتباط اطلاعات بینایی با سایر اطلاعات مغز و حسهای دیگر نقش اساسی دارد. در مطالعات دید رنگ، یکی از ابعاد مهم دید رنگ، مکانیسم دید رنگ با توجه به یافتههای سایکوفیزیکال میباشد. به طور کلی در مکانیسم دید رنگ، شبکیه و سیستم عصبی مرکزی به کمک همدیگر و با مکانیسمهای جداگانه و موازی نقش ایفا مینمایند. برای درک این مکانیسم بایستی با نوروآناتومی و نوروفیزیولوژی سلولهای مخروطی و سایر سلولهای شبکیه، مسیر انتقال اطلاعات مربوط به رنگ در مسیر بینایی، نواحی مختلف کورتکس بینایی، نواحی بینائی رده بالا و مکانیسم های مربوط به تبدیل تحریک به درک آشنا باشیم. مهمترین مکانیسم دخیل در دید رنگ تضاد رنگ در سیستم بینایی میباشد. علاوه بر این، نوع نورونها همراه با ارتباطات آنها با همدیگر و همچنین تأثیر سایر عوامل دخیل در دید رنگ مانند کنتراست، لومینانس، درخشندگی، رنگ مرز منطقه رنگی و رنگ محیط منطقه رنگی، ثبات رنگ ناحیه رنگی و خیلی فاکتورهای دیگر در درک دید رنگ دخالت دارند. در این مقاله مروری نظاممند، مقالات داوری شده منتشر در PubMed بین سالهای 1970 تا 2015 مورد ارزیابی و نقد قرار گرفته، شواهد علمی معتبر با توجه به معیارهای ورود و خروج، جمعآوری، بررسی، خلاصه، بحث و نتیجه گیری شدند. هدف از این مطالعه، مروری بر ساختمان و عملکرد سیستم بینائی در مکانیسم دید رنگ می باشد.
