بررسی مهارتهای ادراکی- حرکتی درکودکان مبتلا به اختلال یادگیری خاص
هدف:اهمیت رشد تواناییهای ادراکی، در بازداری یا پیشرفت عملکرد حرکتی کودک میباشد. یادگیری حرکتی مبدأ یادگیری است و فرآیندهای ذهنی عالیتر، پس از رشد مناسب دستگاه حرکتی و دستگاه ادراکی به وجود میآید و این مطلب اهمیت مشکلات درکی _ حرکتی در افراد دارای اختلالات یادگیری را نشان میدهد. با توجه به شیوع بالای اختلال یادگیری این پژوهش با هدف ارزیابی مهارت های درکی- حرکتی در کودکان مبتلا به اختلالات یادگیری انجام شد. روش بررسی:این مطالعه از نوع مقطعی و مورد_شاهدی بود. گروه آزمون شامل 38 دانش آموز 7 الی 9 ساله مشغول به تحصیل در مدارس عادی شهر سمنان که طبق سنجش اداره آموزش و پرورش شهر سمنان مبتلا به اختلال یادگیری بودند. گروه شاهد شامل 38 کودک غیر مبتلا به اختلال یادگیری بود که با کودک مبتلا در یک کلاس درس بودند. برای ارزیابی مهارت هماهنگی دو طرفه از خرده آزمون هماهنگی دوطرفه در مقیاس رشد حرکتی Bruininks-Oseretsky استفاده شد و به وسیله آمار توصیفی و استنباطی، توسط نرمافزار spss16 و آزمون Mann-withney مورد تجزیه و تحلیل گرفت. یافتهها:تحلیلهای آماری نشان داد که کودکان مبتلا به اختلالات یادگیری در مهارت حرکتی هماهنگی دوطرفه ضعیفتر از کودکان غیر مبتلا عمل میکنند (0/01>p). نتیجهگیری:مطابق یافتههای پژوهش حاضر به نظر میرسد عملکرد کلی کودکان مبتلا به اختلال یادگیری در مهارت هماهنگی دو طرفه ضعیفتر از کودکان غیرمبتلا باشد. از آنجایی که کنترلکننده حرکات دوطرفه کورتکس حرکتی، مخچه و عقدههای قاعدهای هستند و با توجه به نقش عقدههای قاعدهای، و لوب پیشانی (که در ارتباط نزدیک با قشر حرکتی میباشد) در حافظه و یادگیری، به نظر میرسد میتوان از طریق تصحیح و تقویت مهارتهای حرکتی دو طرفه که منجر به بهبود عملکرد کلی مناطق یاد شده در سیستم اعصاب مرکزی می شوند، به ارتقا حافظه و یادگیری در این کودکان کمک کرد.

تأثیر شش هفته حرکات دراز کشیده به پشت روی آب بر شدت درد و ناتوانی مردان مبتلا به کمر درد مزمن ناشی از فتق دیسک کمری
هدف: کمردرد مزمن ناشی از فتق دیسک، یکی از بیماریهای شایع در سطح جامعه می باشد. علاوه بر درد که یکی از مشکلات عمده این عارضه می باشد، مشکلاتی از قبیل کاهش و نقص عملکرد فیزیکی بدن در طی فعالیتهای روزمره نیز گریبانگیر آنها خواهد شد. هدف از این مطالعه بررسی تأثیر شش هفته حرکات دراز کشیده به پشت روی آب بر شدت درد و ناتوانی مردان میانسال مبتلا به کمر درد مزمن ناشی از فتق دیسک کمری می باشد. روش بررسی: 24 مرد مبتلا به فتق دیسک کمری به صورت تصادفی به دو گروه تجربی (12 نفر) با میانگین سنی 3/28±47/75 و یک گروه کنترل (12 نفر) با میانگین سنی 5/78±46/83 تقسیم شدند. گروه تجربی، برنامه حرکت درمانی در آب را به مدت 24 جلسه، هر هفته 4 جلسه دریافت کردند و گروه کنترل در هیچ برنامه تمرینی شرکت نداشتند. اطلاعات در دو مرحله پیش آزمون و پس آزمون جمعآوری شد و با استفاده از آزمون T student در سطح معنی داری 0/05≥p مورد ارزیابی قرار گرفت. یافتهها: نتایج نشان داد شدت درد و ناتوانی در گروه تجربی به طور معنی داری کاهش پیدا کرده است (0/001=p). همچنین تغییرات بین گروهی نشان داد که میزان ناتوانی در گروه تجربی به نسبت گروه کنترل پس از شش هفته مداخله کاهش معنیدار داشته است (0/001=p). نتیجه گیری: نتایج این مطالعه نشان داد که حرکات دراز کشیده به پشت روی آب موجب کاهش درد و سطح ناتوانی عملکردی در مردان مبتلا به کمردرد مزمن ناشی از فتق دیسک کمری میشود.

بررسی تنوع واژگانی و پیچیدگی نحوی گفتار کودکان9-8 ساله پس از کاشت حلزون شنوایی
هدف: در بسیاری از کودکان با آسیب شنوایی شدید تا عمیق عمل کاشت حلزون شنوایی منجر به کسب مهارتهای گفتار و زبان میگردد، با این وجود کودکان برخوردار از کاشت حلزون شنوایی بسته به عوامل مختلفی از جمله زمان تشخیص، سن تجویز سمعک، سن کاشت، شروع توانبخشی و... درجاتی از ضعف را در زمینهی گرامر و واژگان در نمونهی زبانی خود نشان میدهند. هدف از مطالعه حاضر بررسی تنوع واژگانی و پیچیدگی نحوی گفتار پس از کاشت حلزون شنوایی در کودکان فارسی زبان میباشد. روش بررسی: در این پژوهش نمونههای زبانی حاصل از توصیف تصویر در 10کودک برخوردار از کاشت حلزون شنوایی و 10 کودک طبیعی همسان از نظر سن شنوایی و جنس، مورد مقایسه قرار گرفت. جهت بررسی پیچیدگی نحوی از شاخص میانگین طول گفته و جهت بررسی تنوع واژگانی از شاخص های تعداد واژههای متفاوت، تعداد کل واژهها و نسبت نوع به تعداد واژهها در50 گفته آغازین کودک استفاده گردید. متوسط سن شنوایی کودکان برخوردار از کاشت حلزون شنوایی 67/7 ماه (1/77=SD) بود و جهت تحلیل دادهها از آزمون t مستقل استفاده گردید. یافتهها: در آزمون آماری در همهی شاخصهای مورد مطالعه بین دو گروه کودکان برخودار از کاشت حلزون شنوایی و طبیعی تفاوت معناداری وجود نداشت (0/05<p). نتیجهگیری: در کودکان برخوردار از کاشت حلزون شنوایی، مدت زمان استفاده از کاشت حلزون شنوایی عامل مهمی در کسب مهارتهای گفتار و زبان میباشد. به عبارت دیگر تنوع واژگانی و پیچیدگی نحوی در گفتار کودکان برخوردار از کاشت حلزون شنوایی که 72-60 ماه از کاشت آنها میگذرد، تقریبا مشابه با کودکان طبیعی 72-60 ماهه میباشد.

مقایسه و اندازه گیری سرعت حرکات ساکاد چشم در داوران ماهر و مبتدی بسکتبال با استفاده از دستگاه الکترواکولوگرام دینامیک
هدف: حرکات ساکاد سریع در عملکرد موفق داوران و تصمیمگیری آنها در رویدادهای ورزشی ضروری هستند. هدف از این مطالعه تعیین سرعت ساکاد در داوران بسکتبال با استفاده از الکترو اکولوگرام دینامیک میباشد. روش بررسی: این مطالعه بر روی12 نفر داور ماهر و 14 نفر داور مبتدی بسکتبال (مرد) با دامنه سنی (18-38) سال انجام گرفت. تمامی شرکتکنندگان تحت معاینات رفرکشن و حدت بینایی قرار گرفتند. سرعت حرکات ساکاد برای زاویه 40 درجه بینایی (20 درجه به راست و 20 درجه به چپ) با استفاده از الکترو اکولوگرام دینامیک ارزیابی شد. برای مقایسه میانگین سرعت ساکاد در هر دو گروه از آزمون تی مستقل استفاده شد. یافتهها: میانگین سرعت ساکاد به سمت راست و چپ به ترتیب در داوران ماهر60±743/43 و732/00±40درجه بر ثانیه و در داوران مبتدی87/1±669/64 و 86±675/32 درجه بر ثانیه بود. تفاوت معنیداری بین دو گروه داوران وجود داشت (05/0p<). سرعت حرکات ساکاد چشم در داوران ماهر سریعتر از داوران مبتدی بود. نتیجه گیری: به نظر میرسد داوران ماهر به دلیل قرارگرفتن بیشتر در شرایط پویای ورزشی بهتر میتوانند رویدادها را بر پایه تجزیه و تحلیل احتمالی محیط پیشبینی کنند؛ و حرکات ساکاد سریعتری داشته باشند که مربوط به تمرین و تجربه است.

بررسی اضطراب اجتماعی دربزرگسالان مبتلا به لکنت
هدف: لکنت اختلالی حرکتی گفتار است که با علائمی مانند تکرار کلمات و هجاها، کشیدهتر گفتن بعضی آواها و یا قفلشدگی اندامهای گویایی هنگام تلفظ برخی صداها شناخته میشود. از آنجا که به نظر میرسد افراد مبتلا به لکنت تا حدی از اضطراب اجتماعی/ هراس اجتماعی رنج میبرند و با توجه به اثرات اضطراب اجتماعی بر افراد و بر کیفیت زندگی آنها، شناسایی این مشکل به ویژه در افراد مبتلا به لکنت از اهمیت بسیار برخوردار است. بنابراین هدف مطالعه حاضر بررسی مولفههای مختلف آزمون اضطراب اجتماعی شامل ترس، اجتناب از اجتماع و ناراحتی فیزیولوژیک در افراد بزرگسال 18 تا 45 ساله مبتلابه لکنت بود. روش بررسی: در این مطالعه تحلیلی از نوع مورد شاهدی با استفاده از پرسشنامه هراس اجتماعی، 32 فرد مبتلا به لکنت در محدوده سنی 18 تا 45 سال و 32 فرد دارای گفتار روان که از نظر سن و جنس و تحصیلات با گروه دارای لکنت همسان شده بودند، به لحاظ میزان اضطراب اجتماعی مورد بررسی قرار گرفتند. یافتهها: آزمون تی مستقل نشان داد که میانگین نمره اضطراب اجتماعی در افراد مبتلا به لکنت به طور معناداری بیش از میانگین نمره افراد دارای گفتار روان است. همچنین در بین مولفههای این آزمون، میانگین نمره مولفه ترس بین دو گروه تفاوت معناداری داشت. نتیجهگیری: نتایج این تحقیق حاکی از بالاتر بودن میزان اضطراب اجتماعی (هراس اجتماعی) به طور کلی و مولفه ترس به طور اختصاصی، در افراد مبتلا به لکنت نسبت به همتایان دارای گفتار روانشان بود. بنابراین لازم است در برنامههای درمانی که توسط آسیبشناسان گفتار و زبان برای این افراد در نظر گرفته میشود، به این جنبه از اختلال نیز پرداخته شود و بخشی از برنامههای درمانی به تعدیل و کاهش میزان اضطراب اجتماعی افراد و بهخصوص احساس ترس در آنها معطوف گردد.

تاثیر هشت هفته ورزش یوگای بر دامنه حرکتی مفصل شانه زنان مبتلا به سرطان سینه ماستکتومی
هدف: ضعف عضلانی، درد، اختلال حسی، محدودیت حرکتی مفصل شانه از مهمترین مشکلات پس از درمانهای سرطان سینه است. لذا هدف از تحقیق حاضر بررسی تاثیر هشت هفته ورزش یوگای بر دامنه حرکتی مفصل شانه زنان مبتلا به سرطان سینه بود. روش بررسی: تعداد 24 بیمار مبتلا به سرطان سینه در دامنه سنی 47 تا65 سال به صورت در دسترس و تصادفی به دو گروه کنترل و شاهد تقسیم شدند. گروه شاهد برنامه تمرینات یوگا را 2 جلسه در هفته به مدت 8 هفته انجام دادند در حالی که گروه کنترل فعالیت خاصی نداشتند. دامنه حرکتی شانه بیماران توسط گونیامتر قبل و بعد از 8 هفته مورد ارزیابی قرار گرفتند. به منظور تجزیه و تحلیل دادهها از آزمون Tوابسته و T تفاضل مستقل استفاده شد. آزمون مورد استفاده در سطح معناداری 0/05>p در نظر گرفته شد. یافتهها: نتایج این پژوهش نشان داد که بهبودی معناداری درمیزان دامنه حرکت خم شدن، باز شدن، دور شدن، چرخش خارجی و چرخش داخلی شانه گروه شاهد مشاهده شد (0/05>p). در حالی که در گروه کنترل کاهش معناداری مشاهده شد (0/05<p). نتیجهگیری: از یافتههای این پژوهش میتوان نتیجه گرفت که انجام حرکات یوگا باعث افزایش دامنه حرکت اندام فوقانی و نهایتا بهبود عملکرد شانه میشود، بنابراین به نظر میرسد استفاده از تمرینات یوگا عنوان برای افراد سرطانی قابل توصیه میباشد.

بررسی دیرش واکه ای درافراد مبتلا به لکنت وافراد بدون لکنت
هدف: گروهی از محققین لکنت را اختلال در زمانبندی بین عناصر آوایی و وقفه در برنامهریزی همزمان و موفق حرکات عضلات گفتاری جهت تولید یک کلمه میدانند از جمله متغیرهای زمانی میتوان به دیرش واکهای اشاره کرد. دیرش واکهای مختصه زمانی است که به طور دقیق میتواند ماهیت تفاوتهای آواسازی بین افراد مبتلا به لکنت و بدون لکنت را نشان دهد. هدف این مطالعه بررسی و مقایسه دیرش واکهای افراد مبتلا به لکنت و بدون لکنت بود. روش بررسی: این مطالعه از نوع توصیفی- تحلیلی بوده و در آن 22 نفر از افراد دارای لکنت و 22 نفر بدون لکنت از سنین 10 تا 30 سال انتخاب شدند و از آنها خواسته شد که متنی را که شامل هرسه CV,CVCCVCC و شامل 6 واکه را بخواند سپس خلاصهی آن را تعریف کند و گفتار روان شامل هر سه بافت انتخاب و توسط نرمافزار Praat ضبط و ازطریق اسپکتروگرام دیرش واکه درآنها محاسبه شد. یافتهها: آزمون t مستقل نشان داد که لکنت (0/21=p) و سن (0/061=p) تاثیر معناداری بر دیرش واکهای ندارد. اگرچه این مقادیر در گروه مبتلا به لکنت نسبت به گروه بدون لکنت بیشتر بود. نتیجهگیری: مطالعه نشان داد که لکنت بر دیرش واکهای تاثیر معناداری ندارد.

بررسی دید رنگ در بین کارکنان کارخانجات تولید فراورده های نفتی و پتروشیمی
هدف: دید رنگ از مهمترین و تاثیرگذارترین تواناییهای بینایی است. از مشاغلی که دید رنگ در انجام هرچه صحیحتر و حرفهایتر امور نقش دارد، کار در کارخانجات و کارگاههای صنعتی است که بسیاری از دستورها، پیامها، هشدارها و همچنین نمایشگرهای دستگاههای دیجیتالی با نمایش رنگهای متفاوتی بیان میگردند. این تحقیق به منظور بررسی تاثیر مواجهات طولانی مدت با فراورده های نفتی و پتروشیمی بر دید رنگ انجام شد. روش بررسی: ین مطالعه مقطعی در کارخانجات تولید فراوردههای پتروشیمی و پالایشگاه نفت که همگی مذکر بودند در بخشهای صنعتی و اداری انجام شد. این افراد به دو گروه دارای مواجهه به تعداد224 نفر (54%) و بدون مواجهه به تعداد 194نفر (46%) تقسیم شدند. بعد از در نظر گرفتن معیارهای ورود و خروج شامل بیماریهای چشمی، بیماریهای داخلی مانند دیابت، مصرف داروهای نورو توکسینی، حدت بینایی تک چشمی با بهترین اصلاح عیوب انکساری کمتر از 6/10، سن بالای 40 سال، 418 نفر با رضایت کامل در این مطالعه شرکت کردند. ابتدا یک معاینه اپتومتری شامل تعیین حدت بینایی و اصلاح عیوب انکساری و سپس تست دید رنگ بر روی کارکنان بخشهای مختلف انجام شد. تست دید رنگ نیز شامل تست رنگ با صفحات دفترچه ایشیهارا (38صفحهای) و چیدمان مهرهای تست LD-15 d بود. یافتهها: اختلال دید رنگ در افراد شاغل در بخشهای صنعتی (گروه با مواجهه مواد شیمیایی) 19 درصد و اختلال دید رنگ افراد شاغل در قسمت اداری (گروه بدون مواجهه با مواد شیمیایی) 6 درصد تعیین شد. میانگین ضریب اغتشاش دید رنگ در گروه دارای مواجهه با مواد شیمیایی 0/16± 1/12 و در گروه بدون مواجهه 0/07±1/03 بوده و در مقایسه با هم تفاوت معنیداری دارند (0/03=p). نتیجهگیری: تفاوت موجود در اختلالات دید رنگ در افراد با مواجهه مواد نفتی و پتروشیمی نسبت به افراد بدون مواجهه تاییدی بر این فرضیه است که مواجهه با مواد شیمیایی مانند مواد فراوردههای پتروشیمی و نفتی میتواند باعث بروز اختلالات دید رنگ اکتسابی شود.

بررسی فراوانی بیماریهای قارچی سطحی و جلدی شهر نیشابور طی پنج سال، یک مطالعه گذشته نگر
هدف: بیماریهای قارچی سطحی و جلدی هنوز از علل مهم عفونتهایی هست که پوست، مو و ناخن را درگیر میکنند. این بیماریها به علت آلودگی محیط و عدم رعایت اصول بهداشتی، در مناطق محروم از شیوع بیشتری برخوردار میباشند. لذا در مطالعه حاضر یک بررسی گذشتهنگر در شهر نیشابور برای شناخت میزان فراوانی بیماریهای قارچی سطحی و جلدی انجام گرفت. روش بررسی: این مطالعه Cross sectional در یک دوره زمانی 5 ساله (بین سالهای 1393-1388) به بررسی 1281 بیمار ( 48/9%) 627 مذکر و (15/1%) 654 مونث) مشکوک به عفونت های قارچی سطحی و جلدی پرداخت. تشخیص بیماری از طریق آزمایش های میکروسکوپی مستقیم و توسط لام مرطوب با هیدروکسید پتاسیم 15% (KOH) و رنگآمیزی متیلن بلو و گرم انجام شد و سپس دادهها توسط نرم افزار SPSS-16 آنالیز شدند. یافته ها: از 1281 بیمار مشکوک، (15%) 196 نفر مبتلا به بیماری های قارچی سطحی و جلدی بودند. فراوانی هر کدام از این بیماری ها بترتیب شامل درماتوفیتوزیس 74/5% (146 مورد)، اونیکومایکوزیس 12/2% (24 مورد)، تیناورسیکالر 8/8% (17 مورد)، اریتراسما 3/5% (7مورد) و کاندیدیازیس جلدی 1% (2 مورد) بودند. در ضمن، گروه سنی 40-31 سال (21/4%) دارای بالاترین فراوانی بودند. نتیجهگیری: این مطالعه نشان داد که بیماریهای قارچی سطحی و جلدی بویژه درماتوفیتوزیس (کچلیها) سهم مهمی را در بین بیماریهای پوستی در این منطقه دارا میباشند. بیشترین فراوانی این بیماریها یکی در اواخر اردیبهشت تا آخر مرداد و دیگری اوایل شهریور تا اواخر آذر ماه بود. بنابراین مطالعات اپیدمیولوژیک و تشخیص دقیق بیماریهای قارچی پوستی میتواند در پیشگیری و درمان موثر آنها کمک کند.

خستگی در بیماران مبتلا به مولتیپل اسکلروزیس: علل، ارزیابی و درمان
هدف: خستگی از شایعترین علائم در افراد مبتلا به ام اس است و عامل بزرگی در محدود کردن مسئولیتهای کاری و اجتماعی این افراد است .با توجه به عدم وجود مطالعه جامع ثبت شده در بانکهای مطالعاتی ایران، هدف مطالعه حاضر، مروری جامع بر خستگی، علل و ارزیابی و درمان آن در بیماران مبتلا به ام اس می باشد. روش بررسی: در این مطالعه مروری از بانکهای اطلاعاتی داخلی و خارجی بین سالهای 1989 تا 2013 برای جمعآوری اطلاعات استفاده شد. یافتهها: خستگی یکی از عوامل تاثیرگذار بر کیفیت زندگی، انزوای اجتماعی و نیز عدم اشتغال بیماران مبتلا به ام اس است. خستگی تحت تاثیر علل اولیه و ثانویه بروز مییابد. ارزیابی جامع خستگی به انتخاب بهترین استراتژی درمانی برای غلبه بر این علامت مشکل زا کمک میکند. معیار شدت خستگی (FSS)، مقیاس تاثیر خستگی (FIS)، مقیاس تاثیر خستگی اصلاح شده (MFIS) و مقیاس آنالوگ دیداری (VAS-F) رایج ترین ابزارهای سنجش خستگی در بیماران مبتلا به ام اس میباشد. در درمان خستگی از روشهای دارویی و غیردارویی(تمرینات ایروبیک، ذخیرهسازی انرژی و درمانهای شناختی رفتاری) استفاده می شود. نتیجهگیری: با توجه به اهمیت خستگی و تاثیر بارز آن بر کیفیت زندگی فردی و حرفهای بیماران مبتلا به ام اس، ارزیابی دقیق و درمان مناسب آن ضروری میباشد.
