تأثیر کراس-لینکینگ قرنیه بر اسمولاریتی اشک در بیماران کراتوکونوس پیشرونده
هدف: این مطالعه با هدف بررسی تأثیر کراسلینکینگ قرنیه بر اسمولاریتی اشک در بیماران کراتوکونوس پیشرونده با استفاده از دستگاه TearLab انجام شده است. روش بررسی: در این مطالعه 66 چشم از 66 بیمار (محدوده سنی 15 تا 35 سال شامل 40 مرد و 26 زن)، با کراتوکونوس پیشرونده که کاندید جراحی کراسلینکینگ قرنیه بودند، تحت مطالعه قرار گرفتند. این بیماران، فاقد هرگونه سابقه بیماری چشمی و یا بیماری سیستمیک با تظاهرات چشمی بودند و تا سه هفته قبل از مطالعه، از لنز تماسی استفاده نکرده بودند. همچنین این بیماران بعد از عمل نیز تا 6 ماه از داروهای مؤثر بر اشک چشم یا لنز تماسی (به جز مدت درمان بعد جراحی) استفاده نکردند. میزان اسمولاریتی اشک با استفاده از دستگاه TearLab قبل و 6 ماه بعد از کراسلینکینگ قرنیه اندازهگیری شد و پس از جمعآوری دادهها، اطلاعات بدست آمده از طریق روشهای آماری توصیفی و تحلیلی آنالیز گردید. یافتهها: در مقادیر اسمولاریتی اشک قبل و 6 ماه بعد از کراس لینکینگ قرنیه، اختلاف معنیداری مشاهده نشد و میانگین اسمولاریتی، قبل از عمل جراحی کراسلینکینگ قرنیه15/50±303/82 و بعد از عمل 14/49±306/64 بود که بین میانگین اسمولاریتی اشک قبل و بعد از کراسلینکینگ قرنیه، اختلاف معنیداری بدست نیامد (0/192=p). نتیجهگیری: جراحی کراسلینکینگ قرنیه در بیماران دارای کراتوکونوس پیشرونده، باعث تأثیر قابل توجهی بر اسمولاریتی اشک نمیشود و نمیتوان خشکی چشم را به عنوان یکی از عوارض جانبی کراسلینکینگ قرنیه در نظر گرفت.

تاثیر تمرینات مقاومتی با باند الاستیک برتعادل ایستا و درد زنان سالمند دارای آرتروز و واروس زانو
هدف: با افزایش جمعیت سالمندان، شیوع ناتواناییهای جسمانی نیز در حال افزایش است که این امر، توجه سیستمهای مراقبتی، بهداشتی و اجتماعی را به خود معطوف کرده است. از جملة مشکلات شایع در دوران سالمندی کاهش تعادل واختلال پوسچر، کاهش عملکرد عصبی عضلانی، توده عضله، قدرت، استقامت، حرکت و بی ثباتی مفصل است. هدف از پژوهش حاضر، ارزیابی تاثیر هشت هفته تمرین مقاومتی با باند الاستیک، بر تعادل ایستا و درد سالمندان زن، دارای آرترورز و واروس زانو میباشد. روش بررسی: در این مطالعه نیمه تجربی، 24 بیمار زن مبتلا به آرتروز و واروس زانو (دامنه سنی50 تا 65 سال) به طور داوطلبانه آمادگی خود را جهت شرکت در پژوهش اعلام کردند و به صورت تصادفی در دو گروه تجربی و کنترل قرار گرفتند. جهت سنجش تعادل از دستگاه تعادل سنج بایودکس (BIODEX)و میزان درد بیماران از طریق مقیاس دیداری درد (VAS) استفاده گردید. گروه تجربی به مدت 8 هفته تمرینات مقاومتی با باند الاستیک را به مدت 45 دقیقه، سه روز در هفته انجام دادند. جهت تجزیه و تحلیل آماری از آزمون تی استیودنت وابسته و مستقل استفاده گردید. یافتهها: نتایج تحقیق بهبود معنی داری در آزمون تعادل ایستا بین دو گروه تجربی و کنترل نشان نداد (0/147=p). اما در تغییرات آزمون درد بهبود معنیداری بین دو گروه کنترل و تجربی مشاهده شد (0/001=p). نتیجهگیری: یک دوره تمرینات مقاومتی با باند الاستیک نتوانست بر تعادل ایستا سالمندان مبتلا به آرتروز و واروس زانو تاثیرگذار باشد اما بر تغییرات درد زانو این گروه از افراد تاثیر قابل توجهی داشت لذا میتوان از تمرینات مقاومتی با باند الاستیک، در بیماران سالمند مبتلا به آرتروز و واروس زانو، به عنوان یک روش به منظور کاهش درد در مفصل زانو استفاده کرد.

هدف: رویکرد جهانی به کیفیت مراقبت سلامت منجر به تلاش برای مدیریت مؤثرتر منابع و خدمات با در نظر گرفتن افزایش هزینهها، مزایای رقابتی در بازاریابی و ایمنی بیمار شده است. یکی از ابزارهای اصلی کیفیت مراقبت سلامت و بهبود آن اعتباربخشی است. این پژوهش باهدف بررسی فرایند اجرای اعتباربخشی از دیدگاه کارکنان بخش مدیریت اطلاعات سلامت، بهعنوان اصلیترین جایگاه اطلاعاتی و تصمیمگیری در بیمارستانها و یک منبع مهم ارزیابی انجامشده است. روش بررسی: این مطالعه بهصورت مقطعی در کلیه بخش مدیریت اطلاعات بیمارستانهای مشهد با استفاده از پرسشنامهای محقق ساختهای اجرا شد. تحلیل دادهها با آنالیز واریانس یکطرفه برای مقایسه میانگین متغیر اجرای اعتباربخشی در چند گروه انجام گردید. همچنین از آزمون فریدمن بهمنظور مقایسه رتبهها در هر یک از مؤلفههای پرسشنامه استفاده شد. یافتهها: یافتهها نشان داد مدیریت و هدایت مؤثر، آگاهی از اهداف اجرای اعتباربخشی، پویایی و کارایی بخش، ایجاد انگیزه با ارائه پاداش و مشارکت کارکنان در اعتباربخشی از نقاط قوت اجرای آن است. در مقابل افزایش حجم کار کارکنان و تقسیم وظایف، کمبود آموزش روشهای بهبود کیفیت، اطلاعرسانی ناکافی به کارکنان از میزان پیشرفت، کمبود تخصیص بودجه، نبود انگیزه در کارکنان و فقدان کار گروهی وظایف از چالشهای موجود است. نتیجهگیری: آموزش، توسعه فرهنگ کار گروهی و مدیریت مؤثر به اجرای کاملتر فرایند اعتباربخشی منجر میگردد و ضمن تأثیر مثبت بر عملکرد کارکنان، به رفع چالشهای اجرای اعتباربخشی در بخش مدیریت اطلاعات سلامت کمک میکند.

بررسی مقایسه ای انسجام داستان در کودکان فارسی زبان مبتلا به آسیب ویژهی زبانی و کودکان طبیعی
هدف: درمطالعات انجام شده در زبان انگلیسی، کودکان مبتلا به اختلال آسیب ویژهی زبانی، نقایص ویژهای را در انسجام داستان نشان دادهاند؛ در پژوهش حاضر انسجام داستان در این دسته از کودکان و کودکان طبیعی در زبان فارسی بررسی شده است. روش بررسی: این مطالعه از نوع مقطعی است. 50 کودک با رشد طبیعی و 10 کودک مبتلا به آسیب ویژهی زبانی در این مطالعه شرکت کردند. کودکان آسیب ویژهی زبانی از نظر وضعیت شنوایی، هوش غیرکلامی و عاطفی طبیعی بودند. مشکل زبانی آنها با استفاده از آزمون رشد زبان تایید گردید. با استفاده از نسخه فارسی آزمون هنجار گفتمان روایتی، داستانهای تک صحنهای و سه صحنهای از آنها استخراج و نسخهبرداری گردید. سپس بر اساس عناصر انسجام امتیاز داده شد. در هردو مرحله نسخه برداری و امتیازدهی، درصد توافق بین دو مصحّح محاسبه گردید. جهت مقایسه امتیاز انسجام داستانها و صحنهها بین دو گروه، از آزمونهای تی مستقل و من ویتنی استفاده شد. یافتهها: در کودکان طبیعی درصد توافق برای نسخهبرداری هر دو داستان«الف1» و «الف3» 100درصد، امتیازدهی داستان «الف1»، 96 درصد و امتیازدهی داستان «الف3»،94/51 درصد به دست آمد. پس از مقایسه گروهها مشخص شد که در بین دو گروه میانگین امتیاز انسجام داستان «الف1» و «الف3» به تفکیک و همچنین میانگین امتیاز انسجام صحنههای مختلف داستان «الف3» در دو گروه به طور معنادار متفاوت هستند (0/00=p). نتیجهگیری: عملکرد انسجام داستان در کودکان مبتلا به آسیب ویژهی زبانی ضعیفتر از کودکان با رشد طبیعی است.

تاثیر حرکت درمانی در آب بر فعالیت الکتریکی عضلات چهارسررانی و شدت درد زنان مبتلا به استئوآرتریت زانو
هدف: امروزه استئوآرتریت به یکی از بزرگترین مسائل جامعه سالمندان تبدیل شده است که منجر به ناتوانی، مشکلات جسمی، هزینههایی برای خانواده و جامعه میشود. لذا پیدا کردن یک روش درمانی که بتواند به این افراد کمک کند و عوارض ناشی از این بیماری را کاهش دهد از اهمیت زیادی برخوردار است بنابراین هدف از این پژوهش تاثیر حرکت درمانی در آب بر فعالیت الکتریکی عضلات چهارسررانی و شدت درد زنان مبتلا به استئوآرتریت زانو بود. روش بررسی: تعداد 30 زن سالمند مبتلا به استئوارتریت زانو به روش نمونهگیری هدفمند و در دسترس، انتخاب، و به صورت تصادفی ساده به دو گروه تجربی(سن: 4/78±58/73) و کنترل(سن: 5/43±56/93) تقسیم شدند لازم به ذکر است که بین میانگین سن گروه تجربی و کنترل از نظر آماری تفاوت معناداری وجود نداشت (0/963=t، 0/344=p). به گروه تجربی یک دوره حرکت درمانی در آب به مدت20 جلسه با تواتر پنج جلسه درهفته، و هر جلسه 60 - 45 دقیقه انجام شد. قبل و بعد از مداخله، فعالیت الکتریکی عضلات چهارسررانی با استفاده از دستگاه الکترومیوگرافی سطحی biovision ساخت کشور سوئیس و شدت درد با استفاده از مقیاس عینی درد (Visual analog scale) اندازهگیری و ثبت شد. تجزیه تحلیل دادهها با استفاده از نرم افزارspss نسخه 20 و آزمون T student مورد آزمون قرار گرفت. یافتهها: نتایج تحقیق نشان داد که حرکت درمانی در آب باعث افزایش معناداری در فعالیت الکتریکی عضلات پهن مایل داخلی (0/2817=p)، پهن خارجی (0/2681=p) و راست رانی (0/831=p) نشد اما باعث افزایش میانگین فعالیت الکتریکی این عضلات شد. همچنین نتایج نشان داد که این تمرینات باعث کاهش شدت درد در بیماران شده است (0/001=p). نتیجهگیری: حرکت درمانی در آب بر فعالیت الکتریکی عضلات چهار سر رانی تاثیری ندارد. ولی میتوان این روش درمانی را به عنوان روشی مؤثر برای کاهش شدت درد بیماران مبتلا به استئوآرتریت زانو تجویز نمود.

مطالعه هیسترزیس و فاکتور مقاومت قرنیه در دژنراسیون حاشیهای شفاف قرنیه با دستگاه تحلیل کننده پاسخ چشمی
هدف: هدف این پژوهش، مطالعه برخی پارامترهای بیومکانیک قرنیه شامل هیسترزیس و فاکتور مقاومت قرنیهای در دژنراسیون حاشیهای شفاف قرنیه با دستگاه تحلیل کننده پاسخ چشمی میباشد. روش بررسی: این مطالعه توصیفی براساس اطلاعات موجود در پرونده پزشکی بیماران از ژانویه سال 2013 تا اکتبر 2015 انجام شد و تعداد 102 چشم از 63 بیمار 20 تا 50 ساله در این مطالعه وارد شدند. تشخیص دژنراسیون حاشیهای شفاف قرنیه براساس معاینات بالینی و تصویربرداریهای پاراکلینیک شرط ورود به مطالعه و معیارهای خروج شامل سایر بیماریهای قرنیهای، خشکی چشم شدید، گلوکوم، استفاده از داروهای چشمی، حاملگی یا شیردهی، بیماریهای سیستمیک، جراحی چشمی قبلی، سن بالای 50 سال و چشمهای مشکوک به سایر اکتازیهای قرنیهای بودند. مقادیر هیسترزیس، فاکتور مقاومت قرنیهای و سایر پارامترهای خروجی دستگاه تحلیلگر پاسخ چشمی استخراج شدند و با استفاده از نرم افزار SPSS مورد آنالیز آماری توصیفی و تحلیلی قرار گرفتند. یافتهها: در بررسی مقادیر خروجی از دستگاه تحلیلگر پاسخ چشمی در کل افراد تحت مطالعه، میانگین (±انحراف معیار) هیسترزیس قرنیهای 1/47±9/18، فاکتور مقاومت قرنیهای 1/72±8/48، فشار داخل چشمی جبران شده قرنیهای 3/08±14/58 و فشار داخل چشمی مرتبط با گلدمن 3/42±12/38 میلیمتر جیوه بدست آمدند. همچنین برای هیسترزیس قرنیهای (0/12=p) فشار داخل چشمی جبران شده قرنیهای (0/37=p) و فشار داخل چشمی مرتبط با گلدمن (0/07=p) میان دو گروه مرد و زن، اختلاف معناداری وجود ندارد اما برای فاکتور مقاومت قرنیهای میان دو گروه مرد و زن، اختلاف معناداری (0/01=p) وجود دارد. نتیجهگیری: این مطالعه، مقادیر طبیعیهیسترزیس و فاکتور مقاومت قرنیهایحاصل از دستگاه تحلیل کننده پاسخ چشمی را برای دژنراسیون حاشیهای شفاف قرنیه نشان میدهد. این نتایج میتوانند در ارزیابی بیومکانیک قرنیه بعنوان یک شاخص در فرآیند تشخیص و درمان این بیماری مورد استفاده قرار گیرند.

پاسخگویی نظام سلامت در حوزه خدمات سرپایی در مناطق برخوردار و کمتر برخوردار شهر مشهد
هدف: خدمات سرپایی بخش قابل توجهی از نظام ارائه خدمات سلامت را شامل میشود. تسلط بخش غیردولتی در ارائه این خدمات در کنار نقش تولیتی دولت، اهمیت پایش جنبههای غیربالینی کیفیت این خدمات را نشان میدهد. هدف از این تحقیق، بررسی میزان پاسخگویی نظام سلامت در حوزه خدمات سرپایی در مناطق منتخب شهر مشهد بوده است. روش بررسی: این مطالعه توصیفی- کاربردی و از نوع مقطعی بود. نمونه مورد بررسی شامل 889 خانوار ساکن در دو منطقه برخوردار و کمتر برخوردار شهر مشهد بود که به روشنمونهگیری ترکیبی و در سال 1393 انتخاب شدند. ابزار مطالعه، پرسشنامه سه بخشی سازمان جهانی بهداشت بود که روایی و پایایی آن قبلا تائید شده بود. دادهها با استفاده از شاخصهای توصیفی و استنباطی آماری در نرم افزارSPSS تحلیل گردید. یافتهها: یافتههای این مطالعه نشان داد که به طور کلی بیش از 60% افراد، پاسخگویی خدمات سرپایی را در حوزه های مختلف خوب گزارش دادند. سطح پاسخگویی در حوزه محرمانگی بالاتر و در حوزه کیفیت امکانات رفاهی محل ارائه خدمت، پایینتر از سایر حوزهها بوده است. همچنین سطح پاسخگویی از نظر ساکنان منطقه برخوردار بیشتر از منطقه کمتر برخوردار گزارش شده بود. نتیجهگیری: پاسخگویی نظام سلامت هدفی است که در صورت وجود یک تولیت خوب در نظام سلامت محقق می شود. از آنجا که اکثر نهادهای ارائهدهنده خدمات سرپایی خارج از تصدی وزارت بهداشت به عنوان تولیت نظام سلامت است، لذا ایجاد مکانیسمهای مناسب برای پایش میزان تحقق این هدف ضروری به نظر می رسد.

تأثیر تمرینات منتخب پیلاتس بر قدرت عضلانی و افسردگی زنان سالمند
هدف: کاهش قدرت عضلانی یکی از علل اصلی افتادن در سالمندان است، که با عواقب و عوارض جسمانی و افسردگی همراه است. بنابراین هدف از پژوهش حاضر، تعیین تاثیر یک دوره تمرینات منتخب پیلاتس بر قدرت عضلات ران و افسردگی زنان سالمند بود . روش بررسی: در پژوهش نیمه تجربی حاضر تعداد 23 زن سالمند با دامنه سنی 55 تا 60 سال از بین سالمندان داوطلب شهر اصفهان بصورت دردسترس و هدفمند انتخاب و بطور تصادفی به دو گروه تجربی (12 نفر، با میانگین و انحراف معیار سن 4/61±56/75 سال، وزن 5/41±61/75 کیلوگرم، قد 7/01±161/83 سانتیمتر)و کنترل (11 نفر، با میانگین و انحراف معیار سن10/20±59/09 سال، وزن/81±62/78 کیلوگرم، قد 5/03±158/81 سانتیمتر) تقسیم شدند. گروه تجربی 8 هفته تمرینات پیلاتس را 3 جلسه در هفته به مدت 60 دقیقه انجام دادند، ولی گروه کنترل در هیچگونه فعالیتی به غیر از فعالیتهای معمول روزانه شرکت نداشتند. قدرت عضلانی با استفاده از دینامومتر دستی (Power Track II Commander TMJ Tec medical) و میزان افسردگی با استفاده از پرسشنامه افسردگی Beck در دو مرحله قبل و بعد از اجرای تمرینات پیلاتس اندازهگیری شد. تجزیه و تحلیل اطلاعات به وسیله آزمون تی همبسته و آزمون تحلیل واریانس یکطرفه در سطح معناداری (p≤0/005) مورد تحلیل قرار گرفت. یافته ها: نتایج نشان داد که پس از 8 هفته تمرین بهبود معناداری در نمرات قدرت عضلانی چهارسر و همسترینگ پای گروه تجربی نسبت به پیش از دوره تمرینات پیلاتس یافت شد، در حالی که در گروه کنترل تغییر معناداری مشاهده نشد. همچنین تفاوت معناداری در نمرات افسردگی بین پیش و پس آزمون در گروه تجربی دیده شد اما در گروه کنترل تفاوت معناداری مشاهده نگردید. نتیجه گیری: با توجه به نتایج پژوهش حاضر، چنین به نظر میرسد که تمرینات منتخب پیلاتس بر بهبود قدرت عضلانی و افسردگی زنان سالمند موثر است و می تواند به عنوان یک مداخلهای تأثیرگذار مورد توجه قرار گیرد.

بررسی شاخص کیفیت زندگی مرتبط با معلولیت صوت در بزرگسالان مبتلا به فلجی تارهای صوتی
هدف: ابزارهای متعددی به منظور بررسی تأثیر اختلال صوت بر ابعاد مختلف جسمی، روانی و اجتماعی بیماران طراحی شده است که از جمله آنها میتوان به شاخص معلولیت صوت (Voice Handicap Index-30; VHI)اشاره کرد. در بیماران فلجی تارهای صوتی ، صوت بیشتر از دیگر زیر سیستمهای گفتار تحت تاثیر قرار میگیرد. هدف ما در این پژوهش بررسی کیفیت زندگی در افراد فلجی تارهای صوتی با ابزار شاخص معلولیت صوت می باشد. روش بررسی: نمونه از 10 نفر که شامل 9 نفر مرد و 1 نفر زن بود، تشکیل می شد. سن بیماران نیز در محدوده سنی 55-23 سال بود. زمان نمونهگیری از ابتدای تیرماه تا 15 مرداد ماه 1392و محل آن کلینیکهای گفتار درمانی شهر مشهد بود. پس از تکمیل نسخه فارسی پرسشنامه توسط بیماران اطلاعات با استفاده از نرم افزار SPSS و با استفاده از آزمون ضریب همبستگی پیرسون مورد تجزیه و تحلیل آماری قرارگرفت. یافته ها: ضریب همبستگی پیرسون بین زیرآزمون حیطههای عملکردی و عاطفی 0/474، بین زیرآزمون حیطههای عملکردی و جسمی 0/821 و بین زیرآزمون حیطههای جسمی و عاطفی 0/669 بود. در مطالعه حاضر ارتباط بین زیرآزمون حیطههای عملکردی - عاطفی متوسط، زیرآزمون حیطههای جسمی - عاطفی قوی و حیطههای عملکردی - جسمی بسیار قوی ارزیابی گردید. نتیجهگیری: تأثیر معلولیت ناشی از اختلالات صوت به طور عمده از فردی به فرد دیگر متفاوت است و به عواملی مانند شغل، محیط زندگی و کار، واکنش اطرافیان به کیفیت اختلال صوت و شخصیت کلی فرد مرتبط میباشد. اختلال صوت در افراد مبتلا به فلجی تارهای صوتی در کاهش کیفیت زندگی آنها تاثیر بسزایی دارد. ارزیابیهای بیمار محوری مانند برای ارزیابی بیماران مبتلا به فلجی تارهای صوتی در کنار دیگر ارزیابیهای گفتاری توصیه می شود.

اثر تسهیل عصبی عضلانی از طریق گیرندههای عمقی برتحکیم مهارت حرکتی ظریف: مطالعه مقدماتی
هدف: هدف از پژوهش حاضر، مقایسه اثر خواب شبانه بر تحکیم مهارت حرکتی ظریف دانشجویان 19 تا 25 ساله به دنبال یک جلسه تمرین با و بدون تسهیل عصبی-عضلانی از طریق گیرندههای عمقی بود. روش بررسی: 22 دانشجوی راست دست (25-19سال) که دارای کیفیت خواب مطلوب و عدم بیماری عصبیعضلانی بودند به صورت تصادفی به دو گروه آزمایشی (انجام تمرینات تسهیل عصبی-عضلانی) و کنترل تقسیم شدند. آزمودنیهای گروه آزمایشی و کنترل در روز اول به یک جلسه تمرین (54 کوشش) پرتاب دارت پرداخته و بلافاصله در آزمون اکتساب (9 کوشش) شرکت کردند. سپس در صبح روز دوم پس از خواب شبانه، آزمون بازآزمایی (9کوشش) به عمل آمد. برای ارزیابی بهبود دقت اجرا، آزمون اکتساب و آزمون بازآزمایی با یکدیگر مقایسه شدند. برای تجزیه و تحلیل دادهها از آزمون تحلیل واریانس بین-درون آزمودنیهای آمیخته در سطح معناداری 0/05=α استفاده گردید. یافته ها: نتایج تحقیق حاضر نشان داد که تفاوت معنادار آماری در دقت اجرا قبل و بعد از خواب شبانه (0/62=p،3/9=f) بین دو گروه مشاهده نشد (0/71=p،0/13=f). نتیجه گیری: نتایج پژوهش حاضر نشان داد که خواب شبانه به دنبال یک جلسه تمرین با و بدون تسهیل عصبی-عضلانی قبل از اجرای مهارت حرکتی ظریف، تاثیری بر تحکیم مولفه دقت نداشته است
