تحلیل کینتیک راه رفتن در بیماران مبتلا به اسکولیوز نوجوانی ناشناخته

چکیده

هدف: 
درناهنجاری اسکولیوز نوجوانی ناشناخته، هنوز تعامل بین انحراف ستون ­مهره­ای با جنبه­های کینتیکی راه رفتن به خوبی شناسایی نشده است. هدف از مطالعه حاضر تحلیل نیروی عکس‌العمل زمین در مرحله استقرار راه رفتن در بیماران اسکولیوز نوجوانی ناشناخته و مقایسه آنها با نوجوانان سالم بود.
روش بررسی:
تعداد 19 دختر 11 تا 16 ساله مبتلا به اسکولیوز و 18 دختر سالم همگن با سن، قد و جرم مشابه مورد مطالعه قرار گرفتند. نیروهای عکس ­العمل زمین در جهتها­ی عمودی (Fz) (GRF force in Z axis) ، قدامی­ـخلفی (Fy) (GRF force in Y axis) ، و داخلی­ـخارجی (Fx) (GRF force in Z axis) و نیز جنبه ­های کینماتیکی بدن هنگام راه رفتن با استفاده از دو تخته نیروی Kistler ((1000 Hz  و یک دستگاه Vicon با 4 دوربین سری T، اندازه ­گیری شدند. پس از پردازش داد ه­ها و همگ ن­سازی آنها بر حسب وزن، متغیرهای اوج نیروهای عکس ­العمل زمین، نرخ بارگذاری، اندازه ضربه، متغیرهای زمانی نیروی عکس ­العمل و شاخص عدم تقارن در متغیرهای مختلف محاسبه گردیدند. اطلاعات با استفاده از آزمون آزمون آنالیز واریانس چند متغیره و آنالیز واریانس اندازه‌های تکراری در سطح معنی ­داری 0/05 پردازش شدند.
یافته ها:
در هر سه جهت عمودی، قدامی ـ خلفی و داخلی­ ـ خارجی، نیروی عکس ­العمل زمین پای راست بیماران اسکولیوز و افراد سالم مشابه بودند (0/05<p). در پای چپ نیز در جهات قدامی­ ـ خلفی و داخلی­ ـ خارجی، هر دو گروه نیروی عکس­العمل یکسانی داشتند (0/05<p) اما اولین اوج Fz در بیماران اسکولیوز کمتر از افراد سالم بود (0/03=p). اندازه ضربه، نرخ بارگذاری و زمان رسیدن به اوج نیروی عکس ­العمل نیز در دو گروه تفاوتی نداشتند (0/05<p). در همه متغیرها شاخص عدم­ تقارن بیماران اسکولیوز مشابه افراد سالم بود (0/05<p).
نتیجه‌گیری:
بیماری اسکولیوز با کاهش اولین اوج نیروی عکس­ العمل Fz در راه رفتن همراه است. بررسی تاثیر عوامل مختلف از جمله عملکرد عضلات اندام تحتانی می ­تواند در تشریح کم و کیف این افت نیروی عکس العمل مفید باشد. برای آسیب­ شناسی و درک تعامل این پدیده با بروز و پیشرفت انحنا باید مطالعات پیچیده ­تری انجام شوند.
