تاثیر یک جلسه فعالیت تناوبی شدید با و بدون محدودیت جریان خون بر فعالیت الکتریکی عضلات منتخب بازویی دختران فعال
هدف: ورزشکاران نخبه بسیاری از رشته های ورزشی به دنبال کسب توان بی هوازی در سطوح بالا هستند. در سال های اخیر روی روش های مختلف بهبود توان بی هوازی از طریق تمرین، اقدامات زیادی انجام شده است. از جمله سیستم هایی که در این حوزه وجود دارد سیستم محدودیت جریان خون (کاتسو) است و از آنجایی که الکترومایوگرافی، یک ابزار مفید برای بررسی فعالیت الکتریکی عضلات در حالت فعالیت و همچنین بررسی ویژگی های سنتیکی حرکت می باشد لذا پژوهش حاضر با هدف مطالعه، اثر یک جلسه فعالیت تناوبی شدید با و بدون محدودیت جریان خون بر فعالیت الکتریکی عضلات منتخب بازویی در دختران فعال انجام شد. روش بررسی: تعداد 10 نفر از دانشجویان خانم دانشگاه بیرجند با سن 0/81±22/15 سال، شاخص توده بدنی2/56± 20/08 کیلوگرم بر مترمربع انتخاب شدند. فعالیت الکتریکی عضلات دوسر وسه سربازو هنگام اجرای 4 ست 30 ثانیه ای با 5 دقیقه استراحت بین هر ست طی یک جلسه فعالیت تناوبی شدید با دستگاه وینگیت دست تحت شرایط بدون محدودیت و با محدودیت جریان خون اندازه گیری شد. علاوه بر فعالیت الکتریکی برخی از پارامترهای توان بی هوازی مانند میانگین و حداکثر توان بی هوازی نیز تحت دو شرایط مورد سنجش قرار گرفت. یافته ها: فعالیت الکتریکی عضله دوسربازو دست برتر(غالب) تحت شرایط با محدودیت جریان خون افزایش معناداری داشت (0/05>p)، اما در فعالیت الکتریکی عضله سه سربازو دست برتر تحت شرایط با محدودیت جریان خون افزایش معناداری مشاهده نشد (0/05<p). بین میانگین توان بی هوازی و حداکثر توان بی هوازی، محدودیت جریان خون تنها باعث افزایش معنادر حداکثر توان بی هوازی شد (0/05 >p). نتیجه گیری: نتایج نشان داد که فعالیت الکتریکی عضله دوسربازو و همچنین حداکثر توان بی هوازی به طور معناداری طی شرایط محدودیت جریان خون افزایش یافت (0/05>p) به نظر می رسد محدودیت جریان خون یک عامل موثردر افزایش حداکثر توان بی هوازی و فعالیت الکتریکی عضله دوسربازو است.

اثربخشی توانبخشی شناختی بر کارکردهای اجرایی و شدت اختلال ناروانی گفتار در کودکان مبتلا به اختلال ناروانی گفتار
هدف: : از شایع ترین اختلالات روانی کلام لکنت زبان ( که در پنجمین ویرایش راهنمای تشخیصی و آماری اختلالات روانپزشکیبه آن اختلال ناروانی گفتار کودکی گفته می شود) است. مطالعات زیادی پیرامون رابطه بین ابعاد کارکردهای اجرایی و روانی کلامصورت گرفته و رابطه آن با توجه پایدار، حافظه فعال، بازداری پاسخ سنجیده شده است. با توجه به رابطه بین کارکردهای اجرایی و اختلال ناروانی گفتار در پژوهش حاضر سعی شد با استفاده از توانبخشی شناختی و ارتقای کارکردهای اجرایی در سه حوزه توجه پایدار، حافظه فعال و بازداری پاسخ، به درمان اختلال ناروانی گفتار کودکی بپردازیم. روش بررسی:با استفاده از نمونه در دسترس، تعداد 20 کودک بین 7 تا 12 سال با اختلال ناروانی گفتار بر اساس ملاک های پژوهش، انتخاب و از آن ها آزمون (N-BACK) جهت سنجش حافظه فعال، آزمون عملکرد پیوسته جهت سنجش توجه پایدار و آزمون برو، نرو (go/no go) جهت سنجش بازداری پاسخ به عمل آمد. همچنین شدت اختلال ناروانی گفتار کودکان با استفاده از آزمون درصد هجاهای لکنت شده سنجیده شد. سپس آزمودنی ها به طور تصادفی در دو گروه آزمایش و گواه قرار گرفتند. گروه آزمایش 15 جلسه 45 دقیقه ای تحت درمان با روش توانبخشی شناختی قرار گرفتند. سپس از هر دو گروه پس آزمون گرفته شد. گروه آزمایش 3 ماه بعد تحت پیگیری قرار گرفتند و مجددا آزمون ها از آن ها گرفته شد. یافته ها: نتایج تحلیل کوواریانس و آزمون تی وابسته جهت بررسی اثربخشی روش درمان نشان داد که تفاوت معناداری بین توجه پایدار (0/022=p)، حافظه فعال (0/035=p) و کنترل پاسخ (0/039=p) و شدت اختلال ناروانی گفتار (0/013=p) در پیش آزمون و پس آزمون در کودکان مبتلا به اختلال در سیالی کلام وجود دارد. و رگه های معناداری در کارکردهای اجرایی (0/056=p) در پیش آزمون و پس آزمون مشاهده می شود. همچنین نتایج آزمون تعقیبی بونفرونی جهت بررسی پایداری درمان در طی دوره 3 ماهه نشان داد که روش درمان بر روی توجه پایدار، حافظه فعال و کارکردهای اجرایی و شدت لکنت موثر بوده و اثر آن در طول دوره پیگیری ثابت بوده است. همچنین روش درمانی توانبخشی شناختی بر روی کنترل پاسخ موثر بوده اما اثر آن در طول زمان ثابت نمانده است. نتیجه گیری: از یافته های پژوهش می توان نتیجه گرفت که روش توان بخشی شناختی در کاهش نرخ اختلال در سیالی کلام و افزایش کارکردهای اجرایی در کودکان مبتلا به اختلال ناروانی گفتار موثر بوده و میتواند به عنوان یک روش درمان مورد استفاده قرار گیرد.

ترسیم نقشه علمی تحولات نوین فناوری اطلاعات سلامت بر اساس مقالات WoS: 2017-2010
هدف: ترسیم نقشه علمی، نقش مهمی در کشف الگوهای پنهان حوزه های پژوهشی به ویژه حوزه های چند رشته ای و نوین دارد. هدف این مطالعه، شناسایی رویکردهای پژوهشی مهم در حوزه فناوری اطلاعات سلامت در جهان بوده، که از طریق ترسیم نقشه تحولات علمی سال های اخیر این حوزه انجام گرفته است. روش بررسی: این پژوهش، از نوع توصیفی بوده، که با روش علم سنجی انجام شده است. بدین منظور مطالعات و مقالات مرتبط با حوزه فناوری اطلاعات سلامت در جهان از پایگاه WoS بین سال های 2017-2010، به تعداد 5301 مقاله استخراج شد. سپس منابع بدست آمده در نرم افزار Histcite نسخه 12.03.17 وارد و مورد تجزیه و تحلیل قرار گرفت. یافته ها: پس از جمع بندی مقالات استخراجی، 6 خوشه حوزه فناوری اطلاعات سلامت در جهان شناسایی شد. خوشه اصلی و مقاله رابط کانونی در این حوزه در زمینه «هزینه–سودمندی سیستم های اطلاعاتی» به ویژه پرونده الکترونیکی سلامت است. خوشه های فرعی شامل: «موفقیت و شکست سیستم های اطلاعاتی و مشکلات پیاده سازی»، «چالش های فناوری اطلاعات سلامت در حوزه امنیت و محرمانگی و تاثیر آن بر اخلاق پزشکی»، «نقش فناوری اطلاعات سلامت در بهبود کیفیت خدمات سلامت»، «هم کنش پذیری در سطح پرونده الکترونیک سلامت» و «آینده فناوری اطلاعات سلامت و کاربردهای آن نظیر Telemedicine و Telehealth» است. نتیجه گیری: هزینه و سودمندی سیستم های اطلاعاتی حوزه اصلی پژوهش در زمینه فناوری اطلاعات سلامت بر اساس مقالات WoS در جهان بود. این مسئله با موفقیت و شکست سیستم های اطلاعاتی و توجه به کاربرد پذیری سیستم های اطلاعات سلامت ارتباط نزدیک داشته است.

تأثیر تمرینات اصلاحی با رویکرد جدید بر قدرت ایزومتریک افراد مبتلا به سندرم انحراف پرونیشن
هدف: سندرم انحراف پرونیشن یکی از شایع ترین ناهنجاری های بدنی است که موجب ناراستایی های در ساختارهای بخش کمری-لگنی و اندام تحتانی می شود. هدف این پژوهش، تعیین تاثیر تمرینات اصلاحی با رویکرد جدید بر قدرت ایزومتریک عضلات پایین تنه افراد مبتلا سندرم انحراف پرونیشن بود. روش بررسی: در این تحقیق نیمه تجربی، 30 داوطلب که سندرم انحراف پرونیشن داشتند را انتخاب و به صورت تصادفی و مساوی در دو گروه 15 نفری گروه کنترل (میانگین ± انحراف معیار، سن: 1/8±12/6 سال، قد: 9/6±166/4 سانتی متر، وزن: 9/9±68/6 کیلوگرم، شاخص توده بدن:2/2±24/7 کیلوگرم برمتر مربع) و تجربی (میانگین ± انحراف معیار، سن: 1/72±12/60 سال، قد: 6/58±165/33 سانتی متر، وزن: 5/72±67/12 کیلوگرم، شاخص توده بدن: 1/63±24/55 کیلوگرم برمتر مربع) تقسیم بندی شدند. گروه تجربی، تمرینات اصلاحی با رویکرد جدید را به مدت 12 هفته (3 جلسه در هفته به مدت یک ساعت) اما گروه کنترل، فعالیت های طبیعی و روزمره خود را اجرا کردند. قبل و بعد از 12 هفته اجرای تمرینات، قدرت ایزومتریک عضلات پایین تنه ( دینامومتر دیجیتال) مورد ارزیابی قرار گرفت. برای تجزیه و تحلیل داده ها آزمون آماری آنالیز واریانس با اندازه های تکراری به همراه آزمون تعقیبی بونفرونی در سطح خطای پنج درصد استفاده شد (0/05>p). یافته ها: گروه تجربی به دنبال12 هفته تمرینات اصلاحی با رویکرد جدید در قدرت ایزومتریک عضلات پایین تنه بهبودی معناداری پیدا کردند (0/001>p). اما در گروه کنترل تفاوت معناداری مشاهده نشد (0/05p>). به علاوه، پس از انجام مداخله، قدرت ایزومتریک عضلات پایین تنه در گروه تجربی به طور معناداری بیشتر از گروه کنترل بود (0/001>p) در حالی که این تفاوت در پیش آزمون معنادار نبود (0/05<p). نتیجه گیری: به نظر می رسد که تمرینات اصلاحی با رویکرد جدید سبب بهبودی در قدرت ایزومتریک عضلات پایین تنه در افراد مبتلا به سندرم انحراف پرونیشن می شود، لذا برای این دسته از افراد توصیه می شود.

بررسی تاثیر تعامل مناسب معلم بر دانشآموز دارای لکنت: یک مطالعه تک عضوی
هدف: یکی ازشایع ترین اختلالات گفتار، ناروانی گفتار است که ممکن است به شکل ناروانی طبیعی و یا ناروانی غیرطبیعی مانند لکنت ظاهر شود. یکی از رویکردهایی که برای کودکان مبتلا به لکنتِ مدرسهرو ممکن است مفید باشد، چگونگی تعامل معلم با کودک است. هدف این مطالعه بررسی اثر تعامل مناسب کودک و معلم بر لکنت دانشآموز است. روش بررسی: این پژوهش یک مطالعه تک عضوی با طرح AB درباره تاثیر تعامل مناسب معلم بر شدت لکنت، نگرش نسبت به لکنت و کفایت ارتباطی در کلاس درس در یک دانش آموز دارای لکنت میباشد که در آن از درصد هجاهای لکنت شده (Percent of Stuttered Syllables; SS%)، ابزار سنجش شدت لکنت- 4 ( Stuttering Severity Instrument-fourth edition; SSI4)، آزمون نگرش ارتباطی (The Communicative Attitude Test; CAT) و آزمون ارزیابی معلم از کفایت ارتباطی دانش آموز (The Teacher Assessment of Student Communicative Competence; TASCC) استفاده شده است. پس از یک دوره 6 جلسهای گفتاردرمانی، مداخله مثبت معلم همراه با گفتاردرمانی به مدت 6 جلسه صورت گرفت. یک جلسه پیگیری نیز انجام شد. یافته ها: این مطالعه نشان داد که پس از مداخله مثبت معلم همراه با گفتاردرمانی، شدت لکنت دانشآموز از متوسط به خفیف رسید و نگرش وی نسبت به لکنت نیز از منفی به متوسط تغییر کرد. نتایج آزمون TASCC به صورت محسوسی طی درمان افزایش یافت. این نتایج در جلسه پیگیری حفظ شد. نتیجه گیری: نتایج این پژوهش نشان داد که استفاده از راهکارهای کمک کننده برای مدیریت کلاسی که دانشآموز مبتلا به لکنت در آن حضور دارد، در کاهش رفتارهای اولیه و ثانویه، و بهبود نگرش دانشآموز دارای لکنت موثر است. شواهد بیشتر به مطالعات گستردهتری نیاز دارد.

بررسی مقایسه ای شاخص های تنفسی درافراد مبتلا به اختلال اضطراب فراگیر و افراد سالم
هدف: اختلال اضطراب فراگیر (Generalized Anxiety Disorder; GAD) یکی از شایع ترین اختلالات روحی-روانی است. تا کنون مطالعه ای به بررسی شاخص های تنفسی آن ها نپرداخته است. هدف از مطالعه حاضر، بررسی مقایسه ای شاخص های الگوی تنفس افراد سالم و افراد مبتلا به اختلال اضطراب فراگیر می باشد. روش بررسی: در این مطالعه مقطعی 16 فرد مبتلا به اختلال اضطراب فراگیر با 16 فرد سالم که در ویژگی های دموگرافیک با هم همسان شده بودند؛ مقایسه شدند. شاخص های تنفسی مورد بررسی شامل فشار دی اکسید کربن بازدمی و نرخ تنفس که با استفاده از دستگاه کپنوگراف به دست آمد، بازدم سریع در ثانیه اول (Forced Expiratory Volume in First Second ;FEV1) و ظرفیت حیاتی پرفشار (Forced Vital Capacity ; FVC) و نسبت FEV1 به FVC داده های اسپیرومتری بودند و پرسشنامه تنفسی نیمگن جهت ارزیابی هایپرونتیلاسیون به کار رفت. یافته ها: بیماران اضطرابی FEV1 و درصد FEV1 به FVC کمتری درمقایسه با افراد سالم داشتند. میزان دی اکسیدکربن انتهای بازدمی و نرخ تنفس اختلاف معنی داری بین دو گروه داشتند. شاخص دی اکسیدکربن بازدمی در مبتلایان به اختلال اضطراب فراگیر نسبت به افراد سالم کاهش و در مقابل شاخص تعداد تنفس در این گروه افزایش یافت. نمره پرسشنامه نیمگن در گروه بیماران اضطرابی بـه طـور معنی داری بالاتر از افراد سالم بود. نتیجه گیری: نتایج این مطالعه نشان داد الگوی تنفسی افراد مبتلا به اختلال اضطراب فراگیر متفاوت از افراد سالم و هایپرونتیلاسیون است. بنابراین بررسی عملکرد سیستم تنفس افراد مبتلا می تواند در روند زود هنگام تشخیص و درمان مسایل تنفسی و ارجاع آنان به مراکز فیزیوتراپی موثر باشد.

هدف: الکتروکوکلئوگرافی یکی از آزمون های الکتروفیزیولوژیکی است که به منظور تشخیص بیماری منیر مورد استفاده قرار میگیرد. هدف از انجام مطالعه حاضر بررسی نظرات اعضا انجمن شنواییشناسان ایران در رابطه با استفاده بالینی از الکتروکوکلئوگرافی در زمینه تشخیص و درمان بیماری منیر است. روش بررسی: در این مطالعه به منظور جمعآوری اطلاعات از یک نظرسنجی 16 آیتمی استفاده شد. نظرسنجی مورد نظر برای 600 نفر از اعضاء انجمن شنواییشناسان ایران از طریق مصاحبه رودررو توزیع شد. در مجموع 267 نفر از افراد به طور کامل به نظرسنجی مورد نظر پاسخ دادند. یافته ها: در موارد مشکوک به منیر یک طرفه، 71% از شرکت کنندگان در مطالعه حاضر به طور معمول از الکتروکوکلئوگرافی استفاده می کردند.برای ثبت پاسخ، غالب افراد شرکت کننده از محرک کلیک و الکترودهای کانال استفاده می کردند. به باور 32% از افراد شرکت کننده قبل از هرگونه مداخله درمانی تهاجمی حتما باید نتایج الکتروکوکلئوگرافی ناهنجار باشد. غالب افراد (58%) بر این باور بودند که نتایج الکتروکوکلئوگرافی با توجه به وضعیت بیماری تغییر می یابد، اما تنها 32% معتقد بودند که برگشت نتایج الکتروکوکلئوگرافی به حالت هنجار نشان دهنده بهبود بیماری است. تنها 7% از افراد گزارش کردند که به دلیل تغییرپذیری زیاد نتایج الکتروکوکلئوگرافی و عدم ارتباط مناسب آن با علائم بیماری، آزمون را انجام نمی دهند. نتیجه گیری: با توجه به نتایج حاصله اکثریت شنواییشناسان شرکت کننده در مطالعه حاضر از آزمون الکتروکوکلئوگرافی برای تشخیص و مدیریت بیماری منیر استفاده میکنند و عمده این افراد معتقد هستند که می توان از تاریخچه گیری در کنار آزمون الکتروکوکلئوگرافی برای کمک به تشخیص بیماری استفاده نمود. همچنین بر اساس نظر غالب افراد شرکت کننده در مطالعه، نبود دورههای آموزشی کافی میتواند منجر به محدود شدن کاربرد بالینی آزمون شود.

تاثیر تقویت عضلات دور کننده و چرخاننده خارجی هیپ بر وضعیت لگن در صفحه فرونتال در مردان دارای ولگوس پویای زانو طی حرکت فرود تک پا
هدف: افتادگی لگن در سمت مخالف پای تکیه گاه، طبق نظریات بیانگر ضعف عضلات دورکننده و چرخاننده خارجی هیپ می باشد. این ضعف با ایجاد نقص در انتقال نیرو در بدن میتواند ریسک بروز آسیب های اندام تحتانی را افزایش دهد. علیرغم وجود ارتباط تاکنون تحقیقات محدودی در زمینه تاثیر تقویت این عضلات بر وضعیت لگن انجام شده است. لذا هدف از تحقیق حاضر بررسی تقویت عضلات دور کننده و چرخاننده خارجی هیپ بر وضعیت لگن در صفحه فرونتال طی حرکت فرود تک پا می باشد. روش بررسی: سی دو مرد دارای ولگوس زانو بیش از 8 درجه به صورت در دسترس و هدفمند انتخاب و به طور تصادفی در دو گروه تجربی (16 نفر، سن: 1/82±22/35 سال، قد: 0/07±1/78 متر، وزن: 11/6±70/92 کیلوگرم) و کنترل (16 نفر، سن: 1/79±22/66 سال، قد: 0/05±1/8 متر، وزن: 11/61±71/011 کیلوگرم) قرار گرفتند. پس از انجام پیش آزمون، گروه تجربی سه جلسه در هفته (هر جلسه 45-60 دقیقه) و به مدت هشت هفته تمرینات تقویتی عضلات دورکننده و چرخاننده خارجی هیپ را به صورت دو طرفه و گروه کنترل فعالیت های معمول روزانه خود را انجام دادند و در انتها از هر دو گروه پس آزمون گرفته شد. در مرحله پیش و پس آزمون، قدرت عضلات هیپ با استفاده از دینامومتر ایزوکنتیک (کین کام) و اطلاعات کینماتیک طی سه حرکت پرش و فرود ثبت و وضعیت لگن در صفحه فرونتال با استفاده از نرم افزارهای QTM و Visual 3D محاسبه شد. داده ها با استفاده از آزمون تحلیل واریانس با اندازههای تکراری در سطح معناداری (05/0p <) توسط نرم افزار SPSS مورد تجزیه و تحلیل قرار گرفتند. یافته ها: یافته های پژوهش حاضر نشان داد که اثر متقابل گروه آزمایشی و زمان اندازهگیری برای قدرت کانسنتریک و اکسنتریک عضلات دور کننده (0/001>p <0/001 ،p) و چرخاننده خارجی هیپ (0/02=p <0/001 ،p) و حداکثر زاویه بالا رفتن لگن (0/047=p) و دامنه پایین افتادن لگن (0/005=p) در صفحه فرونتال معنیدار بود. نتایج آزمون تعقیبی نشان داد که در گروه تجربی میانگین نمرات قدرت کانسنتریک و اکسنتریک عضلات دور کننده (0/001>p=0/004 ،p) و چرخاننده خارجی هیپ (0/001>p=0/01،p) در پی هشت هفته تمرینات افزایش و زاویه بالا رفتن لگن (0/04 =p) و دامنه پایین افتادن آن (0/008=p) نیز کاهش یافته است. اما در گروه کنترل تغییر معنیداری مشاهده نشد (0/05<p). نتیجه گیری: هشت هفته تمرینات تقویتی عضلات دور کننده و چرخاننده خارجی هیپ میتواند در بهبود قدرت کانسنتریک و اکسنتریک عضلات دورکننده و چرخاننده خارجی و وضعیت لگن در صفحه فرونتال هنگام فرود تک پا موثر باشد..

بررسی ساختار و عملکرد ارتوزهای قدرتی دستکش مانند مورد استفاده جهت توانبخشی دست بیماران با ضعف یا فلج عضلات دست: مطالعه مرور نظام مند
هدف: با توجه به ویژگی های متعددی که ارتوز های قدرتی دارند این وسایل می توانند در توانبخشی و بهبود شرایط افراد با مشکلات حرکتی دست کمک کننده باشند. هدف این پژوهش، مروری بر ساختار و عملکرد ارتوزهای قدرتی دستکش مانندی است که تا به امروز به منظور تمرین، کمک و بهبود عملکرد در بیماران با ضعف یا فلج عضلات دست طراحی شده اند. روش بررسی: مطالعات در پایگاه های اطلاعاتی Pubmed، Scopus ، ISI web of sciences و IEEE دربازه زمانی سال های 2000 تا 2019 مورد جستجو قرار گرفتند. کلید واژه های مورد استفاده جهت جستجو براساس استراتژی PICO انتخاب گردیدند. بعد از ارزیابی مقالات، در نهایت 12 مقاله انتخاب شدند. معیارهای مدنظر برای ورود مطالعات شامل این موارد بود؛ طراحی، ساخت ارتوز قدرتی دستکش مانند، استفاده از ارتوز برای درمان، توانبخشی و بهبود عملکرد دست، استفاده برای افرادی که دچار ضعف و یا فلج در عضلات دست در اثر مشکلات سیستم اعصاب مرکزی بودند. یافته ها: بررسی مطالعات نشان داد که از 12 ارتوز معرفی شده در این مطالعه 10 ارتوز از موتورهای الکتریکی برای ایجاد نیروی محرک استفاده می کردند و 2 ارتوز از سیستم پنوماتیک سود می بردند. در این بین اکثر این ارتوزها از سیستم ها انتقال نیروی کابلی استفاده می کنند. آنچه باعث تفاوت بیشتر در این ارتوزها می گردد سیستم تولید فرمان است که از روش های چون سیگنال موقعیت، سگینال الکتریکی عضله و سیستم های نرم افزاری می توان نام ببرد. نتیجه گیری: ارتوزهای موجود از نظر سیستم انتقال نیرو، سیستم محرک و سیستم تولید فرمان دارای گوناگونی ها فراوانی هستند و این امر باعث شده هر یک از این وسایل قابلیت های متفاوتی برای کمک به بیماران داشته باشند. اگرچه هر یک از طرح های ارتوزی معرفی شده دارای مزایایی جهت برآورده سازی نیازهای جامعه هدف خود هستند اما خالی از محدودیت نیز نمی باشند. رفع محدودیت های این طرح ها می تواند در بالا بردن کارایی و تامین بهتر نیازهای افرادی که از این ارتوزها استفاده می نمایند نقش موثری داشته باشد.

بررسی کینماتیک اندام تحتانی هنگام بالا رفتن و پایین آمدن از پله در افراد مبتلا به سندروم درد پاتلوفمورال: مقاله مروری
هدف: سندروم درد پاتلوفمورال (PFPS ؛Patellofemoral Pain) یکی از رایجترین دلایل زانو درد معرفی شده است، که علت آن ناشناخته مانده و عوامل بیومکانیکی متعددی برای آن، گزارش شده است. از آنجایی که بیماران 5 الی 8 سال پس از درمان، درد و محدودیت عملکرد را گزارش میدهند، بنابراین، بررسی کینماتیک اندام تحتانی برای تعیین مکانیسمهای جبرانی و اثربخشی بهتر مداخلات درمانی امری ضروری به نظر میرسد. هدف از مطالعه مروری حاضر، بررسی کینماتیک اندام تحتانی هنگام بالارفتن و پایین آمدن از پله در افراد مبتلا به PFPS است. روش بررسی: در این مطالعه، جستجوی مقالات از بانکهای اطلاعاتی PubMed، Google Scholar،Elsevier ، Springer آغاز شد و مقالات مربوط به سال، 2000 تا 2018 میلادی استخراج شدند. برای استخراج مقالات از کلید واژهای: PFPS، Hip adduction ، Rear foot eversion ، Flexion knee استفاده شد؛ که در نهایت 1200 مقاله یافت شد. پس از بررسی چکیده و عنوان مقالات،240 مقاله برای بررسی بعدی انتخاب شدند، سپس با توجه به موضوع اصلی مطالعه حاضر، و معیارهای ورود و خروج، 10 مقاله برای بررسی نهایی انتخاب شدند. یافته ها: 10 مقاله استخراج شده، کینماتیک سه بخش پرگزیمال، لوکال، دیستال را طی پایین آمدن از پله در بیماران PFPS بررسی کردند. نتیجه گیری: با توجه به نتایج مطالعات، تغییرات کینماتیکی اندام تحتانی در هنگام بالارفتن و پایین آمدن از پله، در نهایت به افزایش نیروی عکسالعمل و فشار مفصل پاتلوفمورال منجر میشود، که از دلایل اصلی توسعه و ایجاد PFPS است. بنابراین مداخلات توانبخشی که بصورت جامع تغییرات کینماتیکی ایجاد شده را مورد توجه قرار دهند از اثربخشی بیشتری در بهبود علائم بالینی برای مدت زمان طولانی برخوردار خواهند بود.
