اثر دو برنامه‌ی کششی‎ PNF ‎و تقویتی اندام تحتانی بر درد و حس عمقی‎ ‎مفصل زانو قبل و بعد از خستگی موضعی ‏عضله‌ی چهارسر رانی در مردان ورزشکار مبتلا به درد کشککی-رانی

چکیده

هدف:
خستگی عضلات چهارسر رانی موجب درد و کاهش حس­ عمقی زانو می­ شود. هدف تحقیق اثر دو برنامه ­ی کششی PNF و تقویتی اندام تحتانی بر درد و حس عمقی مفصل زانو قبل و بعد از خستگی موضعی عضله­ ی چهارسررانی در مردان ورزشکار مبتلا به سندرم درد کشککی_رانی (Patellofemoral Pain Syndrome; PFPS) بود.
روش بررسی:
این پژوهش نیمه­ تجربی و از نوع پیش­ آزمون و پس ­آزمون بود. جامعه آماری شامل مردان 30-20 ساله PFPS شهر تهران بود. 24 نفر به روش هدفمند و در دسترس انتخاب و تصادفی در دو گروه تمرینات کششی تسهیل عصبی-عضلانی حس­ عمقی (Proprioceptire Neuromuscular Fasilitating; PNF) (12 نفر) و تقویتی (12 نفر) قرار گرفتند. هر گروه هشت هفته و هر هفته سه­ جلسه 60 دقیقه­ ای تمرینات خود را انجام دادند. قبل و بعد از تمرینات (در هر نوبت تست قبل و بعد از خستگی)، درد و حس عمقی زانو به ترتیب با مقیاس اندازه ­گیری بصری درد (Visual Analog Scale; VAS) و دستگاه ایزوکنتیک بایودکس (مدل IV Pro ساخت آمریکا) اندازه ­گیری شد. تحلیل داده­ با آزمون سنجش مکرر ترکیبی با معنی ­داری (0/05>p) بود.
یافته‌ ها:
اثر خالص تمرین بر درد معنی ­دار بود (0/5=ES و 0/05>p و 4/417=(22و1)F). در مورد مقایسه آزمون­ اول و سوم (قبل خستگی) (0/8=ES و 0/05>p و 148/4=(22و1)F) و مقایسه آزمون­ دوم و چهارم بعد خستگی تفاوت معنی­ دار بود (0/4=ES و 0/002>p و 52/14=(22و1)F). کاهش درد بعد تمرین در گروه کششی 29 درصد و تقویتی 18 درصد بود. اثر خالص تمرین بر بهبود حس­ عمقی معنی ­دار بود (0/6=ES و 0/005>p و 2/404=(22و1)F). در خصوص مقایسه آزمون­ اول و سوم (قبل خستگی) (0/6=ES و 0/001>p و 1058/2=(22و1)F) و مقایسه آزمون­ دوم و چهارم (بعد خستگی) تفاوت معنی­ دار بود (0/5=ES و 0/005>p و 51/1=(22و1)F). بهبود حس عمقی بعد تمرین در گروه کششی 15 درصد و در تقویتی 11 درصد بود. تمرینات در گروه کششی PNF  بر هر دو متغیر در پس­ آزمون و بعد از دوره تمرین موثرتر بود. 
نتیجه ‌گیری:
هر دو برنامه بر فاکتور درد و حس عمقی موثر بوده، اما تمرینات کششیPNF  بر کاهش میزان درد و بهبود حس عمقی بیماران موثرتر بود.
